Quality of fracture reduction assessed by radiological parameters and its influence on functional results in patients with pilon fractures-A prospective multicentre study.

INTRODUCTION: The management of pilon fractures remains a challenging issue. Due to the complexity of factors that influence the outcome, it has been questioned if anatomical reductions of articular fracture are relevant. The lack of a commonly accepted assessment of quality of fracture reduction compounded the uncertainty of the importance of anatomical reduction in pilon fracture. The current study aimed to define parameters that can better assess the reduction quality and to investigate the influence of reduction quality on functional outcomes. METHODS: Patients with unilateral pilon fract

Retrosternal displacement after physeal fracture of the medial clavicle in children treatment by open reduction and internal fixation.

Retrosternal displacement of the medial aspect of the clavicle after physeal fracture is rare. We treated six patients with this injury between 1995 and 1998, all as an emergency in order to avoid complications associated with compression of adjacent mediastinal structures. Attempted closed reduction was undertaken, but all required open reduction and internal fixation using a wire suture. There were no associated complications. Five were reviewed clinically and radiologically at a minimum of one year after operation. All had regained full use of the affected arm without pain and had resumed t

Elastic stable intramedullary nailing in paediatric forearm fractures: the rate of open reduction and complications.

The aim of this study was to evaluate the rate of open reduction and complications of elastic stable intramedullary nailing (ESIN) in treating unstable diaphyseal forearm fractures in children. We performed a retrospective review of a consecutive series of 102 paediatric patients with a mean age of 9 years (range: 7-14 years) who underwent ESIN of unstable closed forearm fractures at three different centres. Closed reduction of one or both bones was achieved in 68 (67%) patients and open reduction was required in 34 (33%) patients. The rate of open reduction in single-bone fractures (52.2%) wa

Outcome of closed proximal phalangeal fractures of the hand.

BACKGROUND: The proximal phalanx (PP) of the fingers is fractured more frequently than the middle or even distal phalanges. The problems of malunion, stiffness and sometimes loss of skin or other soft tissues associated with PP fracture increases the disability. The optimum treatment depends on fracture location, fracture geometry and fracture stability. The objective of the study was to analyse the treatment outcome in a series of closed proximal phalangeal fractures of the hand. MATERIALS AND METHODS: Eighty-four proximal phalangeal fractures in 68 patients were enrolled from 2007 to 2009.

Long-term Functional Consequences of Surgically Treated Pediatric and Adolescent Olecranon Fractures: Does the Presence of Associated Fractures and Surgical Procedure Affect the Results?

BACKGROUND: Olecranon fractures are rare conditions in childhood. The aim of this study was to investigate the factors affecting the results in surgically treated pediatric and adolescent olecranon fractures. METHODS: The orthopaedic trauma database of a large academic tertiary center was retrospectively searched for patients who had sustained an olecranon fracture and were treated surgically between 2005 and 2021. Data related to demographic features, additional fractures, and the presence of any disease were obtained from the patient files. Mayo elbow performance score and the Turkish-langu

Closed reduction of multiple MCP joint dislocations.

We report a young man in his 20s who fell from the roof of his house and sustained injury to his left hand. Physical examination and radiographic evaluation revealed closed dislocations of the second, third, and fourth metacarpophalangeal (MCP) joint with fractures of the bases of the third and fourth metacarpals and the base of the proximal phalanx of the middle finger. The injury was managed with closed reduction and splintage with an uneventful outcome. Multiple MCP joint dislocations are rare, and there are only a few reports of closed reduction with a good outcome.

Pure closed posteromedial dislocation of the tibiotalar joint without fracture.

Pure tibiotalar dislocation without an associated fracture is an extremely rare injury. We present three cases of closed posteromedial tibiotalar dislocation without any associated fractures to the foot, ankle, or leg. All patients were treated conservatively with immediate closed reduction under general or local anaesthesia and immobilised in a short leg cast for six weeks without weight-bearing resulting in a satisfactory outcome at the final follow-up. A review of the literature is also presented in this paper.

Traumatic posterior atlantoaxial dislocation without neurological deficit: case report.

