[Application of groin flap combined with medial plantar artery perforator flap for degree Ⅲ- Ⅳ defects of multiple fingers].

OBJECTIVE: To investigate the effectiveness of groin flap combined with medial plantar artery perforator flap (MPAP) for degree Ⅲ-Ⅳ defects of multiple fingers. METHODS: Between January 2018 and June 2019, 12 patients with degree Ⅲ-Ⅳ defects of multiple fingers caused by crushing were admitted. There were 9 males and 3 females with a median age of 29 years (range, 16-42 years). The mean interval between the injury and admission was 3 hours (range, 1-9 hours). The injured fingers of 7 cases were index and middle fingers, 4 cases were middle and ring fingers, and 1 case was index, middle, and r

Three-dimensional structure of a protein from scorpion venom: a new structural class of neurotoxins.

The three-dimensional crystal structure of variant-3 toxin from the scorpion Centruroides sculpturatus Ewing has been determined at 3 A resolution. Phases were obtained by use of K2PtCl4 and K2IrCl6 derivatives. The most prominent secondary structural features are two and a half turns of alpha-helix and a three-strand stretch of antiparallel beta-sheet, which runs parallel to the alpha-helix. The helix is connected to the middle strand of the beta-sheet by two disulfide bridges; a third disulfide bridge is located nearby. Several loops extend out of this dense core of secondary structure. The

Post-traumatic malunion of the proximal phalanx of the finger. Medium-term results in 24 cases treated by "in situ" osteotomy.

We report the clinical and radiographic medium-term results obtained for 20 patients (24 fingers) treated surgically for post-traumatic malunion of the proximal phalanx of the finger. In all cases we performed a corrective osteoclasia or osteotomy at the site of malunion, followed by miniplate and screw fixation or by screw fixation only. The corrective osteoclasia was performed when malalignment was addressed within six weeks after injury. Two patients who had two fractures underwent additional surgery (tenolysis and/or capsulolysis) to improve function and ROM. At the final follow-up, at a m

Diagnostic imaging in hypertrophic osteoarthropathy.

This review discusses our experience with conventional radiography and advanced imaging techniques in the evaluation of diverse soft tissue, skeletal and joint abnormalities frequently observed in patients with both the primary and secondary forms of hypertrophic osteoarthropathy. The manifestations include finger clubbing, elephant feet, cutis verticis gyrata, synovial effusion, periostosis, tuftal hypertrophy and acro-osteolysis. Its coexistence with other skeletal and joint diseases and its imaging differential diagnosis are also reviewed.

Closed Traumatic Zone 2 Flexor Tendon Injury of the Long Finger - A Case Report.

Closed rupture of the flexor digitorum profundus (FDP) tendon causes loss of flexion at the distal interphalangeal joint. Following trauma, these are known to present as avulsion fractures (Jersey finger) commonly in ring fingers. Traumatic tendon ruptures at the other flexor zones are seldom noted and are often missed. In this report, we present a rare case of closed traumatic tendon rupture of the long finger FDP at zone 2. Though it was missed initially, was confirmed with Magnetic Resonance Imaging and underwent successful reconstruction using an ipsilateral palmaris longus graft. Level of

Comminuted Transarticular Fracture of the Middle Phalanx: A Non-standard Surgical Procedure.

Fractures of the proximal interphalangeal (PIP) joint with fragment displacement should be promptly repaired after injury, though this does not ensure the return of pre-injury finger function. The most common surgical treatment involves open reduction and internal fixation (ORIF) or closed reduction and internal fixation (CRIF) using locking compression plate (LCP), intramedullary nails, or screws. In articular fractures of the hand and wrist, the surgical reduction of small fragments is generally less feasible to occur compared to long bone fractures. This article describes the case of a 58-y

Treatment of Extensor Lag Using the Flexor Digitorum Superficialis after Crushing-Penetrating Injury to the Metacarpophalangeal Joint.

We previously reported the beneficial effects of tendon transfer to eliminate extension lag of the interphalangeal joints, using the extensor carpi radialis longus prolonged by palmaris longus tendon grafts after crushing-penetrating injuries around the metacarpophalangeal (MP) joint of the middle finger. We used the flexor digitorum superficialis (FDS) as the alternative donor muscle and treated two cases of severe crushing injuries to MP joint, and then obtained good outcomes.

[Treatment of the swan neck deformity].

The pathogenesis and the anatomic causes for the swan neck deformity are reviewed. Many different soft tissue operations can succeed in preventing catastrophic loss of function if the deformity is treated while still in stage 1 or 2. In the later stages (3 and 4) restoration of finger is often not possible as a result of joint stiffness and irreversible joint changes. In this case, arthroplasty and arthrodesis should be considered.

Research and Development of a 3D-Printed Dynamic Finger Flexion Orthosis for Finger Extension Stiffness-A Preliminary Study.

Finger extension stiffness is a common post-traumatic complication that results in the hand's functional impairment. In clinical practice, a dynamic splint enables the patient to stretch the affected finger independently. However, current dynamic splints have drawbacks, such as limited stretching efficacy, and interfere with the hand's functional activities. Therefore, this study aimed to develop a dynamic finger flexion orthosis capable of stretching each finger joint using additive manufacturing (AM) technology, thereby enabling hand functional activity, and analyze the clinical improvement …

[Motor replacement operations in chronic ulnar nerve paralysis].

Irreparable damage to the motor function of the ulnar nerve results primarily in paralysis of the intrinsic muscles and reduces the functional capacity of the hand by about one-half. Tendon transfers are therefore indicated when the following grasp functions are impaired; (1) claw hand with dyskinetic finger movements; (2) impaired thumb adduction with limited key pinch power; (3) instability of the index MP joint; (4) adduction weakness of the little finger. The various procedures and transposition techniques are discussed and the author's preferred methods of tendon transfer are presented.

