Tennis elbow defines a condition of varying degrees of pain or point tenderness on or near the lateral epicondyle. It is prevalent in individuals who perform a combination of forceful and repetitive activities including athletes and wheelchair users. The most common work-related disorder at the elbow is tennis elbow. Histopathological findings indicate that tennis elbow is a degenerative condition, called tendinosis, of the common extensor tendon, with the extensor carpi radialis brevis tendon more commonly implicated as the primary location of tendinosis. Despite the absence of inflammation, …

Neuropathic arthropathy, also known as Charcot arthropathy, of the elbow joint is a rare and progressive joint disorder with little available information in the literature. Presentation and treatment data on 6 patients with this pathology are reviewed. In addition, outcome data are presented on all 6 patients at a mean follow-up of 51 months. Three of these patients had undergone surgical procedures for the treatment of their pathology. At the time of their latest follow-up, 2 patients had died but reported good pain relief and functional range of motion before their deaths. At a mean follow-u…

Tuberculous arthritis of the elbow joint is not commonly seen by orthopaedic physicians. In the past 20 years, with the pandemic of the human immunodeficiency virus and the increase in immunocompromised people, we have observed a resurgence of tuberculosis. Osteoarticular involvement has increased, too. Spinal and monoarthritis of weight-bearing joints such as the hip or knee are most frequently involved. The elbow joint is an uncommon location of osteoarticular tuberculosis. The aim of this case report is to describe a case of tuberculous arthritis of the elbow and the diagnostic problems tha…

Neglected elbow dislocations, rarely seen in the developed countries, are very attractive due to the difficulties of treatment and diversity of prognosis. A 32 year-old woman was admitted with chronic elbow dislocation and defective forearm pseudoarthrosis due to an accident four months prior to admission. Open reduction and distraction arthroplasty using hinged external fixator for the elbow, open reduction and internal fixation with plate-screws with fibula allograft for the forearm was performed. The elbow was stable and had 110 degrees and -15 degrees of active flexion and extension respec…

This is a report on an unusual complication of the Sauve-Kapandji procedure in patients with rheumatoid arthritis. Two women with rheumatoid arthritis who previously had an ipsilateral Sauve-Kapandji procedure experienced spontaneous transverse divergent elbow dislocations without evident trauma. Their radiographs showed medial dislocation of the proximal ulna, which was separated from the radial head. The radial head and distal end of the ulnar shaft showed remarkable instability by a pronation and supination motion without the radial and ulnar shafts being separated from each other. Stress r…

Thirteen adolescent patients with posttraumatic elbow contractures were treated with open surgical release at an average of 16.2 years of age. When possible, an extensile medial approach to the elbow was used. All patients were treated with 6 weeks of postoperative continuous passive motion in addition to physical therapy and splinting. Eleven patients with >6 months of follow-up were evaluated at an average of 29 months after surgery. Average loss of extension improved from 57 degrees to 15 degrees, and average flexion improved from 109 degrees to 123 degrees. Average total arc of motion impr…

A 43-year-old man underwent revision of a loose total elbow arthroplasty in 1995. The arthroplasty had been implanted 20 years previously for post-traumatic osteoarthritis after a gunshot wound complicated by permanent ulnar nerve palsy. The patient suffered a minimally displaced periprosthetic fracture 4 years after implantation that was treated closed. The patient subsequently developed severe loosening with bony dilation. During revision surgery, while grasping and removing the periprosthetic membrane from within the humeral medullary canal with a Kerrison rongeur, dorsiflexion of the wrist…

The wrist and elbow, an intricate arrangement of bones, tendons, and ligaments, frequently are the source of reported problems in the primary care setting. Understanding the functional anatomy and acquiring adeptness for examination are the key to proper diagnosis of injuries and conditions that plague the wrist and elbow. A brief overview of the anatomy and physiology of the wrist and elbow joints is given. Diagnosis and management of common injuries and reported problems faced by practitioners are discussed. The provider is offered guidance on when to refer for specialty evaluation.

