Pulmonary mucormycosis as a presentation of COVID 19: Case series.

BACKGROUND: Pulmonary Mucormycosis (PM) is a relatively uncommon fungal disease, usually manifested in immunocompromised patients. It has an aggressive course, along with dilemmas in diagnosis and treatment. In view of the surge of Mucormycosis patients in COVID 19 pandemic, clinicians need to consider PM in suspected cases, and act in an expedited manner to avoid misdiagnosis and initiate prompt treatment. CASE PRESENTATION: In this case series, we present four cases of PM with varied presentation, clinical course and discuss management strategies. CONCLUSIONS: A strong suspicion of PM base

Shock Teams: A Contemporary Review.

PURPOSE OF REVIEW: Cardiogenic shock (CS) is a time-sensitive and often fatal condition. To address this issue, many centers have developed multidisciplinary shock teams with a common goal of expediting the recognition and treatment of CS. In this review, we examine the mission, structure, implementation, and outcomes reported by these early shock teams. RECENT FINDINGS: To date, there have been four observational shock team analyses, each providing unique insight into the utility of the shock team. The limited available data supports that shock teams are associated with improved CS mortality

Development of a minimally invasive surgical guide template for flapless extraction of deeply impacted premolars: A technical note and case report.

The emergence of digital surgical guide templates in alveolar surgery has rapidly increased in the past decade, coinciding with the advancements in 3D printing technology. In contrast to conventional freehand procedures, the utilization of digital templates serves as a 'bridge' that facilitates the extraction of impacted teeth with expedited and accurate intraoperative localization, resulting in a notable reduction in surgical duration, minimized trauma, and lowered risk. However, there is significant scope for enhancements in surgical techniques and refinement of surgical guide templates. The

Reactivation of Herpes Zoster After Recombinant Vaccine (Shingrix): A Case Report.

Herpes zoster (HZ) is a common contagious dermatological condition that results from reactivation of varicella-zoster virus (VZV), which currently could be prevented by vaccination. We describe a rare case of varicella infection reactivation after routine zoster vaccination in an immunocompetent female in her 60s who developed dermatomal pruritic and vesicular rash one week after receiving Shingrix vaccine, along with fever, sweating, headache, and fatigue. The patient was treated as a case of herpes zoster reactivation with a seven days course of acyclovir. She continued to do well on follow-

Admission respiratory status predicts mortality in COVID-19.

COVID-19 has significant case fatality. Glucocorticoids are the only treatment shown to improve survival, but only among patients requiring supplemental oxygen. WHO advises patients to seek medical care for "trouble breathing," but hypoxemic patients frequently have no respiratory symptoms. Our cohort study of hospitalized COVID-19 patients shows that respiratory symptoms are uncommon and not associated with mortality. By contrast, objective signs of respiratory compromise-oxygen saturation and respiratory rate-are associated with markedly elevated mortality. Our findings support expanding gui

ED and Inpatient Management of Headache in Children and Adolescents.

PURPOSE OF REVIEW: Pediatric migraine is common, and appropriate abortive treatment is important to limit impact on school performance and mental health. This review will describe the latest evidence for abortive treatment in the emergency department and inpatient settings. RECENT FINDINGS: It is recognized that a protocol for emergency department treatment can increase efficacy and prevent admissions. These protocols commonly include a non-opioid analgesic and dopamine receptor antagonist. A novel approach to treatment with valproic acid is use of a continuous infusion. Administration of ket

Successful treatment of painful chronic wounds with amniotic and umbilical cord tissue: A case series.

Patients with chronic wounds may experience persistent, debilitating pain that cannot be adequately managed with analgesics and that negatively impacts their quality of life. In this case series, three painful chronic and ischemic wounds that were caused by polyarteritis nodosa vasculitis (n = 1) and peripheral arterial disease (PAD) (n = 2) were successfully treated with cryopreserved umbilical cord tissue and/or amniotic membrane and umbilical cord particulate, resulting in notable reduction in pain within 7 days followed by expedited wound closure. No adverse events related to these tissue

Sexually Transmitted Infections: Part I: Genital Bumps and Genital Ulcers.

Sexually transmitted infections (STIs) affect young people in a disproportionate way, with more than half of the infections occurring in 15- to 25-year-olds, although as an age group they constitute only 25% of the sexually active population. Pediatricians should be familiar with the social, behavioral, and biological factors that predispose adolescents to STIs. Preventive visits for teens and pre-teens should incorporate education and counseling about sexuality, safe sexual behavior, and STIs. Pediatricians should be able to identify, diagnose, and manage STIs presenting as genital "bumps" an

Left ventricular (LV) Cardiac penetrating trauma is a rare and grave injury. In cases of penetrating cardiac trauma, pre-hospital Ultrasound by flight doctors can assist identify specific pathology. This pre-hospital triage has now been linked to a change in both pre-hospital and in-hospital management. There are minimal cases reported where Pre-Hospital ultrasound provided definitive diagnosis and, while providing Pre-Hospital blood transfusion, informed a direct to theatre approach. In 2017 in New South Wales, Australia, a new protocol "Code Crimson" has been introduced to formalise a system

Computed tomography and magnetic resonance imaging of peripelvic and periureteric pathologies.

