The outcome of intracapsular cataract extraction seen from the patients' point of view and patients' statements of visual function were obtained by questionnaires to 120 patients one year after the operation. Comparison to visual acuity at discharge after surgery was done. The results indicate that today cataract extraction is very successful in improving vision, visual function and quality of life. The utilization of the visual improvement, however, is not always optimal; e.g. 39% of those patients, who according to visual acuity at discharge should have normal reading capacities, do not or c

A survey is given of 26443 cataract operations at the University Eye Hospital of Graz in the years 1950-1980. Different phases are evident: 1. Extraction with a forceps. 2. A phase of expression. 3. Extraction by means of an erysiphake. 4. Extraction with a cryo-extractor. Enzymatic zonulolysis is of particular importance and performed with trypsin only. Almost all cataract operations are performed by enzymatic zonulolysis with trypsin. Details of the operation technique are pointed out. The connections between cataract surgery and increased intraocular pressure, vitreous body complications an

Four female patients with known unplanned filtering blebs following cataract surgery developed endophthalmitis, two weeks to 30 months after being fit with corneal contact lenses. Three patients were wearing hard contact lenses and one wore a soft lens. Significant blepharoconjunctivitis was present and treated in two patients, but intraocular infection was not averted. When endophthalmitis was diagnosed, all patients had clinical evidence of a bleb infection as well as hypopyon. Despite aggressive medical therapy, two eyes required enucleation while two eyes survived with good vision. We reco

It was the purpose of this study to determine whether age could be used to predict central endothelial cell count in preoperative cataract patients aged 40 to 90. Three separate series of cell counts on a total of almost 3,000 eyes were performed and analyzed regarding age relationship. Although statistically endothelial cell count appears to decrease with age, the decrease is so small that a large range of cell counts was noted for each age. The results of this study indicate that age is not useful in estimating endothelial cell count in this population.

Fifty-seven eyes with extracapsular surgery and Copeland lenses and the same number of intracapsular cataract extractions without lenses are described. All properly chosen lens cases have 6/12 vision or better. Forty percent required capsulotomy because of 6/12 vision or less. Possible cystoid macular edema occurred in two cases. No endothelial changes were present that were not present preoperatively. Five lenses dislocated. Two glaucomas appeared. There was no retinal separation, endophthalmitis, or lens removal for lens-induced irritation.

A prospective fluorescein angiographic study, comparing the incidence of cystoid macular edema in patients 67 years or older after intracapsular cataract extraction with a Binkhorst 4-loop implant, extracapsular cataract extraction with a Binkhorst 2-loop or 4-loop implant, and intracapsular cataract extraction with no implant (controls) was begun at the Bascom Palmer Eye Institute of the University of Miami School of Medicine on May 14, 1974. Excluded from these groups were all patients whom we did not consider to be suitable candidates for lens implant surgery. Fundus fluorescein angiograms

Complications associated with rigid contact lenses are more frequent in aphakia. The limited dexterity of older patients not only may prove an insurmountable barrier to the mastery of daily contact lens insertion and removal, but also may produce significant direct eye trauma, its seriousness determined by the integrity of the cataract wound. Since prolonged wear of aphakic contact lenses is the rule rather than the exception, the risk of irreversible corneal changes such as vascularization and erosions is greater. A poorly fitted aphakic contact lens that does not permit adequate oxygenation

While prolonged wear of contact lenses is possible for the aphakic patient, careful consideration must be given to patient selection and to the potential complications of such use. Currently available lenses are far from ideal and, if used at all, must be carefully monitored. The newer lenses, specifically designed for extended wear, more closely fit our needs. Several manufacturers are testing their devices under strict FDA supervision. FDA guidelines dictate a minimum of two years follow-up before a determination will be made as to which lenses are safe and effective enough for prolonged wea

