Twelve patients underwent successful tibiotalocalcaneal arthrodesis using a locked intramedullary nail with posterior moldable bone grafting. Two of the patients developed a nondisplaced stress fracture around the proximal interlocking screws, which responded to immobilization at 8 and 15 months after surgery. In 7 of 12 patients, > or =2 mm of cortical hypertrophy was noted between the proximal interlocking screws along the medial and/or lateral aspect of the tibia. Although intramedullary nailing can achieve satisfactory tibiotalocalcaneal arthrodesis, the stress risers in the tibial diaphys

PURPOSE: To assess the impact of the intramedullary canal occupying ratio (ICOR) on bone union and the stability of fixation in intertrochanteric fracture treatment using proximal femoral nail anti-rotation (PFNA) surgery.
METHODS: A retrospective analysis included 151 patients aged 60 years and above who underwent PFNA for unilateral intertrochanteric fractures between June 2020 and 2022. Patients were categorised into 3 groups based on ICOR percentiles (0.48-0.93). Bone healing was assessed using the Radiographic Union Score for Hip (RUSH) at 2, 6, and 12 weeks postoperatively. Fixation sta

Constriction band syndrome (CBS) may be rarely associated with pseudarthrosis of tibia. Published literature includes only case reports. We evaluated the outcome of children with frank pseudarthrosis of the tibia with CBS and compared our results with other reported cases. We aimed to formulate a clinical classification of tibial involvement in CBS on the basis of the treatment guidelines. Seven patients with the presence of one or more constriction bands and radiological signs of tibial pseudarthrosis were included in the study. Eight reported cases of CBS with tibial involvement with managem

BACKGROUND: Depending on the size of the proximal bone segment, either a standard locking construct or a recon locking construct can be used in intramedullary nailing for a subtrochanteric fracture. However, the most appropriate construct for a given size of proximal bone segment has not yet been determined. Therefore, this study aimed to identify the appropriate construct using biomechanical testing.
METHODS: Fourteen intramedullary nails for each locking construct (standard and recon) were inserted into 28 synthetic femurs. Fourteen subtrochanteric fracture models were developed by creating

Antegrade intramedullary nailing has become the gold standard to treat two-, three-, and four-part cephalo-tuberosity fractures. We report a retrospective study of 6 patients who have undergone centro-medullary nailing since January 2012. Mean follow-up was 12 months; the average age was 57 years. Clinical evaluation was based on gross and weighted Constant and Murley score, based on the age and sex, compared to the "normal" body surface area. Radiological assessment allowed us to evaluate bone healing, the occurrence of osteonecrosis of femoral head or of post-traumatic arthrosis. Radiologica

The author's modification of thumb reconstruction using the big and second toe elements is presented. Two separate parts were raised on one common pedicle to form future thumb. The main element including bones, tendons and soft tissues were taken from the second toe. The flap comprising nail radix, matrix and lateral fragment of the second toe pulp was prepared and left in situ. Similar flap was raised on the hallux. Winding of the hallux flap round the second toe part allowed to form nearly normal thumb. The nail radix and matrix were harvested subperiosteally without any bone fragments of di

Onychomycosis is a common nail infection caused by dermatophytes, while non-dermatophytes including Aspergillus spp. are causes of nail onychomycosis. Aspergillus niger is not common as a cause of nail onychomycosis. In the current study we present a 60-year-old woman with subungual onychomycosis due to Aspergillus niger mimicking a glomus tumor. Physical examination revealed right thumb had a black color of nail bed. Localized tenderness and severe pain were observed. However, the cold sensitivity test, Loves pin test and Hildreths test were negative. On radiograph, bone erosion was found in

INTRODUCTION: Intramedullary nails are frequently used for treatment of unstable distal tibia fractures. However, insufficient fixation of the distal fragment could result in delayed healing, malunion or nonunion. Recently, a novel concept for angular stable nailing was developed that maintains the principle of relative stability and introduces improvements expected to reduce nail toggling, screw migration and secondary loss of reduction. The aim of this study was to investigate the biomechanical competence of the novel angular stable intramedullary nail concept for treatment of unstable dista

