Brain abscess and epidural empyema caused by Salmonella enteritidis in a child: successful treatment with ciprofloxacin: a case report.

Focal intracranial infections caused by Salmonella are rare, especially those produced by S. enteritidis. We describe the case of a 26-month-old girl who underwent surgery for a frontoparietal ependymoma and presented with epidural empyema and a brain abscess due to S. enteritidis following an episode of gastroenteritis. The child was successfully treated by surgical drainage along with 9 weeks of antibiotic therapy including ciprofloxacin.

A rare but fatal case of granulomatous amebic encephalitis with brain abscess: the first case reported from Turkey.

The incidence of protozoal and helminthic infestations of the central nervous system (CNS) is less than 1%, but these infestations tend to follow a fatal course. They are more common among children, the elderly and immunocompromised individuals. CNS infections due to Entamoeba histolytica have been known for a long time. In recent years, especially in developing countries, there has been an increase in CNS infections due to free-living amebas (FLAs). Acute CNS infection due to Naegleria fowleri, which ends in death within 2-7 days, is termed primary amebic meningoencephalitis (PAM); subacute o

Primary Nocardia farcinica brain abscess with secondary meningitis and ventriculitis in an immunocompetent patient, successfully treated with moxifloxacin.

We report a rare case of Nocardia farcinica occipital brain abscess in an immunocompetent patient with no underlying risk factors successfully treated with the antibiotic moxifloxacin. The patient underwent craniotomy and abscess drainage. Initial post-operative treatment with co-trimoxazole produced a limited response. Despite the development of skull base meningitis and ventriculitis subsequent addition of moxifloxacin produced an excellent outcome.

Successful medical treatment of multiple brain abscesses due to Nocardia farcinica in a paediatric renal transplant recipient.

Brain abscesses caused by Nocardia are rare, but it is very important to detect and treat them early because the associated mortality is 3 times higher than that associated with other bacterial brain abscesses. This infection is prevalent among adults on long-term immunosuppressive therapy; we report the case of a male kidney transplant recipient aged 12.7 years who developed early multiple Nocardia-induced brain abscesses that were successfully treated with linezolid, a novel antibiotic therapy.

Borrelia garinii spirochetemia in a previously healthy child presenting with Streptococcus anginosus group brain abscess: a case report.

BACKGROUND: The clinical significance of transient Borrelia burgdorferi sensu lato complex (hereafter referred to as B. burgdorferi) spirochetemia remains poorly understood and has rarely been documented. We report the isolation of viable B. garinii genospecies from the blood of a previously healthy child presenting with a Streptococcus anginosus group (SAG) brain abscess. CASE PRESENTATION: An eight-year-old girl presented with four days of headache, fever, nausea, slightly reduced general condition, and mild facial asymmetry. Initial evaluation, including lumbar puncture, raised suspicion o…

Delayed brain abscess formation after mechanical thrombectomy for probable septic cerebral embolism: A case report and literature review.

BACKGROUND: Septic cerebral embolisms (SCE) are serious neurological complications of sepsis. An embolic occlusion due to infected thrombi can precipitate ischemic stroke and a spectrum of secondary pathologies, including intracerebral hemorrhages, mycotic aneurysms, meningitis, and brain abscesses, which are a major complication of SCE. However, delayed abscess formation following cerebral infarction is rarely encountered, and the underlying mechanisms remain poorly understood. Here, we present a rare case of SCE in a patient who underwent a mechanical thrombectomy (MT) for large-vessel occlu…

A Large Language Model Workflow for Auditable Brain Abscess Risk Stratification and Pre-residency Scholarship: A Technical Report.

Large language models (LLMs) are increasingly available to medical trainees, but transparent and auditable methods for incorporating them into scholarly work and emerging artificial intelligence (AI) literacy curricula remain limited. This technical report describes a mentored educational framework in which a de-identified brain abscess case report was transformed into a reproducible, LLM-supported risk-stratification model to teach rigor, verification, and structured AI use to a fourth-year medical student. A single clinical case was reconstructed using structured variables from history, imag…

Brain abscess mimicking acute stroke syndrome caused by dual Filifactor alocis and Porphyromonas gingivalis infections: A case report.

