Worms, flies and four-legged friends: the applicability of biological models to the understanding of intestinal inflammatory diseases.

Diseases of intestinal inflammation, including Crohn's disease, ulcerative colitis and necrotizing enterocolitis, cause substantial acute and chronic disability in a large proportion of the population. Crohn's disease and ulcerative colitis, which are collectively referred to as inflammatory bowel disease (IBD), lead to recurrent episodes of intestinal dysfunction and systemic illness, whereas necrotizing enterocolitis is characterized by the development of dramatic and all too often fatal intestinal necrosis in infants. To determine the molecular underpinnings of these disorders, investigator

Early optimal nutrition improves neurodevelopmental outcomes for very preterm infants.

Recent advances in perinatal and neonatal intensive care have resulted in significant improvements in the survival of preterm extremely low-birthweight (PELBW) infants; however, extrauterine growth restriction (EUGR) and undernutrition occur frequently during hospitalization and are associated with adverse outcomes, including bronchopulmonary dysplasia, sepsis, and neurodevelopmental impairment. Early optimal parenteral nutrition with adequate amino acids and lipids, especially long-chain polyunsaturated fatty acids, has been shown to decrease the incidence of EUGR, bronchopulmonary dysplasia,

[Surgical closure of ductus arteriosus in the extreme or very extreme preterm infant].

OBJECTIVES: The surgical close of patent ductus arteriosus in the extreme or very extreme premature presents difficulties, because of its anaesthesia, surgical intervention and the clinical circumstances that usually accompany this pathology. We reviewed our experience in order to understand the factors that have contributed to the obtained results. METHODS: In all the reviewed patients we evalue: gestational age, birth weight, drug treatment, surgical close, incidence of bronchopulmonary dysplasia, necrotizing enterocolitis and renal failure, time of hospitalization, mortality and its causes

Cytomegalovirus Enterocolitis in a Term Neonate.

BACKGROUND: Cytomegalovirus (CMV) enterocolitis is rare in term neonates. CASE CHARACTERISTICS: A term newborn with persistent pneumonia from birth developed enterocolitis on day 18 of life. OUTCOME: Polymerase chain reaction (PCR) for CMV DNA was positive in urine sample. Antiviral therapy for six weeks resulted in successful treatment without any stricture formation. MESSAGE: CMV enterocolitis should be considered as a differential diagnosis in atypical cases of necrotizing enterocolitis in neonates.

Necroqizing enterocolitis with pneumatosis intestinalis in systemic lupus erythematosus and polyarteritis.

Pneumatosis intestinalis was encountered in association with fatal necrotizing enterocolitis in systemic lupus erythematosus (SLE) and polyarteritis nodosa. The radiologic identification of mottled, bubbly, and linear collections of intramural intestinal gas distinguish this ominous complication from benign pneumatosis cystoides intestinalis. In the setting of intestinal vasculitis due to SLE or polyarteritis nodosa, these characteristic radiologic features indicate necrotizing enterocolitis. Since corticosteroids may mask clinical progression of the intestinal lesion, radiologic evaluation is

Ischaemic enterocolitis: an expression of the intravascular coagulation syndrome.

In 20 cases of ischaemic enterocolitis unassociated with lesions of major vessels, fresh thrombi were found in the small vessels of the bowel wall and were widespread in other tissues. The capillaries of the renal glomeruli and vessels in the lung were the commonest sites. Pseudo-membranous enterocolitis is shown to be an early or more slowly evolving but identical entity to other forms of so-called ischaemic enterocolitis. This is a condition which includes ischaemic enterocolitis, postoperative enterocolitis, necrotizing enterocolitis, and probably some cases of clostridial and staphylococca…

Neutropenic enterocolitis in aplastic anemia - a case report from Nepal.

INTRODUCTION AND IMPORTANCE: Neutropenic enterocolitis (NE) is a life-threatening necrotizing enterocolitis, particularly in neutropenic patients. CASE PRESENTATION: We are presenting a case of a 22-year-old male who presented chief complaints of abdominal pain, reddish black stool, abdominal distention, and low-grade fever with laboratory and bone marrow findings suggestive of aplastic anemia. Computed tomography scan of the abdomen and pelvis with contrast was used to establish the diagnosis of necrotizing enterocolitis. Nonsurgical management including broad-spectrum antimicrobials, bowel

Gastrointestinal morbidity after Norwood palliation for hypoplastic left heart syndrome.

BACKGROUND: Neonates with hypoplastic left heart syndrome are at high risk for developing gastrointestinal complications after first stage palliation. These complications likely play a major role in their morbidity and mortality. The goal of this review was to examine the incidence and clinical impact of gastrointestinal morbidities in these newborns. METHODS: The charts of all neonates with hypoplastic left heart syndrome who underwent stage-one palliation between January 1997 and December 2001 were reviewed to determine the incidence of gastrointestinal complications. Demographic, periopera

Causes of gastrointestinal hemorrhage in neonates and children.