Periprosthetic distal femoral fractures (PPDFF) following knee arthroplasty represent an increasing challenge for trauma surgery and orthopedics, particularly for older multimorbid patients. Treatment concepts vary depending on the fracture type, prosthesis stability, bone quality and patient factors [1, 6-10, 13]. Depending on the fracture type and bone quality, modern locking angle plates, retrograde intramedullary nails with angulated stems, double plate and nail-plate constructs provide a high primary stability and usually enable early functional mobilization [5, 6, 11, 13, 30-33]. INTRODUCTION: The Ilizarov distraction osteogenesis is a recognized method of limb lengthening in orthopaedic practice. Its most challenging problems are long duration of external fixation and related pin-tract infection and joint contractures. The solution might be the use of a bioactive degradable intramedullary implant stimulating bone healing. Case Presentation. We present a case of a 14-year-old boy with 6 cm posttraumatic shortening of the femur and associated varus deformity of 20 degrees. He was treated with the Ilizarov technique of femur lengthening over an intramedullary degradable

BACKGROUND: Primary bone tumours have traditionally been treated by forequarter amputation. However, recent limb salvage techniques have become available which are comparable for tumour control and preserve a useful distal limb. This report describes the use of the Huckstep ceramic shoulder with ceramic spacers, non-vascularized fibular grafts and Huckstep nail with ceramic spacers.
METHODS: Three techniques are described for the reconstruction of the humerus following primary tumour excision. The seven primary bone tumours in this series included three patients with osteosarcoma, two with ch

INTRODUCTION: Osteogenesis imperfecta (OI) presents significant surgical challenges due to bone fragility, narrow medullary canals, and complex deformities. While telescoping rods like the Fassier-Duval (FD) system are commonly used in growing patients, they are unsuitable when the canal diameter is too small or when patients approach skeletal maturity. The Simple Locking Intramedullary (SLIM) nail offers a solid, non-telescoping alternative in these cases.
METHODS: We describe the surgical technique for SLIM nail implantation and highlight key technical pearls developed through institutional

Thumb reconstruction has been a very challenging issue for hand surgeons. In this report, we present a case of thumb reconstruction with combination of the wrap-around flap prefabricated by the medialis pedis perforator flap with phalanx and nail bed banked from the amputated thumb. A 22-year-old man suffered from the left thumb amputation as well as large soft tissue defect of hand and comminuted fracture in wrist due to a crush accident. The distal phalanx and nail bed of left thumb were exposed and no suitable vessels for microsurgical anastomosis could be found, resulting in the great diff

Intramedullary nailing is widely used for prophylactic stabilization of femoral metastases. Polymethylmethacrylate (PMMA) cement augmentation is often applied, yet its biomechanical necessity across lesion sizes remains uncertain. To address this, we evaluate whether intramedullary nailing (IMN) alone provides sufficient stabilization, whether cement augmentation offers additional displacement reduction, and how it affects stress distribution in the bone and implant across varying lesion sizes. Finite element analysis is performed on femoral models simulating 1 cm, 3 cm, and 5 cm midshaft meta

INTRODUCTION: Lower extremity fractures are a frequent complication in patients with chronic spinal cord injury due to significant bone mineral density loss and altered bone metabolism. While non-operative management has traditionally been favored, advances in surgical techniques have expanded treatment options, including intramedullary nailing and plate fixation. However, the optimal approach remains controversial, especially in paraplegic patients with distal femoral fractures. This case report highlights the challenges and complications associated with surgical management in this unique pat

INTRODUCTION AND IMPORTANCE: Foreign body ingestion is most common in children, as well as adults with neurodevelopmental and psychiatric problems. The most commonly swallowed foreign bodies in adults include fish bones, chicken bones, toothpicks, and dentures. Sharp and elongated ones are the most typical causes of gastrointestinal perforation among the aforementioned. In about 1% of cases, foreign bodies are known to remain lodged within the bowel and cause luminal erosion and perforation.
CASE PRESENTATION: A 23-year-old male construction laborer presented to the Emergency Surgical Unit of