BACKGROUND/AIM: To assess the refractive outcomes of secondary intraocular lenses (IOL) in patients with traumatic aphakic eyes with corneal penetrating injury and compare different corneal curvature measurement methods.
METHODS: Patients with unilateral penetrating eye injuries underwent corneal wound repair and cataract extraction, followed by secondary IOL implantation. Corneal curvature measurements were taken on the contralateral healthy eye (Group A), from the affected eye before removing corneal sutures (Group B), or after suture removal (Group C). The refractive outcomes were compared

PURPOSE: To report clinical experience with a modified toric plate-haptic lens (VisTor; Hanita Lenses) also suitable for four-point scleral fixation in eyes without capsular support.
METHODS: The commercial VisTor lens design was modified by adding two positioning holes in the lens haptic to facilitate four-point fixation. The lens axis was marked preoperatively, and the insertion points of the sutures were marked 2 mm from the limbus and 2 mm on each side of the toric marks. Polypropylene 6-0 or 7-0 sutures were inserted through the sclera and threaded through the designated holes in the int

IMPORTANCE: The optimal timing for secondary intraocular lens (IOL) implantation in children with bilateral aphakia remains uncertain, with important implications for long-term visual outcomes and the risk of complications such as glaucoma. Determining when to implant to achieve the best visual outcomes while minimizing complications is critical for improving clinical decision-making in pediatric cataract management.
OBJECTIVE: To report the visual acuity (VA) and complications in children with bilateral aphakia after lensectomy for pediatric cataracts, undergoing secondary IOL implantation a

PURPOSE: To develop a new, sutureless intrascleral intraocular lens (IOL) fixation technique based on the Yamane double-needle and Canabrava 4-flanged techniques.
METHODS: The Fusion-Flanged technique involves using a 7-0 polypropylene suture loop threaded through the eyelets of the IOL. Afterward, both ends of the suture are passed outside the eye through a 27-gauge needle. This technique creates a single flange by joining the 2 suture ends through handheld cautery, and this flange is then embedded into the scleral tunnel to securely fix the IOL in place.
RESULTS: This technique was perform

Introduction: We previously reported a case of a patient with bilateral congenital cataract identified in the Ogasawara village, Bonin Islands, Japan, on a visit by an ophthalmologist and describe its course over 17 years from initial surgery. Here, we report on a secondary intraocular lens (IOL) implantation that was subsequently performed at 22 years and 5 months of age. Methods: After cataract surgery at 7 months, the aphakic patient began amblyopia treatment using extended-wear soft contact lenses (SCLs). After 10 years of age, SCLs were chosen to achieve crossed monovision considering the

We assessed the 10-year postoperative outcomes of pediatric cataract patients who underwent surgery at the age of 6 years or younger. A retrospective review of medical charts was conducted for 457 eyes of 277 patients, with the age at surgery averaging 1.3 ± 1.5 years (mean ± SD) and the follow-up duration averaging 12.8 ± 2.4 years (ranging from 10 to 17 years). The cohort included 250 eyes of 125 cases with bilateral aphakia (age at surgery 0.5 ± 0.8 years), 110 eyes of 55 cases with bilateral pseudophakia (1.9 ± 1.6 years), 42 cases with unilateral aphakia (1.1 ± 1.3 years), and 55 cases wi

We present a literature review of surgical techniques of intraocular lens placement in eyes with insufficient capsular support, focusing on the most recent publications, together with a retrospective multicentre consecutive case series analysis of 103 eyes undergoing pars plana vitrectomy and sutureless intrascleral (SIS) fixation of a standard three-piece PCIOL. Many different approaches appear in the literature without any specific procedure achieving superior outcomes. Advantages and disadvantages vary between techniques. Common complications related to IOL fixation techniques were as follo

In order to evaluate conditions for secondary intraocular lens (IOL) implantation detailed analysis of ultrasound biomicroscopy (UBM) images of 26 patients (33 eyes) with aphakia was performed. The etiology of cataract was congenital (12 eyes), traumatic (9 eyes), senile (8 eyes) and uveal (4 eyes). The results showed that UBM provides an opportunity to evaluate the implantation site considering capsule integrity, configuration and parameters of ciliary line. The authors have demonstrated that UBM is scarcely the only visualization method in aphakic eyes that lets find and evaluate structural