Soft-tissue defects in the area of the periolecranon may be a source of concern to the reconstructive surgeon who aims for durable protection with a minimum of drawbacks. Lamberty and Cormack described the antecubital fasciocutaneous flap both as a local transposition and as a free flap. The island version of this flap enables a single-stage transfer of thin, pliable, sensitive skin into the region of the periolecranon without further scarring around the defect. In general, most of the donor site can be closed primarily together with a small, full-sheet, split-thickness skin graft on the remai

Initial treatment of most compressive neuropathies at the elbow is nonoperative, consisting of rest, avoidance of elbow flexion, and, when necessary, temporary immobilization of the elbow and wrist. If symptoms persist, particularly when accompanied by muscle weakness, surgery is usually indicated. Operative procedures include decompression without transposition of the nerve (in situ or by means of medial epicondylectomy) and decompression with transposition of the nerve carried out in a subcutaneous, intramuscular, or submuscular fashion. The indications, advantages, disadvantages, and surgic…

We reviewed the cases of 10 patients with cubitus valgus deformity who had a supracondylar elbow fracture during childhood, in order to identify the causes of valgus deformity. Cubitus valgus deformity occurred in 3 cases because of failure to recognize the deformity at the initial treatment. In 7 patients the deformity was recognized but was insufficiently treated. In this series there was no evidence of progressive valgus deformity after healing of the fracture. Incomplete correction is the main cause of valgus deformity. It is suggested that, in order to reduce the incidence of valgus defor…

Elbow arthroscopy was performed in 100 dogs with foreleg lameness localised to the elbow joint, bilaterally in 48 of them, making a total of 148 joints examined. Lesions in the area of the medial coronoid process varied from cartilage fissures in 13 to fragmentation in 91, chondromalacia-like lesions in 27. This last lesion has not been reported previously in the veterinary literature. Kissing lesions on the medial side of the humeral condyle were present in 83 joints, whereas lesions of osteochondritis dissecans were found in only three. All these lesions were associated with varying signs of…

A rare case of median nerve entrapment after posterior elbow dislocation is reported. Nerve entrapment was both in the healed medial epicondyle fracture and within the elbow joint and we recommend placing this type of entrapment separately in an extended classification of median nerve entrapment following elbow dislocation.

Nineteen consecutive cases of posttraumatic arthrofibrosis of the elbow secondary to a fracture or fracture-dislocation and treated with arthroscopic debridement and manipulation were retrospectively reviewed. All of the patients had pain and stiffness in their elbows, and all had failed a conservative therapy program. All 19 patients were followed postoperatively for an average of 29 months (range, 12 to 51). One hundred-point scoring systems were used to evaluate subjective (pain, swelling, locking, and activities) and objective (range of motion) results. The average preoperative subjective …

Twenty-one total elbow arthroplasties using the Capitello-Condylar design of Ewald were performed at the Denver Orthopedic Clinic from 1977 to 1986. Indications for operation included intractable pain unrelieved by medical therapy as well as late state III or IV rheumatoid arthritis. The average total elbow score at 48 months after operation was 89.5 points, including excellent pain and function ratings. Modest gains in motion occurred with lesser gains in patients with juvenile rheumatoid arthritis than those of the adult variety. Forty percent of the elbows suffered some type of complication…

Muscle activity about the elbow during tennis strokes in nine professional and collegiate level players was studied using indwelling EMG and high speed photography. Eight muscles were evaluated for the serve, forehand, and backhand strokes. The serve was divided into six stages and the ground strokes into four stages. EMG tracings were subjected to analog-to-digital conversion and a relative measure of quantity was obtained. Analysis of variance and Turkey tests were then done to assess statistical significance (P less than 0.05). The ground strokes showed low activity in all muscles tested du…

The elbow joint was evaluated by means of computed tomography (CT) immediately following double-contrast arthrography. Normal baseline anatomy and representative abnormal studies are illustrated. Intraarticular abnormalities, such as osteocartilaginous bodies, hyperplastic synovium, fracture fragments, and osteophytes, were identified and precisely located on postarthrography CT scans. This technique, which enabled such abnormalities to be seen, has provided an anatomic and mechanical basis for seemingly idiopathic instances of limited elbow motion.

The records of 340 patients with a clinical diagnosis of tennis elbow were surveyed and the results of conservative treatment assessed. Of 157 patients successfully treated by local steroid injections, 92% achieved their good result after one or two injections. Repeated injections were not helpful. Simple lateral release of the common extensor origin was carried out on 42 elbows in 37 patients. Thirty-seven elbows in 32 patients were personally reviewed. Satisfactory pain relief was achieved in 33 elbows (89%). There were no significant complications.
