BACKGROUND: Transcatheter aortic valve replacement (TAVR) has become an established alternative to surgical aortic valve replacement (AVR) for higher risk patients. Periprocedural TAVR complications decreased with a growing expertise of implanters. Yet, TAVR can be accompanied by life-threatening adverse events such as intraprocedural cardiopulmonary resuscitation (CPR). This study retrospectively analyzed predictors and outcomes in a cohort of patients from a high-volume center undergoing periprocedural CPR during TAVR.

Postoperative urethral strictures following extracorporeal circulation in open heart surgery are not uncommon. The etiology is not clear. Ischemia and pressure of the Foley catheter may cause a lesion that eventually leads to urethral stricture. We retrospectively analyzed 300 patients who had undergone open heart surgery. In 1 group the bladder was drained by a Foley catheter during extracorporeal circulation, while another group underwent suprapubic drainage. The endocrine, humoral and hemodynamic factors that affect renin secretion are studied during open-heart surgery in a group of 40 patients, 20 with mitral and 20 with aortic valve disease. During the incission period, the PRA (plasma renin activity) increases due to a sympatoadrenergic hyperactivity. These increase is noted also during bypass, and is closely related with the hypotension that extracorporeal circulation produces. It is also discussed the influence of the exclusion of pulmonary circulation, that decreases conversion of angiotensin I to II and catabolism of norepinephrine.
