The authors present the case history of a patient in whom an optical keratoprosthesis was implanted after recurrent corneal ulcers in aphakia at the age of 80. The globe was examined histologically post mortem. A solid retroprosthetic membrane, a hemorrhage in the vitreous and retinal detachment after trauma were found to be the causes of the visual disorder. Wound healing following implantation of a keratoprosthesis is discussed in the light of the histopathological findings.

A comparison between 100 percent correct response thresholds of stereopsis for nine subjects measured with and without viewing using a 33 percent difference in ocular image magnification is presented. Two subjects performed threshold settings, using the method of adjustments, with 1, 5, 10, 15, and 33 percent magnification differences. The extent of the increase explains reports of stereopsis in corrected unilateral aphakia.

In order to evaluate the efficacy and safety of intraocular lens (IOL) implantation in paediatric aphakia, we reviewed the records of a consecutive series of 40 children, under 15 years of age, who had IOLs inserted for congenital and developmental cataracts between September 1990 and December 1992. A modified Yamane technique with a 26-gauge needle for aphakia correction is described. Single corneal or corneoscleral incision is set at 12 o'clock. The 26-gauge needles are used to make 2 angled incisions parallel to the limbus to fix the haptics of a 3-piece intraocular lens. The modifications provide an easier and safer way of haptic insertion into the needle tip and decreases the possibility of bending or breaking the haptic.

PURPOSE OF REVIEW: Modern pediatric cataract surgical techniques combined with a greater understanding of the natural history of aphakia and pseudophakia have changed the approach to the surgery of pediatric cataracts.
RECENT FINDINGS: Advanced surgical techniques, new pharmacologic options and long-term refractive planning have improved surgical success.
SUMMARY: It is essential that the ophthalmic surgeon who cares for children with cataracts is aware of these issues.

Congenital anomalies of the lens include a wide range from lens coloboma to primary aphakia and doubling of lens. There have been few case reports of double lens; the etiology suggested is metaplastic changes in the surface ectoderm that leads to formation of two lens vesicles and hence resulting in double lens. We report a case with bilobed lens, which raises the possibility of explaining the etiology of double lens.

The possibility of YAG laser interventions on the posterior capsule of the lens with subsequent argon laser coagulation of the retina in patients with aphakia and artiphakia is studied. The optimal intervention for secondary cataract in patients with artiphakia is circular resection of the posterior capsule. Intraocular lenses prevent such complications as vitreous hernias and allow creating a larger and better shaped opening in the posterior chamber, thus providing a better access for laser coagulation of the retina.

Intraocular lens implants are being used in ever increasing numbers for the correction of aphakia. For that reason optometrists should familiarize themselves with the types of implants used and the advantages and disadvantages of each. This article will review the types of implants the optometrist is likely to see. The changing trends in implant surgery will be considered along with explanations for the changes. Several studies of patients with implants are reviewed. Finally, a format for evaluating implant patients is presented.

The indications and the adaptation method of a combined contactoptical system called piggy-back system, are dealt with. This system comprises the adaptation of a soft contact lens on which is applied afterwards, a hard one. The optical and the therapeutic results obtained in 7 groups of patients (56 eyes) with astigmatism, aphakia, keratokonus, keratoplasty, injuries, chronic lesions of the epithelium, and hyposecretion of the tear fluid, are discussed together with the results of the adaptation of the lenses.

We studied 11 infants and young children fitted with contact lenses for correction of unilateral or bilateral aphakia. The visual-acuity estimates obtained from behavioral cues (alternate-cover test and central-fixation monitoring) were compared with results obtained from pattern-reversal visual-evoked potentials (VEP). We conclude that VEP may be more sensitive than behavioral observation for monitoring the visual progress in certain aphakic children.