A case is presented in which a patient sustained a closed head injury with subsequent acute subdural hematoma and delayed posterior dislocation of the atlas without fracture of the odontoid process and with an intact C-1 arch. The subdural hematoma was treated operatively. The atlas dislocation was managed with closed reduction without manipulation and subsequent posterior fusion of C-1, C-2, and C-3. The patient was intact neurologically on admission and remained so throughout his hospitalization.

Fracture of mandibular condyle—to open or not to open: an attempt to settle the controversy.

OBJECTIVE: To compare the outcome of the open method versus the closed method of treatment for mandibular condylar fracture. STUDY DESIGN: Fifty patients with fractures of the mandibular condylar processes were evaluated. All fractures were displaced, with a degree of deviation between the condylar fragment and the ascending ramus of 10 to 45 degrees (mediolaterally). The patients were randomly divided into two groups, with group 1 receiving open reduction internal fixation and group 2 receiving closed reduction. The follow-up was done over the period of 6 months. RESULTS: Statistically sign

Lateral subtalar dislocation: review of the literature and case presentation.

Lateral subtalar joint dislocations are rare injuries which are usually the result of a violent eversional force, driving the talar head medially and displacing the rest of the foot laterally. Associated fractures and obstruction to closed reduction are more common with lateral than medial dislocations. Avascular necrosis of the talus and navicular as well as subtalar instability may result from these injuries. Severe post-traumatic subtalar arthritis may warrant triple arthrodesis. Prompt closed reduction of lateral subtalar dislocation is recommended to avoid soft-tissue or neurovascular com

Irreducible epiphyseal plate fracture of the distal ulna due to interposition of the extensor carpi ulnaris tendon. A case report.

It is well known that tendon or muscle interposition can cause fractures to be irreducible by closed means. A rare tendon interposition of extensor carpi ulnaris occurred in an 11-year-old boy. The isolated displaced distal ulnar physeal injury was the result of a motor vehicle accident. Attempts to perform a closed reduction failed. At operation, an interposed extensor carpi ulnaris tendon was found. After repositioning the tendon, a near anatomic reduction was easily accomplished. Only two cases seem to have been reported in the literature on the need for open reduction of distal ulnar physe

Capitellar and Trochlear Fractures.

Fractures of the capitellum and trochlea account for a small proportion of elbow trauma. Clinicians need to be vigilant in their assessment as they are commonly associated with other injuries about the elbow. To optimize outcomes, the goals of management include a stable, anatomic reduction and early range of motion. Closed reduction of noncomminuted fractures may be successful but requires close follow-up. Open reduction and internal fixation is the preferred management of displaced capitellum-trochlear fractures. Elbow stiffness is the most commonly reported complication in operatively treat

Isolated Carpal Dislocation of the Pisiform with Distal Radius Fracture in Two Adults: A Rare Entity.

Background  Pisiform dislocations are an extremely rare injury. There are reports in the literature of isolated dislocations, but to our knowledge there are no reports of distal radius fractures with associated pisiform dislocations. Methods and Results  We present two cases of isolated pisiform dislocation and distal radius fracture in the adult population. Both patients were managed conservatively with closed reduction in both the distal radius and pisiform, and subsequently achieved good pain relief and progressive return of wrist function. Conclusion  For adult patients with distal radius

Triplane distal tibial epiphyseal fracture.

The uncommon type IV Salter-Harris distal tibial epiphyseal so-called triplane fractures has been described as a three-fragment fracture by several authors, but other descriptions based on roentgenograms alone include the injury among two-fragment fractures. We report here on one case with three fragments and five with two fragments. Good results have been obtained by closed methods, but there is more than 2 mm of joint incongruity after reduction, open reduction and internal fixation are indicated. Premature closure of the epiphyseal plate resulted but it was not an important consideration be

Simultaneous ipsilateral Monteggia fracture-dislocation and distal radius fracture: A report of a pediatric case and review of the literature.

We present the case of a 12-year- old boy who suffered from a combination of Monteggia fracture-dislocation along with ipsilateral distal radius fracture. The patient underwent closed reduction of the Monteggia lesion and the distal radius followed by percutaneous pinning. The postoperative course was satisfactory. Associated Monteggia fracture-dislocation and ipsilateral distal forearm fracture are rare. Our case and the literature review highlight the presence of common clinical features in pediatric patients with these particular injuries, including male dominance, age around 9 years, fall
