In 5-15% of individuals suffering essential blepharospasm, treatment with botulinum toxin is insufficient. In these cases the following measures can be applied: increase the dose, increase or vary the injection sites, change the botulinum toxin medication, use additional oral medication, or perform surgery. Certain measures can also be carried out by the patient. In cases of hemifacial spasm, inadequate results of botulinum toxin injections are even more unusual. But for these cases, additional oral medication or the neurosurgical Janetta operation can be used.

Three cases of Meige syndrome with severe bilateral facial spasm were reported. All patients suffered from abnormal facial movement characterized by blepharospasm and oromandibular dystonia, and had atrophic changes of bilateral basal ganglia which were recognized by brain CT scan. Bilateral facial nerve block at the foramen stylomastoideum was applied to those three patients. Excellent results were obtained by this block technique in all the patients. The technique is not so sophisticated and is recommended for the treatment of the patient with Meige's syndrome.

A 35-year-old female with a history of Hashimoto's thyroiditis (HT) presented with episodic dysphasia and a burst of axial jerks with abdominal muscle contraction and asymmetric blepharospasm as manifestations of encephalopathy associated with HT. Brain magnetic resonance imaging (MRI) showed scattered supratentorial white matter hyperintense T2 foci. Brain single photon emission computerized tomography (SPECT) demonstrated a cortical hypoperfusion in the upper frontal and parietal areas. Four patients of thalamic strokes with different symptoms are reported. The first had thalamic haemorrhage and developed delayed blepharospasm. The second patient had occlusion of posterior cerebral artery causing infarction of lateral thalamus and occipital lobes. We aimed to study the attitude of Italian neurologists in the use of conventional MRI in patients with idiopathic adult-onset focal dystonia.
