Bladder metastasis of breast origin are rare. Lobular carcinoma is the most frequent histological subtype of the primary tumor. This secondary location can be the only one or can be associated with other locations. The prognosis is poor. The period between primary breast tumor and the development of bladder metastasis is variable. Herein is reported the case of a 68-year-old woman presenting with irritative disorders. Urological examination was performed and made the diagnosis. When having a history of breast cancer, the occurrence of urinary symptoms require radiographics and a cystoscopy.

Hydrocolpos presenting at birth in a neonate with a cloacal malformation may lead to massive distension, with compression of adjacent structures. At times, the hydrocolpos requires urgent drainage. OBJECTIVE: To report a case of adeno-carcinoma arising in an augmented bladder 41 years after the procedure.
METHODS: After troublesome evaluation and fistula closure with urinary diversion by percutaneous nephrostomy, the patient underwent palliative chemotherapy and radiotherapy.

Rectourethral fistula (RUF) is a relatively rare complication of radical prostatectomy, but is extremely difficult to treat. Multiple surgical approaches have been described for definitive treatment, but to date none of them have been determined to be the gold standard, either due to a high recurrence rate of the condition or due to the morbidity associated with the procedure. In this case report, we describe a successful repair of iatrogenic RUF through a multidisciplinary approach consisting of cystoscopy, urethral stent placement, colonoscopy, and TEM-assisted rectal advancement flap.

Epidermolysis bullosa is a rare inherited bullous disorder of the skin that occasionally involves the urinary tract. We describe a child with a history of epidermolysis bullosa who presented with hematuria, urgency, and dysuria. On radiographic investigation, bladder wall lesions were found. On cystoscopy, these were bullous lesions on the urinary bladder that at biopsy were shown to have inflammatory changes without evidence of malignancy. This is consistent with rare involvement of the bladder in epidermolysis bullosa. Symptomatic treatment resolved this patient's urologic complaints.

We report a rare complication of cesarean section in a 46-year-old woman presenting with incontinence. The patient was noted at cystoscopy to have a lesion on the posterior wall of the bladder. Histologic examination of the biopsied lesion demonstrated endocervical tissue, and subsequent magnetic resonance imaging revealed a vesicocervical fistula. We describe a case of a 50-year-old man with lesions localized in the region of urethral meatus and on the epithelium of the foreskin. The lesions were observed as the white spots with flat or somewhat depressed surface, locally similar to leukoplakia, lichen sclerosus atrophicus or cicatrix, without any subjective symptoms. Cystoscopy, micturition urography and biopsy were performed by urologist. The diagnosis of lichen planus Wilsoni was made based on the biopsy examination. OBJECTIVE: To report the results of treatment of adenocarcinoma of the urinary bladder with transurethral resection and intravesical bacillus Calmette-Guerin (BCG).
METHODS: Out of 183 patients in our department treated with BCG between 1992 and 1996, three patients had adenocarcinoma, stage T1.
RESULTS: All three patients had normal cystoscopy and negative cytology 53-82 months after the start of treatment.
CONCLUSIONS: BCG appears to be effective not only in the treatment of transitional cell carcinoma, but also in adenocarcinoma of the bladder.

Posterior urethral polyps are rare. A case is reported in a 7-year-old boy presenting with pain and burning micturition. Ultrasound and cystography revealed the diagnosis by showing a round solid structure in the bladder neck. Cystoscopy confirmed the diagnosis by showing its implantation base at the summit of the verumontanum. The polyp was removed by endoscopic resection and histological examination of the resection specimen showed a fibrous polyp. A brief review of the clinical features, diagnostic methods, natural history and treatment of this benign tumour is presented.

The results of our study highlight the need for change in the pretreatment workup of clinical stage Ib cervical cancer. The routine use of excretory urography, barium enema, cystoscopy,and sigmoidoscopy is not justified. MR evaluation is recommended in patients with lesions larger than 2 cm (the group with the greatest increase in predictive value). Although CT scanning is not recommended for the evaluation of parametrial invasion, both CT scanning and MR imaging provide similar positive and negative posttest probabilities for the evaluation of nodal disease.

Several temporary abdominal wall closure techniques have been described in the literature. We present our experience with an inexpensive and efficient method of temporary abdominal closure when bowel edema and distension preclude safe primary closure. Our technique is a variation of the silon (silo) closure used in the repair of gastroschisis and omphalocele, using a pre-gas-sterilized, soft 3-L plastic cystoscopy fluid irrigation bag cut to an oval shape and stapled or sutured to the skin edges of the wound.

Nd:YAG laser irradiation and flexible cystoscopy were used in the outpatient management of 16 cases (21 sessions) with superficial bladder tumors. Irradiation of tumors was accomplished in all cases except 3 in which the visual field was affected by bleeding after the cup biopsy of tumors. Tumors at the bladder neck were easily irradiated through the flexible cystoscope. No tumor recurred at or near the previously irradiated site. This method provided a safe and cost-effective means to treat superficial bladder tumors in selected cases.

Two successfully treated cases of benign colovesical fistula are presented. The condition should be suspected whenever patients with colorectal disease develop urinary symptoms. Flexible colonoscopy and a lateral abdominal X-ray should be the initial investigations; but barium enema, cystoscopy and, rarely, CT scan may be necessary to confirm the presence of a colovesical fistula. Patients should be managed conservatively during the acute phase, and then be treated definitively by a single-stage bowel resection. Simple closure or resection and closure is used for the bladder-defect.

The original concept of laparoscopy developed as a branch of cystoscopy. Since the use of the Nitze cystoscope at its inception, laparoscopy has kept pace with the innovations in our cystoscopic inventory. The recently emerging interest of urologists in utility of the laparoscope for a variety of urologic surgery is a welcome renascence of this procedure, which has been used mainly by our gynecologic colleagues, in part because of our own lack of pursuit in its development. Laparoscopy should now become an integral part of our urologic teaching and practice.

The 51chromium test is an inexpensive, noninvasive test to evaluate patients for enteric-urinary fistulas. Of 11 patients studied the 51chromium test demonstrated correctly a fistula in 5 of 6 patients with fistulas and showed no lesion in all 5 without fistulas. Excretory urography, barium enemas, visible contrast medium, cystograms and upper gastrointestinal series failed to demonstrate the fistulas with similar accuracy. Cystoscopy suggested a possible lesion in 4 patients but was definite for a fistula in only 2 of those patients.

Ten patients with various stages of enterovesical fistulization in Crohn's disease are discussed. These fistulae can develop at different times in the course of the disease, but may also cause the presenting symptoms. Cystoscopy and radiologic examination rarely visualizes the fistulae. Conservative treatment by medication can sometimes suppress the symptoms, but cannot cure. Surgical treatment, which can consist of resection of intestine but need not involve resection of parts of the bladder, gives good results at least so far as the urinary infection is concerned.
