Inflammatory bowel disease and prostate cancer: from epidemiological controversy to mechanistic hypotheses.

Inflammatory bowel disease (IBD) is a chronic systemic inflammatory disorder with rising global prevalence. Epidemiological evidence linking IBD to prostate cancer (PCa) risk remains conflicting. Observational cohorts suggest a possible elevated risk, though this remains uncertain and may be influenced by detection bias. Mechanistically, chronic intestinal inflammation may indirectly promote prostate carcinogenesis through systemic cytokines, immune cell infiltration, and activation of the NF-κB/STAT3/AKT signaling pathway, coupled with oxidative stress and DNA damage. The gut-prostate axis, d

Impact of confounding by smoking on cancer risk estimates in cohort studies of radiation workers: a simulation study.

Previous studies on cohorts of radiation workers have provided valuable insights into the effects of low-dose-rate radiation; however, some concerns regarding the potential confounding effects of smoking have been expressed. Although some studies have collected smoking data and adjusted for this variable, their limited numbers and the presence of other confounders obscure the extent of the impact of smoking on their results. To address this, we conducted a simulation study to quantitatively evaluate the bias from confounding and modeling conditions, similar to actual epidemiological studies. O

Cataract Induced by Glucocorticoids.

Glucocorticoids (GCs) remain a cornerstone therapy for noninfectious uveitis and autoimmune disorders; however, chronic administration is strongly associated with sight-threatening complications, particularly glucocorticoid-induced cataracts (GIC). This comprehensive review synthesizes current evidence on the molecular pathogenesis, epidemiological patterns, and clinical management of GIC. Epidemiological analyses indicate that over 50% of patients receiving systemic corticosteroids for >60 days develop ocular complications, with cataract formation (36%) and glaucoma (16%) representing the pre

Epidemicity of cholera spread and the fate of infection control measures.

The fate of ongoing infectious disease outbreaks is predicted through reproduction numbers, defining the long-term establishment of the infection, and epidemicity indices, tackling the reactivity of the infectious pool to new contagions. Prognostic metrics of unfolding outbreaks are of particular importance when designing adaptive emergency interventions facing real-time assimilation of epidemiological evidence. Our aim here is twofold. First, we propose a novel form of the epidemicity index for the characterization of cholera epidemics in spatial models of disease spread. Second, we examine i

The scrub typhus in mainland China: spatiotemporal expansion and risk prediction underpinned by complex factors.

In mainland China, a geographic northward expansion of scrub typhus has been seen, highlighting the need to understand the factors and identify the risk for disease prevention. Incidence data from 1980 to 2013 were used. A Cox proportional hazard model was used to identify drivers for spatial spread, and a boosted regression tree (BRT) model was constructed to predict potential risk areas. Since the 1980s, an invasive expansion from South Natural Foci towards North Natural Foci was clearly identified, with the epidemiological heterogeneity observed between two regions, mainly in spatial distri

Statins to treat Alzheimer's disease: an incomplete story.

The link between cholesterol and Alzheimer's disease (AD) has been explored for almost two decades. The link stems from the observation that atherosclerotic heart disease increases the risk for AD and that people expiring from coronary artery disease had AD changes in their brains. Cholesterol is a cofactor of amyloid deposition, with substantial evidence showing that high cholesterol diets in animal models can accelerate amyloidogenesis. This link led investigators to posit the use of cholesterol-lowering agents as treatments for AD and cognitive decline. Indeed, the epidemiological data sugg

A proposed comprehensive model of care for inflammatory bowel disease in Poland.

Epidemiological data of the Polish healthcare model of care indicate that the current management of inflammatory bowel disease (IBD) requires improvements in clinical practice and service organization. Integrated care models for patients with IBD, particularly those centred on a multidisciplinary team, offer a promising approach to improving outcomes, reducing admissions, and rationalizing resources. The coordination of specialists, nurses, dietitians, psychologists, and other professionals within a single reference unit could strengthen continuity of care with primary care providers and enabl

What can we learn from Fli1-deficient mice, new animal models of systemic sclerosis?

Systemic sclerosis is a complex multifactorial disease characterized by autoimmunity, vasculopathy, and selective organ fibrosis. A series of genetic and epidemiological studies have demonstrated that environmental influences play a central role in the onset of systemic sclerosis, while genetic factors determine the susceptibility to and the severity of this disease. Therefore, the identification of predisposing factors related to environmental influences would provide us with an informative clue to better understand the pathological process of this disease. Based on this concept, the deficien

The impact of strain-specific immunity on Lyme disease incidence is spatially heterogeneous.