This case report is about a woman who had a brain abscess in the left parietal lobe that atypically presented as acute stroke-like syndrome. Pus samples from brain abscess aspiration revealed the periodontal pathogens Porphyromonas gingivalis and Filifactor alocis. After dental health care and 8 weeks of combined antimicrobial therapy, the patient recovered completely.

An unusual case report of brain abscess caused by prevotella loescheii identified using the metagenomic next-generation sequencing.

OBJECTIVES: Brain abscess (BA) is a rare and life-threatening disease which remains to be a challenge for physicians despite recent advances in both the diagnosis and treatment strategies. Appropriate management of brain abscesses requires a combined surgical and medical approach to prevent associated life-threatening complications. In clinical practice, new diagnostic techniques, such as metagenomic next-generation sequencing (mNGS) and quantitative electroencephalogram (qEEG), can help physicians identify the causative pathogens of brain abscesses in order to provide early pathogen-targeted …

Opportunistic Fonsecaea pedrosoi brain abscess in a patient with non-cirrhotic portal fibrosis-induced hypersplenism--a novel association.

Non-cirrhotic portal fibrosis, a common cause of splenomegaly in tropical countries, can lead to hypersplenism and pancytopenia. Hypersplenism in this setting has not been associated with opportunistic infections. We describe a patient with hypersplenism secondary to non-cirrhotic portal fibrosis who developed a Fonsecaea pedrosoi brain abscess and succumbed to the illness despite aggressive management.

Fusobacterium nucleatum as an Emerging Culprit in Brain Abscesses: A Narrative Synthesis of 25 Years of Clinical and Diagnostic Data.

Brain abscesses are life-threatening infections, predominantly caused by anaerobic organisms. The role of oropharyngeal microbiota, presence in dental plaque biofilms, and hematogenous spread is established in the literature. However, due to its rare occurrence, limited literature is available on its management. The rationale of this narrative review was to elucidate risk factors, treatment strategies, and outcomes of brain abscesses caused by Fusobacterium nucleatum (F. nucleatum). After a detailed literature search using PubMed, 12 cases were analyzed, containing a total of 13 patients, over…

Community-Acquired Escherichia coli Meningoencephalitis and Brain Abscess in an Immunocompromised Patient: A Case Report and Literature Review.

Gram-negative bacterial meningitis (GNBM) is rare, primarily affecting infants. Escherichia coli (E. coli) is the most common cause, with high mortality and poor clinical outcomes. We report a rare case of an immunocompromised 61-year-old male with a history of chronic lymphocytic leukemia who presented with altered mental status and fever. He was diagnosed with E. coli meningoencephalitis complicated by brain abscess, likely secondary to otitis media. Despite initial medical management, neurosurgical intervention was required due to abscess progression. The patient fully recovered following c…

Clival canal defect: a rare cause of recurrent meningitis and fungal brain abscess in an immune-competent child.

A girl in her early childhood years (2-5 years) presented with multiple episodes of meningitis. Following the sixth episode, she clinically deteriorated with the development of multiple brain abscesses and hydrocephalus, requiring a ventriculoperitoneal shunt and prolonged antibiotic and antifungal therapy. MRI of the brain revealed a possible anatomical defect at the C1 level of the cervical spine, communicating into the nasopharyngeal cavity. Needle aspiration of the brainstem collection confirmed Aspergillus fumigatus on molecular testing. A transoral endoscopic adenoidectomy exposed an und…

Triple-drug antibiotic therapy for disseminated nocardial abscess in the mediastinum and brain of an immunocompetent patient: a case report.

BACKGROUND: Nocardia are widely present in nature and considered opportunistic pathogens. They can result in hematogenous spread infection through the ruptured skin or respiratory tract when the host's immune system is compromised. Currently, 119 species of Nocardia have been identified, with 54 capable of causing infections in humans. Nocardial brain abscesses are a rare intracranial lesion that accounts for only 2% of all brain abscesses, but have a mortality rate of 20-55%. This article reports a successfully cured case of mediastinal Nocardia infection with disseminated brain abscess. CAS…