Etiologic factors in finger osteoarthritis.

Etiological factors in finger osteoarthritis are both local (single injury or repetitive joint use) and general. Epidemiological studies, twin studies, and studies of the ethnic influence have established that genetic factors play a role. The disease is more common in postmenopausal women, suggesting an influence of hormonal factors. Findings from studies of associations between finger osteoarthritis and osteoarthritis at other sites (spine, hips, knees), hyperostosis, and obesity, as well as the fact that the disease is less common in patients with osteoporosis and absent in hemiplegia, sugge

"Tethered" median nerve stress test in chronic carpal tunnel syndrome.

Tension of the median nerve produced by simultaneous extension of the supinated wrist and distal interphalangeal joint of the index finger was noted to result in proximal volar forearm pain radiation in patients with chronic carpal tunnel syndrome. This sign was less frequent in patients with a more acute syndrome. Adhesions between the median nerve and the overlying transverse carpal ligament and the development of a pseudoneuroma can individually or together occur in the chronic carpal tunnel syndrome limiting distal nerve excursion of the tethered nerve during simultaneous wrist and index f

Report of an unusual case of trigger finger secondary to phalangeal exostosis.

Trigger finger is commonly secondary to stenosing tenosynovitis. Space occupying lesions in the tendon bed, although uncommon, may prevent smooth tendon gliding. These include lipoma, anomalous muscle insertions, tumours of the tendon sheath and haemangiomas. We describe a patient who had triggering of the left middle finger at the proximal interphalangeal joint due to an exostosis blocking the flexor tendons gliding. Removal of the exostosis relieved the problem. The clinician must be aware that there are other causes for triggering. These may be identified with pertinent findings in the hist

Intramuscular tenotomy of flexor digitorum superficialis in the distal forearm after surgical excision of dupuytren's disease.

Contracture of the proximal interphalangeal joint after surgery to excise Dupuytren's disease, despite release of the contributory structures within the finger, can be caused by flexor digitorum superficialis (FDS) contracture. We describe five cases where FDS contracture was released by intramuscular tenotomy in the distal forearm. Standard postoperative therapy for Dupuytren's fasciectomy was used and clinical review showed improved finger extension with no loss of strength. We suggest that intramuscular tenotomy of FDS in the forearm can be used safely where indicated after excision of the

Lateral band translocation for swan-neck deformity: Outcomes of 41 digits after a mean follow-up of eight years.

BACKGROUND: Swan-neck deformity (SND) of the fingers can cause major functional impairment. The Zancolli-Tonkin procedure is a crossed dynamic tenodesis that prevents overextension of the proximal interphalangeal (PIP) joint and promotes extension of the distal interphalangeal (DIP) joint. We assessed the outcomes of this procedure in patients with SND due to various causes. HYPOTHESIS: The Zancolli-Tonkin procedure provides effective and stable correction of SND due not only to RA, but also to other conditions. PATIENTS AND METHODS: Consecutive patients managed at two centres between 2000 a

Multifocal giant cell tumors of tendon sheath along a single tendon: A case report with review of literature.

The giant cell tumor of tendon sheath (GCTTS) or nodular tenosynovitis arises as discrete solitary nodule in the tendon sheath near joints of toes and fingers. Multifocal giant cell tumor of tendon sheath is a rare entity, of which the etiology is not yet fully understood and it is different from diffuse type of GCTTS. Diffuse type of GCTTS occurs around large joints having a main mass from which a small satellite nodule may arise. Multifocal GCTTS along a single tendon is a more rare entity. Herein, we describe a case of multifocal GCTTS along the tendon sheath of flexor digitorum profundus t

Instructional Course Lecture. Arthrodesis of the interphalangeal joints with headless compression screws.

Interphalangeal arthrodesis is a reliable method of pain relief for arthritic proximal and distal interphalangeal joints in the fingers. Indications include osteoarthritis, acute trauma, chronic reconstruction for trauma, rheumatoid and other inflammatory arthritides, and at the distal interphalangeal joint, chronic mallet finger deformity and unreconstructible flexor tendon defects. Solid arthrodesis imparts stability to the digital skeleton. Headless compression screws can be reproducibly inserted and are a good method to provide fixation adequate to accomplish interphalangeal arthrodesis. S

Extra-articular fractures of the proximal phalanges of the fingers: a comparison of 2 methods of functional, conservative treatment.

PURPOSE: For nonsurgical treatment of fractures of the proximal phalanges of the triphalangeal fingers, different dynamic casts have been described. The main principle behind these casts is advancement and tightening of the extensor hood, caused by a combination of blocking the metacarpophalangeal joints in flexion and actively flexing the proximal interphalangeal joints. In contrast to established treatment protocols using functional forearm casts, the Lucerne cast allows for free mobilization of the wrist joint. The purpose of the current multicenter study was to compare the results of conse

Double cross finger flaps from the middle to the index or ring finger: A case series of 4 patients with an emphasis on donor finger morbidity.

INTRODUCTION: The use of two cross finger flaps from one digit has not been previously reported and the technique raises concerns regarding donor finger morbidity. In this paper, the authors report on a case series of double cross fingers flaps harvested from the middle finger to reconstruct large defects in the adjacent index or ring finger; with an emphasis on documenting morbidity in the donor middle finger. METHODS: A total of four cases of double cross finger flaps were retrospectively reviewed. Demographic data, surgery, and postoperative complications were documented. Donor middle fing