Lyme disease, caused by the bacterium Borrelia burgdorferi, is the most common tick-borne infection in the US. Recent studies have demonstrated that the incidence of human Lyme disease would have been even greater were it not for the presence of strain-specific immunity, which protects previously infected patients against subsequent infections by the same B. burgdorferi strain. Here, spatial heterogeneity is incorporated into epidemiological models to accurately estimate the impact of strain-specific immunity on human Lyme disease incidence. The estimated reduction in the number of Lyme diseas…

Fitness and virulence costs of Candida albicans FKS1 hot spot mutations associated with echinocandin resistance.

The identification of FKS1 mutations in Candida albicans associated with echinocandin resistance has raised concerns over the spread of drug-resistant strains. We studied the impact of fks1 mutations on C. albicans virulence and fitness. Compared with wild-type strains for FKS1, echinocandin-resistant C. albicans strains with homozygous fks1 hot-spot mutations had reduced maximum catalytic capacity of their glucan synthase complexes and thicker cell walls attributable to increased cell wall chitin content. The fks1 mutants with the highest chitin contents had reduced growth rates and impaired

Reframing Optimal Control Problems for Infectious Disease Management in Low-Income Countries.

Optimal control theory can be a useful tool to identify the best strategies for the management of infectious diseases. In most of the applications to disease control with ordinary differential equations, the objective functional to be optimized is formulated in monetary terms as the sum of intervention costs and the cost associated with the burden of disease. We present alternate formulations that express epidemiological outcomes via health metrics and reframe the problem to include features such as budget constraints and epidemiological targets. These alternate formulations are illustrated wi

A dynamic, climate-driven model of Rift Valley fever.

Outbreaks of Rift Valley fever (RVF) in eastern Africa have previously occurred following specific rainfall dynamics and flooding events that appear to support the emergence of large numbers of mosquito vectors. As such, transmission of the virus is considered to be sensitive to environmental conditions and therefore changes in climate can impact the spatiotemporal dynamics of epizootic vulnerability. Epidemiological information describing the methods and parameters of RVF transmission and its dependence on climatic factors are used to develop a new spatio-temporal mathematical model that simu

[Zika virus : from research to vaccine].

Zika virus has recently emerged as new teratogenic agent. Research is drastically increasing to allow a better comprehension of the role of this emerging virus in the induction of fetal cerebral anomalies. Several epidemiological and microbiological aspects may explain a higher virulence of the current strain. Various animal models have been developed and confirm the materno-fetal transmission as well as the induction of cerebral development's disorders. In parallel, various companies are working to develop a vaccine and antiviral medicine against Zika virus. Despite promising results, the roa

[Vitamin A: an indirect factor in the prevention of cancer].

Although vitamin A intervenes essentially in the mechanism of sight, its possible role in the prevention and treatment of cancer is widely discussed. Epidemiological studies have demonstrated a correlation between vitamin A deficiency and an increase in the frequency of cancers in man. Several hypotheses have been put forward concerning the mechanism of interaction between vitamin A and cancer on the basis of experimental models showing that vitamin A exerts a protective action against spontaneous or induced neoplasias in animals. The toxicological aspects of vitamin A administration and the r

Density-dependent fecundity in Schistosoma mansoni infections in man.

An analysis is presented of autopsy data collected by Cheever in 1968 on the association between Schistosoma mansoni worm burdens and faecal egg counts. Significant negative relationships are found between the number of eggs/worm pair/g faeces and both the number of worm pairs and the total number of worms. A general, non-linear, statistical routine is shown to be the most practical method of fitting non-linear models to the observed density-dependent fecundity relationship. The conceptual and practical implications of density-dependence are discussed in the context of epidemiological field st

Exposure measurement error in air pollution studies: A framework for assessing shared, multiplicative measurement error in ensemble learning estimates of nitrogen oxides.

BACKGROUND: Increasingly ensemble learning-based spatiotemporal models are being used to estimate residential air pollution exposures in epidemiological studies. While these machine learning models typically have improved performance, they suffer from exposure measurement error that is inherent in all models. Our objective is to develop a framework to formally assess shared, multiplicative measurement error (SMME) in our previously published three-stage, ensemble learning-based nitrogen oxides (NOx) model to identify its spatial and temporal patterns and predictors. METHODS: By treating the e

A Bayesian Framework for the Network Analysis of Transmission Dynamics in Infectious Disease.

Understanding the transmission dynamics of infectious diseases is critical for effective public health intervention. Traditional models often rely on simplifying assumptions that overlook the complexity of real-world contact patterns. In this study, we present an extended Bayesian framework that integrates genomic, temporal, and network data to reconstruct transmission networks with greater accuracy. By incorporating network structure as a prior, the model accounts for social and spatial proximity, allowing transmission probabilities to vary with contact or social distance. We further enhance
