Management of a proximal pelvic limb skin laceration in a dog using a skin flap and an external fixator.

A nine-year-old, spayed female crossbred dog sustained a traumatic skin lesion to the right proximal pelvic limb, extending to the extensor surface of the stifle. Surgical management of the wound included primary closure using a body skin flap fashioned from the right caudolateral abdominal wall. To achieve closure and prevent tension on the wound, the coxofemoral and stifle joints on the affected side needed to be immobilised in flexion. Flexion immobilisation of the stifle was achieved using the anatomical principle of muscle reciprocation. The ipsilateral tarsal joint was immobilised in fle

[Change in procedure to reamed intramedullary nail in diaphyseal femoral fractures after stabilization with external fixator].

Due to the special pathophysiological condition of the polytraumatized patient, intramedullary nailing of the femur is not generally recommended. Fracture treatment by traction is not suitable because of the poor intensive care and the persisting instability. The external fixator is able to stabilize the fracture without additional burden to the patient. The stability is sufficient for the first set of treatment. Stabilization requires a changing procedure to an internal osteosynthesis. It is possible to reduce the danger of pin-tract infections with a differentiated concept. The principle of

Use of a type-III trilateral external skeletal fixation device in three deer with comminuted fractures.

A type-III trilateral external skeletal fixation device was used to repair comminuted fractures of metacarpal and tibial bones in 3 deer. Mean weight of the deer was 25 kg, and mean age was 4 months. Mean interval from application until removal of the device was 6.5 weeks. All deer had complete function of their previously fractured limbs and had minimal complications during healing. Several factors should be considered when choosing a method of repair for wildlife with fractured limbs. Economic constraints and demeanor of the animal may limit the number of options. Analysis of results of our

Results of combined internal and external fixation for the treatment of severe AO-C3 fractures of the distal radius.

Combined internal and external fixation for distal radius fractures has been previously reported, but the indications, technique, and results are not defined. This study reviews an experience with a specific surgical strategy for the treatment of a homogeneous population of AO-C3 fractures of the distal radius. The technique is composed of combined palmar and dorsal surgical approaches for open reduction and internal fixation of fracture fragments after neutralizing the wrist with a rigid external fixator. Thirteen fractures in 12 patients were followed an average of 27 months. Wrist motion av

Treatment of complex fractures of the distal radius. Combined use of internal and external fixation and arthroscopic reduction.

A group of fractures of the distal radius are inherently unstable and cannot be treated by closed reduction and cast or external fixation. These fractures are high-energy motor vehicle accidents often seen in young adults but possible in nearly every age group. They are not typical Colles's fractures because they are intra-articular with displacement of the joint articular surfaces. The goals of treatment are to obtain an anatomic reduction and then to proceed with treatment techniques to maintain the reduction by whatever methods are available or necessary within the experience of the treatin

External fixation as an alternative when treating 2nd and 3rd degree open lower leg fractures.

The infection rate of open lower leg fracture is extremely high. Surgical treatment guarantees a reduced risk of infection when compared with conservative treatments. But even osteosynthetic methods such as screw or compression plate fixation show unsatisfactory results because of the additional traumatization of the primarily injured soft tissue combined with a reduction of blood circulation. From July 1973 till September 1976 we treated 57 2nd and 3rd degree open shank fractures with osteosynthetic methods: 46 with compression plate osteosynthesis and 11 primarily or secondarily with externa

Feasibility of Virtual Reality Immersion in Reducing Pain and Anxiety during External Fixator Care Procedures.

INTRODUCTION: External fixator care after limb reconstruction often involves pin-site cleaning and frame adjustments that can be painful and anxiety-provoking, particularly for paediatric patients. Virtual reality (VR) immersion may support procedural coping. This feasibility study evaluated the practicality and tolerability of VR during fixator care and explored pain and anxiety trends. PATIENTS AND METHODS: A prospective, randomised feasibility study enrolled 29 patients aged 5-21 years undergoing limb lengthening or reconstruction with external fixators. Participants were randomised to 'VR…

Development of a low-cost external fixation system: an off-label solution for resource-limited settings.

PURPOSE: External fixators are essential for the surgical management of long-bone fractures, particularly in resource-limited settings where access to commercial systems is often restricted by cost and logistics. This study aimed to develop a low-cost external fixation concept using widely available construction materials and evaluate its biomechanical performance under axial loading. METHODS: An external fixator composed of M10 threaded rods, washers, and nuts was developed. Five configurations were tested: SP (Steinmann pins), KT (threaded K-wires), KS (smooth K-wires), and BKS (bilaterally…

Soft Tissue Complications Following Temporary External Fixation vs Splinting of Trimalleolar Ankle Fractures.

BACKGROUND: Temporary management of closed trimalleolar ankle fractures may involve closed reduction and splint application vs external fixation (ex-fix). One of the primary concerns of using splinting is loss of reduction and soft tissue compromise. The objective of this study was to determine if there is a clinically significant difference in short-term soft tissue complications (STCs) in patients with trimalleolar or trimalleolar-equivalent ankle fractures treated with temporary closed reduction and splinting vs ex-fix. METHODS: In this multicenter retrospective review from 2 level 1 traum

A Cost-Effectiveness Analysis of Intramedullary Nailing Versus External Fixation for Open Tibial Fractures in Tanzania.

BACKGROUND: Open tibial fractures are a cause of substantial orthopaedic morbidity in low- and middle-income countries. These injuries represent a substantial cost burden to both individual patients and society because of their high propensity for complications, such as infection, nonunion, and malunion. External fixation and intramedullary (IM) nailing are both utilized for definitive treatment of open tibial fractures, but given the differences in cost and lack of clear superiority of intramedullary nailing, cost-effectiveness becomes important to consider in low- and middle-income countries
