Congenital infection with Plasmodium malariae: a rare case of intrauterine transmission in Germany.

BACKGROUND: Malaria remains the leading parasitic disease worldwide with a significant global morbidity and mortality burden. Plasmodium malariae, the least prevalent of the five Plasmodium species that cause human malaria, has unique characteristics including prolonged prepatent periods and life-long persistance. In non-endemic countries and particular in neonates with coexisting diseases diagnosis and therapy pose challenges. CASE PRESENTATION: We report a rare case of severe congenital P. malariae malaria in a 2-month-old female infant born in Germany to a Nigerian mother. The infant prese…

Current Capacity for Diagnosing Alzheimer's Disease in Germany and Implications for Wait Times.

BACKGROUND: Amyloid-targeting therapies for Alzheimer's disease (AD) might become available in Germany soon. The combination of a large pool of prevalent cases and a complex diagnostic process to determine eligibility for these treatments is likely to challenge health systems' capacity. OBJECTIVE: To analyze Germany's healthcare system capacity to identify treatment-eligible patients in a timely and equitable manner. METHODS: We modeled patients' diagnostic journey and projects wait times due to capacity constraints for AD specialist visits and PET scans from 2024 to 2043. Model parameters w…

Trends in the use of thyroid diagnostics and treatments between 2008 and 2019 in Germany.

Following implementation of a national prevention program against iodine deficiency-related disorders in 1993 in Germany, the prevalence of thyroid disorders declined, but aggregated drug dispensation data suggested a recent increase in thyroid hormone use. We aimed to systematically investigate trends in the use of thyroid diagnostic and treatment measures between 2008 and 2019 in Germany. Using the GePaRD database (claims data from ~ 20% of the German population), we determined the annual age-specific and age-standardized prevalence of the use of thyroid diagnostics (thyroid-stimulating horm…

Parasitological examination results of zoo animals in Germany between 2012 and 2022.

Parasitic infections in zoo animals are a critical concern for both animal health and management. The aim of this study was to assess the occurrence of endo- and ectoparasites among zoo animals in Germany. A retrospective analysis of the submitted samples of a diverse range of zoo animals (5768) from a ten-year period (2012-2022) was conducted. Overall, 31.1% of those samples tested positive for at least one parasite. In the examined samples, helminths (28.4%) were found more often than protozoans (10.3%) or ectoparasites (0.8%). Among the various animal groups the following parasites were fou…

The Association between Aortic Valve Stenosis and a Subsequent Diagnosis of Depression in Germany.

Background/Objectives: Aortic valve stenosis (AS) represents one of the most common valve diseases in the western world. It often leads to severe symptoms that can lead to a restriction of everyday life and thus to psychological stress. Therefore, we aimed to investigate the association between AS and depression in outpatients in Germany. Methods: The IQVIATM Disease Analyzer database was used to identify 14,681 individuals with non-rheumatic AS (ICD-10: I35.0 or I35.2). They were propensity score matched (1:1) based on age, sex, average yearly consultation frequency during the follow-up, and …

Timing and chronicity of child maltreatment in Germany: results from a representative sample.

OBJECTIVES: Child maltreatment (CM) is a major risk factor across the lifespan. While research on CM and its consequences has risen strongly during the last decades, research is mainly focused on the prevalence of types of CM incidents. As valid prevalence rates on timing and chronicity of CM are lacking to date, we aimed to assess the timing of experienced CM by describing the age of onset, duration, and prevalence at each year of age for each CM subtype in a population-based sample. STUDY DESIGN: Cross-sectional, observational study in a representative sample. METHODS: Using different samp

Imported Loiasis at a Clinical Reference Center in Germany: A Retrospective Case Series.

Loiasis is a rarely imported infectious disease that is often difficult to diagnose and treat. Here we describe clinical features and treatment outcomes of 11 patients with imported loiasis seen at a German reference center between 2013 and 2023. Clinical presentations varied by patient origin, with eye-worm migration and ophthalmological symptoms being more common among patients from endemic areas and Calabar swelling, subcutaneous swelling, and pruritus more prevalent among returning travelers from nonendemic regions. Eosinophil counts were higher in returning travelers. Diethylcarbamazine w

Optimal botulinum toxin therapy of dystonia in Germany: what would it cost?

Botulinum toxin (BT) therapy is the therapy of choice for most forms of dystonia. We want to describe its costs, if all dystonia patients in Germany would have access to optimal BT therapy. For this, we combined the latest data on epidemiology of dystonia and dosing of BT therapy for dystonia. Missing data were generated for this study. Based on official German pharmacy sales prices, optimal treatment for all dystonia patients in Germany with a population of 84.1 million would generate annual drug costs of €155.5 million (cervical dystonia 89.3, tardive dystonia 22.1, generalised dystonia 17.9…

Incidence of Lyme Borreliosis in Germany: A retrospective observational healthcare claims study.

Lyme borreliosis (LB) is the most common tick-borne disease in Germany. Although the incidence of LB in Germany has been assessed in several studies, those studies either used data from statutory surveillance, which frequently underreport cases, or data from health claims databases, which may overestimate cases due to non-specific LB case definitions. Here, using a more specific case definition, we describe the incidence of medically-attended LB by disease manifestation, age group, and federal state for the period 2015-2019. Both inpatient and outpatient cases were analyzed from a claims datab…
