Minimally invasive image-guided cochlear implantation surgery: first report of clinical implementation.

OBJECTIVES/HYPOTHESIS: Minimally invasive image-guided approach to cochlear implantation (CI) involves drilling a narrow, linear tunnel to the cochlea. Reported herein is the first clinical implementation of this approach. STUDY DESIGN: Prospective cohort study. METHODS: On preoperative computed tomography (CT), a safe linear trajectory through the facial recess targeting the scala tympani was planned. Intraoperatively, fiducial markers were bone-implanted, a second CT was acquired, and the trajectory was transferred from preoperative to intraoperative CT. A customized microstereotactic fram…

Cochlear implantation is safe and effective in patients with MYH9-related disease.

BACKGROUND: MYH9-related disease (MYH9-RD) is a rare syndromic disorder deriving from mutations in MYH9, the gene for the heavy chain of non-muscle myosin IIA. Patients present with congenital thrombocytopenia and giant platelets and have a variable risk of developing sensorineural deafness, kidney damage, presenile cataract, and liver abnormalities. Almost all MYH9-RD patients develop the hearing defect, which, in many individuals, progresses to severe to profound deafness with high impact on quality of life. These patients are potential candidates for cochlear implantation (CI), however, no …

Cochlear implantation in a bilateral sensorineural hearing loss patient with relapsing polychondritis.

Relapsing polychondritis (RP) is a rare disease which presents chondritis in multiple organs. Characteristic features include auricular chondritis, arthritis, nasal chondritis, ocular inflammation, respiratory tract involvement and audiovestibular damage. Fifty percent of cases of RP are associated with inner ear symptoms such as dizziness and hearing loss. We have recently encountered a case of RP in a 34-year-old man who had recurrent chondritis of both auricles and progressive bilateral profound sensorineural hearing loss; he had been treated many times with steroids, immunosuppressants, pl

Cochlear implantation after surgery for bilateral jugular foramen paragangliomas: a case report.

This paper describes a case of cochlear implantation for deafness due to bilateral glomus jugulare tumors (paragangliomas) as well as an unusual complication after cochlear implantation in a 31-year-old male. A 31-year-old male with profound sensorineural hearing loss on the right side and deafness on the left side, caused by bilateral jugular foramen paragangliomas, was implanted with a multi-channel cochlear implant (Combi 40+, Med-El) on the right side during subtotal petrosectomy for removal of the right-sided glomus jugulare tumor. No postoperative medical complications were observed. The…

Cochlear implantation outcome in prelingually deafened young adults. A speech perception study.

The outcome of cochlear implantation in patients with deafness of prelingual onset is largely unpredictable due to high individual variability. This study evaluated speech perception performances in a group of 18 prelingually deafened subjects (aged 13-30 years) which was homogeneous with respect to duration of deafness, hearing aid use before cochlear implantation, mode of communication and administration of auditory-oral speech therapy. Word discrimination length, word and sentence identification, phoneme identification and word and sentence recognition were tested before cochlear implantati…

Cochlear implantation trauma and noise-induced hearing loss: Apoptosis and therapeutic strategies.

Cochlear implantation trauma and noise-induced hearing loss both involve a physical disruption of the organ of Corti and may involve several mechanisms of cell death at the molecular level, i.e., necrosis, necrosis-like programmed cell death (PCD; type 2 PCD), and apoptosis (type 1 PCD). This article reviews several promising therapeutic strategies that are currently being developed. One of these promising new strategies involves the use of a highly effective peptide inhibitor of the c-Jun N-terminal kinase cell death signal cascade (i.e., D-JNKI-1) to prevent apoptosis of injured auditory hai…

Speech perception skills of deaf infants following cochlear implantation: a first report.

OBJECTIVE: We adapted a behavioral procedure that has been used extensively with normal-hearing (NH) infants, the visual habituation (VH) procedure, to assess deaf infants' discrimination and attention to speech. METHODS: Twenty-four NH 6-month-olds, 24 NH 9-month-olds, and 16 deaf infants at various ages before and following cochlear implantation (CI) were tested in a sound booth on their caregiver's lap in front of a TV monitor. During the habituation phase, each infant was presented with a repeating speech sound (e.g. 'hop hop hop') paired with a visual display of a checkerboard pattern on…

[Comparative analysis of approaches to surgical cochlear implantation in adults and children].

