Tracheobronchial amyloidosis is a manifestation of amyloidosis of the respiratory tract characterized by focal or diffuse deposition of amyloid in the submucosa of the trachea and proximal bronchi. Tracheobronchial amyloidosis is not associated with systemic amyloidosis or pulmonary parenchymal involvement. It affects predominantly men aged over fifty. Depending on the part of the tracheobronchial tree that is affected, stenosis of the airways causes a variety of unspecific symptoms. Diagnosis is reached by means of typical presentation in CT scan followed by bronchoscopy and histopathological…

Lipomas are benign tumors that originate from mesenchymal tissue, such as subcutaneous tissue. Intrathoracic lipomas are rare, and they can occur in the chest wall, mediastinum and bronchi. In the present case, the patient had an intrathoracic lipoma that was located in the horizontal fissure of the right lung. Retrospective review of chest radiographs taken at a previous health checkup confirmed that the tumor was growing. The patient had no symptoms, and computed tomography and magnetic resonance imaging suggested that the tumor was a hamartoma. The tumor was resected by video-assisted thora…

"Large airway diseases" is being used as an all-encompassing phrase to describe a broad spectrum of pathological entities, which involves the trachea, main, lobar, and segmental bronchi of up to 3 mm diameter. Imaging modalities such as radiography, computed tomography, and magnetic resonance imaging contribute to the identification and diagnosis of each entity. Knowledge of clinical information, normal cross-sectional anatomy, and imaging characteristics of large airway diseases is necessary for appropriate radiologic evaluation. This review provides information about congenital and acquired …

A 63-year-old man who had received a permanent tracheostoma after oesophageal cancer surgery developed fistulas in the left and right main bronchi and suffered repeated aspiration pneumonia. Placing an indwelling Dumon Y-stent using a rigid bronchoscope is an option to treat fistula and stenosis at the tracheal bifurcation, but in some cases, it may be difficult to use a rigid bronchoscope in patients with a permanent tracheostoma. In this study, we report placing a Dumon Y-stent in a patient with a permanent tracheostoma using a specially modified stent and forceps instead of a rigid bronchos…

Non-cystic fibrosis bronchiectasis (bronchiectasis) is an abnormal dilatation of the bronchi and bronchioles, resulting from a prolonged recurrent infectious process due to various causes and predisposing factors. This disease has several etiologies and affects a heterogeneous population of patients. The most important viewpoint for the surgical management of bronchiectasis is to select appropriate candidates for surgery and conduct suitable anatomic lung resections at the right time under appropriate conditions. The ideal candidates for lung resection are symptomatic patients with focal disea…

We describe the case of a boy with spondyloepiphyseal dysplasia congenita. At birth, he experienced severe respiratory distress necessitating tracheotomy. Endoscopy done because mechanical ventilation failed to resolve desaturations disclosed severe tracheo-bronchomalacia. A Polyflex silicone stent was placed in the trachea (replaced by Y-Dumon stent) and 2 Palmaz metallic stents in the mainstem bronchi (overlapped with 2 Jomed stents 5 years later). Airway stenting guaranteed a suitable respiratory status and allowed a child who was expected to die at birth, to reach 13.5 years old in good co…

Mantle cell lymphoma (MCL) is a subtype of B-cell non-Hodgkin's lymphoma. Most cases of MCL have extranodal involvement at the time of the initial diagnosis; however, endobronchial involvement is rare. An 87-year-old man was referred to our hospital because of dyspnea on exertion. A chest CT revealed diffuse irregular wall thickening of the trachea and bilateral bronchi. A bronchoscopy revealed a diffuse irregular surface of the tracheal and bilateral bronchial mucosa and polyposis-like lesions. He was diagnosed as having MCL based on an endobronchial biopsy, and the diagnosis was confirmed us…

A 6-month-old boy with congenitally corrected transposition of the great arteries, ventricular septal defect, and mild pulmonary valve stenosis presented with acute obstructive respiratory life-threatening events. Airways obstruction was caused by aneurysmal dilation of the branch pulmonary arteries compressing the carina and bronchi. The child was ventilator dependent. A double switch operation with Lecompte maneuver was performed, bringing the pulmonary arteries forward and relieving the airway compression. The child made a complete recovery and remains well 9 months later with no clinical o…

Congenital lung malformations (CLM) comprise a broad spectrum of maldevelopment that spans a continuum of maldevelopment involving parenchyma, bronchi, and vasculature. They may present as respiratory symptoms at birth and can be the source of morbidity and mortality. Early diagnosis is important for adequate therapy to prevent complications, such as recurrent infections, or to help normal lung development. Prenatal diagnosis of CLM using ultrasound (US) and magnetic resonance imaging (MRI) has increased, but chest radiography and computed tomography (CT) still play important roles in making d…