The causes of gastrointestinal hemorrhage in children were detailed in 1964 by Spencer. We investigated the causes of gastrointestinal hemorrhage in 165 children seen at our institution over a 13-year period. The most common causes of gastrointestinal hemorrhage in the hospitalized patients were necrotizing enterocolitis (44%), anal fissures (23%), and guaiac-positive stools of unknown cause (16%). The mortality rate was 5.4%. Necrotizing enterocolitis was the most common cause of death. Only 16 patients required surgery. Gastrointestinal hemorrhage is an infrequent cause of mortality and morb

[Hemorrhagic colitis in newborn infants. Apropos of 32 cases].

The authors report 32 cases of neonatal hemorrhagic colitis observed during a 19-mth period in the neonatal unit of the University Medical Center of Caen. Neonatal hemorrhagic colitis is characterized by rectal bleeding alone occurring mostly in premature infants. There are no accompanying clinical, biological or radiological signs of necrotizing enterocolitis. The evolution is always benign without any particular therapy. Prematurity and cesarean section appear to be the most important etiological factors. The pathogenesis is probably multifactorial. The differential diagnosis with necrotizin

Histological severity of fetal inflammation is useful in predicting neonatal outcome.

Intrauterine inflammation contributes to neonatal infection-related morbidity. A new histological framework of placental inflammation has recently been proposed; however, the association between this method and clinical findings has not been defined. To assess the clinical relevance of this system, we studied placental findings in 272 singleton neonates born at less than 34 weeks gestation. The incidences of sepsis, intraventricular hemorrhage, chronic lung disease, and necrotizing enterocolitis increased in a stepwise fashion with severity of placental inflammation. After adjusting for gestat

Treatment of postoperative enterocutaneous fistulas with octreotide in two neonates.

Enterocutaneous fistula (EF) in newborns and prematures is a well-recognized complication after necrotizing enterocolitis and other abdominal surgical procedures. Conservative management consists of bowel rest, antibiotics, wound care, and the administration of drugs that either reduce gastrointestinal motility or secretions. Octreotide decreases gastrointestinal secretions, inhibits or blocks the effects of gastrointestinal hormones, diminishes gut motility and thus reduces the flow through the fistula. We used octreotide and were able to report successful spontaneous closure of a fistula in

A retrospective review of ampicillin-sulbactam and amoxicillin + clavulanate vs cefazolin/cephalexin and erythromycin in the setting of preterm premature rupture of membranes: maternal and neonatal outcomes.

OBJECTIVE: The purpose of this study was to compare the efficacy and outcomes of 2 different antibiotic regimens that are used to prolong latency in preterm premature rupture of membranes. The primary objective was to determine whether the use of ampicillin-sulbactam/amoxicillin + clavulanate was associated with an increased risk of necrotizing enterocolitis. STUDY DESIGN: A retrospective review of pregnancies that were complicated by preterm premature rupture of membranes from 1999-2006 at 2 institutions was performed. Outcomes were compared between subjects who received parenteral ampicilli

Outcome analysis for gastroschisis.

BACKGROUND/PURPOSE: Several factors are reportedly associated with an adverse outcome in gastroschisis, including mode of delivery, in utero diagnosis, type of closure, concurrent anomalies, intestinal atresia, and necrotizing enterocolitis (NEC). Since 1969, the authors have treated 185 patients who had gastroschisis. The authors analyzed their database to identify variables associated with increased morbidity and mortality. METHODS: A retrospective study of all patients with gastroschisis treated at our institution in the last 30 years was performed. The characteristics of survivors and non

Bubbles in the belly: How well do radiology trainees recognize pneumatosis in pediatric patients on plain film?

OBJECTIVES: Pneumatosis intestinalis is a radiographic finding that refers to the presence of gas within the walls of the small or large bowel. This sign is diagnostic in the setting of premature infants with suspected necrotizing enterocolitis. Earlier detection of NEC on X-ray is vital to improve the overall management in these infants. The identification of pneumatosis intestinalis concerning for necrotizing enterocolitis by the "on-call" trainee is critical in the care of the preterm infant. Our objective was to study trainees' ability to identify pneumatosis on an emergent overnight film.

[Hirschsprung disease. Results of surgical treatment].

The Authors report their experience in the treatment of 86 patients, in pediatric age, with Hirschsprung's disease: 40 with congenital rectosigmoid or subtotal megacolon, 8 with total colic or ileocolic aganglionosis and 38 with ultrashort aganglionic segment. Out of the patients 29% had necrotizing enterocolitis with 40% mortality rate in rectosigmoid or subtotal forms and with 75% mortality rate in total colic or ileocolic forms. In the last group the mortality was higher related also to difficulties and complications of surgical treatment. Necrotizing enterocolitis is the major cause of dea

Food protein-induced enterocolitis syndrome with pneumatosis intestinalis in an exclusively breastfed infant: A case report and literature review.

A 1-month-old male, exclusively breastfed, presented with 24 h of bloody stools, vomiting, metabolic acidosis, and pneumatosis intestinalis. The patient was initially treated for necrotizing enterocolitis (NEC). However, after suspecting food protein-induced enterocolitis syndrome (FPIES), oral feeding was resumed using an exclusive elemental formula, and the biochemical and radiological findings were resolved.
