Penetrating injuries of the face.

We reviewed 78 consecutive cases of penetrating facial injuries treated at Ben Taub General Hospital in Houston, Texas, between 1992 and 1994, and we analyzed injury patterns on the basis of (1) the mechanism of injury, and (2) the entry zone of the wounds. We found that gunshot wounds were more likely to require emergent airway establishment than shotgun wounds or stab wounds (p = 0.03). We noted a higher prevalence of globe injury among shotgun wounds than among gunshot wounds (p = 0.02). Nine (12%) patients had intracranial penetration of a bullet or shotgun pellet. Patients with gunshot wo…

Changing patterns in the epidemiology and treatment of zygoma fractures: 10-year review.

A ten year retrospective review of 259 zygoma fractures is presented to highlight changes in epidemiology and treatment. Motor vehicle-related trauma resulted in a majority of the injuries (80.6%), with a high incidence of multiple facial fractures (43.2%). The number of zygomatic and other facial fractures decreased over the duration of the study (by 50.0% and 20.1%, respectively, p < 0.05), perhaps reflecting lowered speed limits and the increased use of seat belts. The proportion of fractures receiving open reduction and internal fixation (ORIF) remained relatively constant (46.3%). At pres

[Classification and qualification of nose injuries--clinical and forensic aspects].

INTRODUCTION: Nose injuries are of great importance in ENT and medico-legal practice. The importance lies in the frequency of injuries, diverse approaches to diagnosis, treatment and qualification of injuries used in legal proceedings in court. Objective assessment of health impairements due to an injury, requires knowledge about morphofunctional nasal characterics, therapeutic procedures and medico-legal expertise. NOSE INJURES: ETIOLOGY AND DIAGNOSIS: Nasal fractures are the most common types of facial fractures which occur in different situations. They are often combined with soft tissue i

Pathophysiology of facial nerve paralysis induced by herpes simplex virus type 1 infection.

Herpes simplex virus type 1 (HSV-1) has been proven to be a cause of Bell's palsy; however, the underlying pathophysiology of the facial nerve paralysis is not fully understood. We established a mouse model with facial nerve paralysis induced by HSV-1 infection simulating Bell's palsy and investigated the pathophysiology of the facial nerve paralysis. The time course of the R1 latency in the blink reflex tests paralleled the recovery of the facial nerve paralysis well, whereas electroneurographic recovery tended to be delayed, compared to that of the paralysis; these responses are usually seen

[Examination of Bell's palsy in consideration of the Austrian private accident insurance].

In western Europe peripheral facial palsy is the most common cranial nerve disorder. The constitutional palsy may be ideopathic or combined with other disorders, above all Lyme's disease and otogenic zoster. The traumatic palsy is nearly always combined with other severe injuries and above all caused by accidents.Constitutional palsies have a less degree of partial recovery compared to the traumatic forms. The expert has to evaluate the actual status of palsy at the time of examination and also to consider prognostic outcome. The Austrian private accident insurance covers the risk of injuries

Complications associated with segmentation of the maxilla: a retrospective radiographic follow up of 82 patients.

The aim of the present study was to retrospectively examine the clinical and radiographic incidence and frequency of injuries to teeth and their surrounding tissues adjacent to interdental osteotomies in conjunction with segmentation of the maxilla. Vertical interdental osteotomies have been performed in combination with Le Fort I osteotomy for correction of various dento-facial deformities. All our orthognathic patients are followed prospectively in a standardized manner. Eighty-two consecutive patients who underwent dento-facial correction by segmented maxillary osteotomy alone or in combina

Facial paralysis in longitudinal temporal bone fractures: a review of 26 cases.

The management of facial nerve injury resulting from temporal bone fracture continues to be a topic of discussion. The indications for surgery as well as the proper timing and extent of surgical exploration are the principal questions debated. This study reviews a large series of patients (26), treated for facial paralysis following a longitudinal temporal bone fracture. The most frequent site of injury was the area of the geniculate ganglion. The injury to the facial nerve was often severe. We recommend early exploration in these cases when electrical studies indicate a severe injury. In most

Neglected orbitozygomaticomaxillary fractures with complications: A case report.

INTRODUCTION: Zygomatic complex fracture is one of the most frequently occuring injuries to facial structures and often involves damage to the surrounding structures, including the orbital floor and/or medial orbital wall. These complicated injuries can cause both serious opthalmic and aesthetic complcations that make reconstructions more difficult. CASE PRESENTATION: A 22 year old male presented with facial asymmetry. On examination, there were neglected fractures of left orbitozygomaticomaxillary complex with various compications. We performed lateral orbitotomy, bone graft, medial and late

Simultaneous bilateral facial palsy as a result of isolated mandibular fractures.

Post-traumatic bilateral facial nerve palsy is usually associated with fracture of the temporal bones in patients who have sustained a significant head injury. Facial nerve injury can also very rarely occur because of isolated mandibular fractures, without associated temporal bone fractures. Here is reported a case of bilateral facial nerve palsy in a patient with mandibular symphysis and bilateral condylar fractures of the mandible without evidence of temporal bone fracture or significant head injury.

A review of paediatric maxillofacial injuries--a hospital-based study.

This study describes the patient profile with maxillofacial injuries attending a trauma unit at a children's hospital. A retrospective record-based analysis was carried out of children (less than 13 years of age) presenting with maxillofacial injuries from January 1991 to December 2001. Of the almost 90,000 trauma records, 13% of children presented with maxillofacial injuries. 60% were boys, with a quarter less than 3 years of age. Most presented with superficial lacerations and abrasions. The face (other than the facial bones) and the mouth/oropharnyx were most often injured. Injuries were ma…

Post-traumatic facial nerve paralysis: three cases of delayed temporal bone exploration with recovery.

No consensus exists today on the management of immediate-onset post-traumatic facial nerve paralysis. Controversy surrounds the timing of surgical intervention and the role of electrophysiologic testing. Three patients are presented who sustained immediate, complete facial paralysis following closed head trauma. They did not have prompt facial nerve decompression. In each case, electroneurography and electromyography showed complete nerve degeneration and denervated muscle. Despite the results of the electrophysiologic tests, all patients underwent late surgical decompression of the nerve: one

Nerve injuries in neonates.

A clinical study of 40 cases of nerve injuries in neonates with one-year follow-up is presented. The total incidence of such injuries was 1.50%. Thirty-three cases sustained nerve injury due to obstetrical trauma and in 7 cases it was due to postnatal trauma like intragluteal injection and tight splintage. Almost in all cases there was history of prolonged or obstructed labour. Upper limb nerve injuries were commonest followed by facial palsy. Twenty-three cases (76%) showed complete recovery while 17% had partial and 7% no recovery at all. Hence nerve injuries are not uncommon in neonates. If

Surgical Reconsideration of Traumatic Facial Paralysis.

INTRODUCTION: Despite the different pathophysiological mechanisms underlying Bell's palsy, in assessing severe traumatic facial paralysis, many surgeons rely on electrophysiological criteria to determine whether facial nerve exploration is warranted. To assess the value of preoperative electroneurography (ENoG) and the time of surgery, we analyzed data from three tertiary medical centers. MATERIALS AND METHODS: The records of 517 patients with a degenerative ratio (DR) greater than 80% on ENoG were collected, and two groups were defined: group A (90% DR ≤ ENoG) and group B (80% DR ≤ ENoG < 90