Infiltration by lung tumor of adjacent anatomic structures including major vessels, main bronchi, and chest wall not only influences the oncologic severity of the disease but also increases the technical complexity of surgery, requiring extended resections and demanding reconstructive procedures. Completeness of resection represents in every case one of the main factors influencing the long-term outcome of patients. Technical and oncologic aspects of extended operations, including resection of Pancoast tumors and chest wall, bronchovascular sleeve resections, and en bloc resections of major th…

A 19-year-old man with known Hunter syndrome presented with dyspnea, and was admitted to our hospital. Bronchoscopy revealed tracheal narrowing with excessive granulation tissue formation in the trachea. Three-dimensional CT clearly demonstrated severe stenosis in the trachea and both main bronchi. Autopsy showed granulomatous tissue proliferation and deposition of mucopolysaccharide in the tracheal wall. We demonstrated the clinico-radiological-pathological correlation of bronchial lesions in Hunter syndrome, and emphasized that three-dimensional CT is helpful in deciding upon therapeutic str…

The aim of the study was to evaluate the ventilatory disturbances in alcohol addicted males. The examined group consisted of 59 alcoholics acutely poisoned with ethanol. 30 healthy individuals, occasionally drinkers, were the control group. The acetaldehyde concentration in relation to results of spirometric examination was analyzed. It's serum concentration in alcoholics was higher then in the coi* trol group. The frequency of central bronchi obstruction in ethanol dependent patients was significantly higher compared with the control. In examined group negative correlation between serum aceta…

Tracheobronchopathia osteochondrodysplasias is a benign and rare chronic disease, whose etiology remains obscure. It is characterized by the presence of subepithelial osteocartilaginous focal lesions without any relation to tracheal rings, essentially localized in the lower two thirds of the trachea and the major bronchi. We report a new case of tracheobronchopathia osteochondrodysplasias in a 47-year-old man, localized at the lower half of the trachea, revealed by haemoptysis. The lesion was suspected by bronchoscopy and the diagnosis was made histologically, showing heterotopic bone formatio…

Tracheal cartilaginous sleeve (TCS) is a congenital malformation characterized by fusion of the tracheal arches that may be isolated to a few tracheal arches, include the entire trachea, or extend beyond the carina into the bronchi. TCS has been reported only in children with craniosynostosis. Seven cases of TCS and Crouzon syndrome (CS) are mentioned in the literature. In addition to our case study, a review of the literature on TCS in CS, a classification of TCS and treatment options will be provided. Tracheotomy and frequent airway endoscopy coupled with a keen understanding of the shifting…

Tracheobronchial rupture can be associated with blunt thoracic trauma. An important factor in the physiopathology of these lesions is reflex closure of the glottis, which can be related to closed chest trauma. We report a case of nonpenetrating thoracic trauma that caused a long membranous tracheal rupture from the subcricoid area to the main carina, extending to both main bronchi. In addition, a complex esophageal rupture occurred due to the great energy liberated by the airway rupture acting as a real tracheal burst. Both lesions were diagnosed by flexible bronchoscopy. The postoperative per…

Bronchial cytology has become an essential diagnostic means in lung pathology for the diagnosis and typing of primary or secondary lung cancers. Bronchial cytology, initially consisting in the study of expectorations, has become increasingly interesting with the development of flexible fiberendoscopes, which allow the visual exploration of areas of the lungs as far down as the subsegmental bronchi. The effectiveness of this type of cytology applied to detected and located lesions accounts for the development of aspiration and brushing. However, the obtention of good cytological results primari…

A 5 year old girl suffered from nearly total stenosis of the bronchus intermedius and exspiratory collaps of both main bronchi. Because of preterm birth she was postnatally intubated and mechanically ventilated for 6 weeks. Her somatic development was significantly retarded in comparison with the monozygotic twin, and pulmonary impairment increased again in the last preoperative year. By the combination of latest surgical technics (pericardial patch, interposition of a costal segment, fibrine glue) the bronchial abnormalities could be corrected successfully. In the first year after surgery cat…

Cryptosporidial infection was diagnosed in a flock of 4-week-old common quails (Corturnix cortunix). The main gross pathological changes were excess mucus in the trachea, nasal mucosal congestion and shrunken bursa of Fabricius (Bursa cloacalis). Microscopically, the main changes were epithelial deciliation and hyperplasia and inflammatory cell infiltration of the lamina propria in the trachea, bronchi and nasal cavity; epithelial hyperplasia in some oesophageal and salivary glands, and epithelial hyperplasia and follicular atrophy in the bursa. Protozoan parasites attached to the affected epi…
