Penetration of ceftibuten into middle ear fluid.

The penetration of ceftibuten, an extended-spectrum oral cephalosporin, into middle ear fluid (MEF) was evaluated in pediatric patients during a course of daily oral doses of 9 mg/kg of body weight for 10 days. Plasma and MEF collected at 2, 4, 6, or 12 h after at least 3 days of dosing were analyzed for ceftibuten by a high-pressure liquid chromatography method, and the data were used to calculate pharmacokinetic parameters. Plasma and MEF had almost identical maximum concentrations (Cmax) of ceftibuten (14 micrograms/ml). These Cmax values in MEF during acute otitis media were well in excess…

Lasers in surgery for chronic ear disease.

This article describes specific situations in which the laser has been found to be useful in surgery for chronic ear disease. In the opinion of the authors, the most important application is the atraumatic removal of cholesteatoma from a mobile stapes. Additional uses include precise and hemostatic removal of diseased tissue (polyps, granulations, adhesions) and manipulations upon an intact ossicular chain without induction of vibrational trauma. Potential complications such as facial nerve and inner ear injury are considered.

Microangiopathy of brain, retina, and inner ear.

Microangiopathy of brain, retina, and inner ear is a rare syndrome manifesting as arteriolar occlusions of the brain, retina, and inner ear, with resultant encephalopathy, visual, and hearing loss. Despite exhaustive laboratory examinations in these patients, no evidence of a systemic disease can be found. We treated and followed an adolescent with this disorder who initially presented with a branch retinal artery occlusion. A unique finding in this case was retinal vessel wall hyperfluorescence noted five days prior to retinal infarction. The patient developed recurrent branch artery occlusio…

Effect of middle ear on otoacoustic emissions.

Otoacoustic emissions (OAEs) are believed to originate from outer hair cell movement and are considered to be transmitted to the external auditory meatus through the middle ear in a retrograde fashion. Therefore, the effect of the middle-ear on OAEs seems to be large. However, few investigations of middle-ear effects on OAEs have been published because no apparatus that can measure the middle-ear dynamic characteristics is easily available except a conventional impedance meter. We developed a sweep frequency impedance meter (SFI) which gives us much more information on middle-ear dynamic chara…

Microfiberoptic evaluation of the middle ear cavity.

Endoscopic instruments have revolutionized surgical diagnosis and treatment. Recently, a high resolution microfiberoptic endoscope has been developed that has vast potential for otologic use. This microfiberoptic endoscope was used in cadaver and human studies to visualize the middle ear cavity. The technique used involved placing a 1.0-mm or smaller microfiberoptic scope into the middle ear via a tympanic membrane perforation, through a myringotomy tube or up the eustachian tube. Using the scope, the mesotympanum and hypotympanum can be well visualized. Similarly, the round window, oval windo…

Non-explosive blast injury of the ear.

Non-explosive blast injury of the ear refers to the otological trauma caused by a blow to the ear that seals the external auditory meatus. It results in a sudden increase of air pressure within the ear canal that strikes the tympanic membrane. The present study portrays the various aspects of middle and inner ear damage in 91 patients resulting from an assault we entitled a 'non-explosive blast injury' to the ear. Sixty cases were caused by a slap or a fist, 13 patients suffered sport accidents, mostly in ball games, and 18 patients were injured during swimming and water sports activities. The…

[Bioceramic prosthesis of the ear ossicles in tympanoplasty].

Anvil bones bioceramic prostheses were used in tympanoplasty performed in 60 patients with chronic otitis media purulenta (COMP). Complete and partial variants of the prostheses were tried. Follow-up results available for 60 patients demonstrate that the above tympanoplasty improved hearing in 41 out of 60 patients, made possible complete cavity epidermization, arrested inflammation, secured safety of the tympanic transplant in 51 patients. Bioceramic prostheses of the anvil bones provide stable hearing improvement even in COMP cases with severe impairment of the anvil bones.

Congenital anomalies limited to the middle ear.

Congenital anomalies of the middle ear are occasionally encountered during surgery for conductive hearing loss and are unexpected in patients with no other deformities. We reviewed 12 such patients operated on at The New York Eye and Ear Infirmary from 1985 through 1989. Nine of the patients (75%) had unilateral conductive hearing loss whereas three (25%) had bilateral symptoms. One had bilateral congenital middle ear anomalies. Three patients (25%) had anomalies limited to the malleus and scutum. Five patients (47%) had agenesis of the oval window. After reconstructive surgery, 72% of patient…

Whiplash, postural control, and the inner ear.