The aim of a surgical stage of cochlear implantation (CI) is to place the implanted part of the device in the temporal space. Criteria of the decision on the side of the intervention and components of it in CI are outlined. To help selection of an optimal variant of surgical intervention in CI at the initial stage, the techniques of the implanted parts insertion are reviewed. Types of skin cuts, approaches to the cochlea, location of cochleostoma, methods of fixing the stimulator are described. The importance of intraoperative registration of the musculus stapedius reflex and action potential …

Audiophonological results after cochlear implantation in 40 congenitally deaf patients: preliminary results.

The aim of this study is to evaluate the prognostic factors of audiophonological results in cochlear implant in congenitally deaf patients. Between 1991 and 1996. 40 congenitally deaf children underwent cochlear implantation in our department, at an average age of 7 years (median: 5 years). The results of speech therapy were evaluated with a mean follow-up of 2 years and were classified according to four criteria: perception of sound, speech perception, speech production and the level of oral language. For each criterion, a score was established ranging from zero to four. These scores were wei…

Case Report: Streptococcus suis-associated meningoencephalitis with profound bilateral labyrinthine deafness and intra-operative cochlear fibrosis treated by cochlear implantation.

Streptococcus suis is a common zoonotic pathogen that usually causes monomicrobial meningitis. Occult bloodstream detection of Klebsiella pneumoniae, together with proven central-nervous-system S. suis infection, followed by irreversible cochlear fibrotic changes requiring cochlear implantation, appears exceptionally rare. We report a 52-year-old male with chronic heavy alcohol intake and occupational swine-exposure history who developed severe sepsis, recurrent suppurative meningoencephalitis caused by S. suis, and permanent bilateral labyrinthine-related deafness. Combined blood and cerebros…

Prevalence of Macrophages Within the Cochlear Vessels Following Cochlear Implantation in the Human: An Immunohistopathological Study Using Anti-Iba1 Antibody.

HYPOTHESIS: The prevalence of monocyte-derived macrophages within cochlear vessels may increase following cochlear implantation. BACKGROUND: Recently, we reported an increase in the number of ionized calcium-binding adaptor molecule 1 (Iba1)-positive macrophages in selected cochlear sites such as the osseous spiral lamina and Rosenthal's canal following cochlear implantation. Activation of the immune system induces the recruitment of monocyte-derived macrophages. The prevalence of monocyte-derived macrophages within cochlear vessels may increase following cochlear implantation. However, the d…

Binaural Fusion Sharpens on a Scale of Octaves During Pre-adolescence in Children with Normal Hearing, Hearing Aids, and Bimodal Cochlear Implants, but not Bilateral Cochlear Implants.

PURPOSE: The breadth of binaural pitch fusion, the integration of sounds differing in frequency across the two ears, can limit the ability to segregate and understand speech in background noise. Binaural pitch fusion is one type of central auditory processing that may still be developing in the pre-adolescent age range. In addition, children with hearing loss potentially have different trajectories of development of central auditory processing compared to their normal-hearing (NH) peers, due to disruption of auditory input and/or abnormal stimulation from hearing devices. The goal of this stud

Prelingually Deaf Children With Cochlear Implants Show Better Perception of Voice Cues and Speech in Competing Speech Than Postlingually Deaf Adults With Cochlear Implants.

OBJECTIVES: Postlingually deaf adults with cochlear implants (CIs) have difficulties with perceiving differences in speakers' voice characteristics and benefit little from voice differences for the perception of speech in competing speech. However, not much is known yet about the perception and use of voice characteristics in prelingually deaf implanted children with CIs. Unlike CI adults, most CI children became deaf during the acquisition of language. Extensive neuroplastic changes during childhood could make CI children better at using the available acoustic cues than CI adults, or the lack

Conditional Minimal Detectable Change for the Cochlear Implant Quality of Life-35 Profile Associated With Improved Functional Abilities 12 Months After Cochlear Implantation.

IMPORTANCE: It is essential to measure an individual patient's baseline and follow-up abilities to demonstrate changes in clinical outcomes over time. Inherent in this strategy is interpreting whether the measured change is clinically significant and beyond measurement error. Conditional minimal detectable change (cMDC) values are widely used in many disciplines but have rarely been established for outcome measures in otolaryngology or hearing research, and never in cochlear implantation. OBJECTIVE: To determine cMDC values for the Cochlear Implant Quality of Life-35 (CIQOL-35) Profile instru…
