Systemic sclerosis: Advances towards stratified medicine.

Epidemiological studies reporting demographic, clinical and serological factors predictive of various outcomes in systemic sclerosis (SSc) range from the prediction of mortality to the development and progression of disease manifestations. However, predicting the disease trajectory in the individual patient is a challenging but important step towards a stratified approach to disease management. Recent technological advances provide the opportunity for new subgroupings of disease based on risk stratification, through the systematic analysis of high-dimensional clinical data combined with genes,

Human metapneumovirus as a major cause of human respiratory tract disease.

BACKGROUND: Human metapneumovirus (hMPV) is a newly identified paramyxovirus that appears to be one of the most significant and common viral infections in humans. The virus, first isolated in 2001, is a clear cause of lower respiratory tract disease in both the very young and the frail elderly. The virus causes acute wheezing in children or, less commonly, croup or pneumonia. METHODS/RESULTS: Molecular epidemiology studies have shown that field strains exhibit sufficient sequence diversity to designate 2 subgroups of circulating viruses. Small animal and nonhuman primate models of infection h

Crack, sex, and STD.

Increasing rates of syphilis, gonorrhea, chancroid, and sexually transmitted human immunodeficiency virus infection appear to be related to crack cocaine use. This article critically reviews 16 epidemiologic studies that examine drug use, sexual behavior, and sexually transmitted disease (STD). Eight studies found an association between crack and STD, one study found no association between crack and STD, and seven studies found STD to be related to other drugs or methods of cocaine use. The exchange of sex for money or drugs was associated with STD in seven studies. Publications that were revi

Epidemiologic designs for the study of acquired immunodeficiency disease: options and obstacles.

Epidemiologic methods are designed to identify risk factors involved in production of a particular disease, even when the specific etiologic agent is unknown. However, a major problem currently presenting obstacles to research on the acquired immunodeficiency syndrome (AIDS) relates to classification; approaches are described that will permit reclassification when better laboratory techniques are developed or when more information is available from studies on natural history. Further descriptive studies will be valuable in the determination of whether extension of disease has occurred to new p

Risk for human immunodeficiency virus (HIV) infection among persons with severe mental illnesses.

Individuals diagnosed with a severe mental illness are at significantly enhanced risk for infection with the human immunodeficiency virus (HIV). To better understand elevated seroprevalence in this population, we review the research literature that has investigated HIV-related risk behavior among adults who have a severe and persistent mental illness. This review indicates that 54%-74% of adults report that they have been sexually active in the last year with approximately one third reporting two or more partners. Among those who were sexually active, condom use was inconsistent. A significant

Basis for an FCE methodology for patients with work-related upper limb disorders.

A reported reduction in work-related functional capacity in Work-related Upper Limb Disorders (WRULD) patients is among the most common problems in WRULD. The extent to which this reduction in functional capacity can be objectified remains unknown. A validated instrument to test functional capacity in this patient group is unavailable. The objective of this study was to design a Functional Capacity Evaluation (FCE) for WRULD patients working with Visual Display Units (VDU) and provide evidence for content validity. A review to epidemiological literature was conducted to identify physical risk

Proposal and Initial Validation of a Brief Questionnaire for Epidemiological Assessment of Low Back Pain.

BACKGROUND: Low back pain (LBP) is one of the most prevalent pain conditions worldwide and a major contributor to disability. Although numerous epidemiological studies have investigated LBP, heterogeneity in pain definitions and assessment indicators has limited comparability across studies. A brief and reproducible questionnaire incorporating core LBP indicators commonly used in pain epidemiology may facilitate consistent assessment of LBP occurrence and impact in population-based research. METHODS: We proposed a brief low back pain questionnaire (B-LBPQ) designed to assess the occurrence an

Methods for modeling gene-environment interplay using polygenic risk scores.

Polygenic risk scores (PRS) are increasingly recognized as pivotal tools for quantifying disease risk through the aggregation of multiple genetic variants. As sample sizes in genome-wide association studies (GWAS) continue to expand and PRS become more powerful, they are set to play a key role in translational research and personalized medicine. Understanding the interplay of PRS with environmental factors is critical for interpreting and applying PRS in diverse contexts. This interplay manifests in two forms: PRS-by-environment interaction (PRS × E) and gene-environment correlation (rGE). How

Towards a European Registry and Biorepository for Patients with Spinal and Bulbar Muscular Atrophy.

Pathomechanisms of spinal and bulbar muscular atrophy (SBMA) have been extensively investigated and are partially understood, but no effective treatment is currently available for this disabling disorder. Its rarity, the slow disease progression, and lack of sensitive-to-change outcome measures render design and conduction of clinical trials a challenging task. Therefore, it is fundamental to strengthen the network of clinical centers interested in SBMA for clinical trial readiness. We propose to create and maintain an International SBMA Registry where as many well-characterized patients as po

New and classic insights into presumed ocular histoplasmosis syndrome and its treatment.

PURPOSE OF REVIEW: Presumed ocular histoplasmosis syndrome classically presents with atrophic choroidal scars, which are thought to progress into choroidal neovascularization in a small proportion of patients. The pathophysiology of the disease, including its underlying etiology, continues to be controversial and subject to ongoing research. Even more important is the controversy that surrounds the optimal treatment for choroidal neovascularization in patients with presumed ocular histoplasmosis, in particular that of subfoveal localization. RECENT DEVELOPMENTS: Intense efforts oriented towar

Pharmacotherapy for self-injurious behavior: preliminary tests of the D1 hypothesis.

The D1 dopamine hypersensitivity model of self-injurious behavior leads to a testable clinical hypothesis: the mixed D1/D2 dopamine antagonist fluphenazine may improve the symptoms of self-injurious patients. The hypothesis was tested in an open pilot trial in six patients and a partially controlled trial in nine patients. Some degree of clinical improvement was observed in 11 of the 15 patients. The trials represent a partial affirmation of the D1 hypothesis. However, it is also clear that conventional methodology for psychopharmacologic research is inappropriate for the proper clinical evalu