Many patients with "whiplash syndrome" experience unrelenting neck stiffness and pain. This abnormal muscular tension is postulated to be causally related to a central disorder of postural control, which has evolved secondary to injury of the inner ear labyrinthine structures. Moving platform posturography was used to demonstrate the presence or absence of a static or dynamic equilibrium disorder in 48 patients who had experienced the oscillation forces induced by a rear-end automobile collision. Other vestibular tests were used to document dysfunction of the semicircular canals and the otolit…

The effect of noise exposure on the aging ear.

The effect of noise exposure on auditory sensitivity and inner ear morphology was compared in aged and young mature mice. Hearing thresholds were obtained by auditory evoked brain stem responses (ABR) before and after noise exposure, and hair cell loss was quantified. The study was done in two parts: first to assess the effect of noise exposure on subjects with presbycusis, and second to assess its effect on aged subjects without measurable presbycusis. In the first experiment C57BL/6 mice, with an age-related hearing loss, were used as a model for presbycusis. C57BL/6 mice exhibiting presbycu…

["Otinum" in the therapy of middle ear diseases].

Otinum, a new drug made in Poland, was used to treat inflammatory diseases of the middle ear of 40 patients. The drug was applied topically as drops or tampons inserted into the external acoustic meatus. As controls, 16 patients with identical problems, who were treated with 3% boric acid alcohol, were used. Otinum had a distinct pain-killing and anti-inflammatory effect, as well as a mild keratolytic effect. Otinum is recommended to be used for acute otitis media and exacerbated chronic otitis media in the absence of pathogenic microorganisms.

The role of tissue expansion in reconstruction of the ear.

The following guidelines are suggested for the use of tissue expansion in ear reconstruction based on both personal experience and my evaluation of the reported results of expansion in the hands of other surgeons experienced in ear reconstruction: 1) Cases should be carefully selected to avoid those with excess scarring from previous surgery or trauma as well as avoid the other common contraindications to the use of tissue expansion. 2) In most congenital cases the expander should be placed through a remote incision within the postauricular hairline, and excision of the cartilage vestige delay…

Blast injuries of the ear in military operations.

Exposure to blast waves from the explosion of ordnance is a common feature of combat operations. The ear is the organ most sensitive to such strong pressure waves. Blast damage to the ear may range from minor acoustic trauma to major disruption of middle and inner ear structures. Standard hearing protectors should offer a significant degree of shielding from these injuries. Aural injury alone seldom should cause a soldier to abandon a combat mission. However, the potential for a large temporary or permanent neurosensory hearing loss does raise issues about such a soldier's effectiveness on the…

The glomus tympanicum: a middle ear chemoreceptor?

Negative pressure in the middle ear has been identified as an important factor in the pathogenesis of middle ear disease. Unfortunately, to date, the physiological mechanisms that control middle ear pressure are poorly understood. Recent interest has focused on the possible role of carotid body-like tissue in the middle ear (the glomus tympanicum) as a chemoreceptor for changes in the gas composition of the middle ear. From the few anatomical studies of normal glomus tissue in the temporal bone it would seem that glomus bodies may not be consistently present in the middle ear. In this study, g…

The Red Ear-drum: To Treat or Not To Treat?

Acute otitis media is typified by the patient with fever, earache, and a "red" ear-drum. Suppurative otitis media, however, often presents without specific symptoms, and redness is the least reliable physical finding. Impaired tympanic membrane mobility on pneumatic otoscopy and bulging and opacification of the ear-drum are more reliable and important physical signs. Review of placebo-controlled trials reveals that antimicrobial therapy produces more rapid relief of fever and earache in patients with the most symptoms. Effects on patients with fewer symptoms are more modest. Amoxicillin remain…

Congenital cholesteatoma of the middle ear in children.

Congenital cholesteatoma, once considered to be a rarity, has shown an exponential increase in its incidence as evidenced by numerous clinic … esteatomas behind intact tympanic membranes, treated at the Montreal Children's Hospital over a 10-year period. They represent 22% of the entire cholesteatoma population. There appears to be two groups of patients, each with a distinct clinical presentation that correlates well with the surgical outcome. Whether this implies different pathogenetic mec …