Peripelvic and periureteric areas are frequently overlooked in the imaging evaluations of the urinary system on computed tomography and magnetic resonance imaging. Several neoplastic and non-neoplastic disorders and diseases including lipomatosis, angiomyolipoma and angiolipomatous proliferation, vascular lesions, lymphangiomatosis, Rosai-Dorfman disease, Erdheim-Chester disease, extramedullary hematopoiesis, IgG4-related disease, lymphoma, mesenchymal tumors, trauma, and Antopol-Goldman lesion may involve these areas. Ulnar neuropathy: evaluation and management.

Ulnar neuropathy is commonly encountered, both acutely after elbow trauma and in the setting of chronic compression neuropathy. Careful clinical evaluation and discerning evaluation of electrodiagnostic studies are helpful in determining the prognosis of recovery with nonoperative and operative management. Appreciation of the subtleties in clinical presentation and thoughtful consideration of the timing and type of surgical intervention are critical to optimizing outcomes after treatment of ulnar neuropathy. The potential need for decompression at both the cubital tunnel and Guyon's canal must

Gastrointestinal Complications in Patients Who Undergo Radical Cystectomy with Enhanced Recovery Protocol.

Gastrointestinal (GI) complications are among the most common complications following radical cystectomy and urinary diversion. The most common is postoperative ileus, although its precise pathophysiology is not completely understood. Enhanced recovery after surgery (ERAS) protocols include evidence-based steps to optimize postoperative recovery and shorten hospital stay, mainly through expedited GI function recovery. They include avoiding bowel preparation and postoperative nasogastric tube, early feeding, non-narcotic pain management, and the use of cholinergic and mu-receptor opioid antagon

Gastric volvulus in children: lessons learned from delayed diagnoses.

Gastric volvulus in infants, children, and adolescents is a rare event. The purpose of this study is to examine this congenital abnormality and its consequences, using cases with significant adverse outcomes. We will specifically discuss issues of diagnosis and treatment of acute gastric volvulus in association with diaphragmatic anomalies. In addition, the different types of gastric volvulus are compared and contrasted, highlighting areas that may allow for early recognition and prevention of complications related to this foregut obstructive process. Three clinical cases are reviewed, all wit

The fat embolism syndrome. A review.

While fat embolism occurs in most (more than 90%) patients with traumatic injury, the fat embolism syndrome (FES) occurs in only 3%-4% of patients with long-bone fractures. FES involves multiple organ systems and can cause a devastating clinical deterioration within hours. The major clinical features of FES include hypoxia, pulmonary edema, central nervous system depression, and axillary or subconjunctive petechiae. Improvements have been made in supporting the respiratory compromise and adult respiratory distress syndrome that these patients develop. Aggressive measures to improve the pulmona

Urgent versus scheduled resection in high-grade glioma: patient selection and healthcare utilization in a single-center cohort.

PURPOSE: Patients with high-grade glioma reach surgery during the initial, index hospitalization (urgent), or through scheduled/outpatient pathways. There is a paucity of literature examining what patients require expedited versus elective surgery. This single-center retrospective study describes complications, utilization, and survival by pathway. METHODS: Adults with WHO grade 3-4 gliomas undergoing primary resection (2021-2026) were classified as urgent (admitted without a planned resection date) or scheduled (discharged and readmitted for planned surgery, or managed exclusively as an outp

Better way: initial acceptability testing of using artificial intelligence tools to accelerate development of trauma clinical guidance.

INTRODUCTION: Representatives of the trauma community have voiced a need for a new approach to developing clinical guidance. In this study, we test the initial acceptability of a proposed 12-step approach that aims to reduce the current clinical guidance timeline from more than 24 months to 24 weeks. METHODS: Investigators hypothesized that artificial intelligence (AI) tools could be leveraged to improve and make the process of clinical guidance development more efficient, facilitating AI initial output that could later be reviewed by subject matter experts (SMEs), ensuring ethical standards

Beyond the STEMI paradigm: Prevalence, predictors, and angiographic signatures of Wellens, de Winter, and Aslanger patterns in acute coronary syndromes.

OBJECTIVE: Several high-risk electrocardiographic (ECG) phenotypes in acute coronary syndrome (ACS)-Wellens, de Winter, and Aslanger-signal critical ischemia or occlusion despite absent diagnostic ST-segment elevation myocardial infarction (STEMI) and would benefit from expedited reperfusion. Data on their joint estimates are scarce. The objectives were: (1) to quantify their prevalence; and (2) to compare their baseline characteristics and angiographic vessel-burden. METHODS: In this prospective single centre observational study, 1223 consecutive ACS patients undergoing invasive coronary ang

Enhancing Clinical Documentation and Management of Cauda Equina Syndrome: The Development and Impact of a Structured Assessment Proforma.

Background Cauda equina syndrome (CES) is a neurosurgical emergency associated with potentially devastating neurological sequelae and significant medico-legal implications. Timely surgical decompression is critical; however, effective management relies on precise documentation of clinical findings. In many settings, documentation of key neurovascular assessments remains suboptimal. This study aimed to evaluate and improve the quality of clinical documentation in suspected CES cases by implementing a structured assessment proforma, thereby facilitating timely and accurate clinical evaluation, e
