Minimally invasive percutaneous plating of proximal humeral shaft fractures with the Proximal Humerus Internal Locking System (PHILOS).

BACKGROUND: This study evaluated our results after minimally invasive percutaneous plating of proximal humeral shaft fractures with the Proximal Humerus Internal Locking System (PHILOS, Synthes, Switzerland). MATERIALS AND METHODS: Between 2005 and 2008, 15 patients with unilateral displaced proximal humeral shaft fractures were treated and followed up over a median period of 27 months (range, 12-38 months). The final follow-up included anteroposterior and lateral x-rays, range of shoulder motion, pain by visual analog scale (VAS), the Constant-Murley shoulder score, the Disabilities of Arm,

The outcome of peri-operative humeral condylar fractures after total elbow replacement in patients with rheumatoid arthritis.

We compared the outcome of peri-operative humeral condylar fractures in patients undergoing a Coonrad-Morrey semiconstrained total elbow replacement with that of patients with rheumatoid arthritis undergoing the same procedure without fractures. In a consecutive series of 40 elbows in 33 patients, 13 elbows had a fracture in either condyle peri-operatively, and 27 elbows were intact. The fractured condyle was either fixed internally or excised. We found no statistical difference in the patients' background, such as age, length of follow-up, immobilisation period, Larsen's radiological grade, o

Bilateral asynchronous humeral shaft fractures in a patient with autosomal dominant osteopetrosis type II (Albers-Schonberg disease).

Osteopetrosis is a rare metabolic disease which presents with fragile osteosclerotic bone. A thirteen-year-old girl with an underlying autosomal dominant osteopetrosis (ADO) type II who sustained a left oblique humeral shaft fracture after trivial trauma is presented. Radiographic results showed many characteristic features of osteopetrosis, including incomplete remodeling of a previous fracture of the contralateral humeral shaft. The authors obtained good healing and alignment of the left humeral shaft fracture by sugar tong slab.

Deep Bone Abscess After Pin Tract Infection of an Operatively Treated Supracondylar Humeral Fracture.

Deep bone abscesses are a rare complication following a pin tract infection. This article shows the importance of early detection, surgical management, and microbiology-guided antibiotic therapy to treat the abscess. We aim to highlight the key presenting factors, the radiological signs, and intraoperative features of a deep bone abscess, to facilitate detection in these rare circumstances. Closed or open reduction and internal fixation with Kirschner wires (K-wires) has become the standard of care for displaced supracondylar fractures. Although pin tract infection (PTI) is the most common com

Sudden Release of a Fixed Contracture as a Clue to Occult Humeral Fracture in a Neonate With Arthrogryposis Multiplex Congenita.

A neonate with multiple congenital contractures consistent with arthrogryposis multiplex congenita was found to have an initially clinically occult humeral shaft fracture immediately after axillary cleansing. The first abnormal sign was the sudden release of a previously fixed upper-limb posture, which initially resembled improvement in contracture, before swelling, erythema, or deformity became apparent. Radiography confirmed a spiral fracture of the midshaft of the left humerus, which healed with conservative treatment. This case highlights that an abrupt apparent improvement in a fixed cont

Post-reduction reclassification dilemma in proximal humeral fracture-dislocation: A case report and practical considerations.

INTRODUCTION: Proximal humeral fracture-dislocations (PHF-D) are complex injuries, often requiring urgent intervention. However, management protocols remain unclear when anatomical reduction of the glenohumeral joint is achieved, but significant displacement of the greater tuberosity persists. The lack of consensus on whether to reclassify such injuries after reduction creates uncertainty in rehabilitation strategies. PRESENTATION OF CASE: We present the case of a 53-year-old female who suffered an anterior shoulder dislocation with a displaced greater tuberosity fracture after falling due to

Inferior subluxation of the humeral head after plate surgery for proximal humerus fracture is not always benign.

BACKGROUND: Inferior subluxation of the humeral head following shoulder trauma, osteosynthesis, or rotator cuff repair has been considered as a benign or temporary phenomenon due to muscle fatigue or capsular injury. However, the clinical impact is still unclear as to their occurrence after plating for proximal humerus fractures. This study aims to investigate their patterns and identify associated risk factors as well as clinical outcomes. METHODS: The research included patients who received locking plate surgery following an acute proximal humerus fracture with a minimum follow-up of 12 mon

The necessity of ulnar nerve exploration and translocation in open reduction of medial humeral epicondyle fractures in children.

AIMS: Medial humeral epicondyle fractures (MHEFs) are common elbow fractures in children. Open reduction should be performed in patients with MHEF who have entrapped intra-articular fragments as well as displacement. However, following open reduction, transposition of the ulnar nerve is disputed. The aim of this study is to evaluate the need for ulnar nerve exploration and transposition. METHODS: This was a retrospective cohort study. The clinical data of patients who underwent surgical treatment of MHEF in our hospital from January 2015 to January 2022 were collected. The patients were alloc

Osteochondral Shearing Fracture of the Humeral Head Associated with Traumatic Anterior Shoulder Dislocation: A Case Report.

CASE: Osteochondral shearing fracture of the humeral head after an anterior shoulder dislocation is a condition that has rarely been reported in literature. We report a case of a large posteriorly located fragment of such a fracture in a 23-year-old man. We performed open reduction and internal fixation with resorbable pins through a deltopectoral approach and subscapularis tenotomy. CONCLUSION: In our case of a large osteochondral fracture of the posterior humeral head, treatment by open reduction and internal fixation showed an excellent radiographic result at 1 year and clinical result up

Reduction and fixation of proximal humeral fracture with severe medial instability using a small locking plate.

BACKGROUND: Currently, the reduction and support of comminuted medial cortex of humeral fracture remains a challenge, Therefore, a novel reduction and fixation technique that employs an anteromedial small locking plate was explored in this study, and its viability and the associated complications were assessed. METHODS: Fifteen cases of proximal humeral fractures with medial instability (five cases were classified as three-part and ten as four-part by Neer classification) were treated by the proposed reduction technique using an anteromedial small locking plate. Subsequently, the radiological…

The Community Orthopaedic Surgeon Taking Trauma Call: Lateral Humeral Condyle Fracture Pearls and Pitfalls.

Lateral condyle fractures are second only to supracondylar humeral fractures when it comes to pediatric elbow fractures. Whether minimally displaced or substantially displaced, these growth plate fractures may present significant treatment challenges. These include nonunion, malunion, avascular necrosis, and growth disturbance. It must be remembered that lateral condyle fractures often represent significantly displaced intra-articular fractures involving the epiphysis that demand anatomic reduction and stable internal fixation. This article focuses on some of the pertinent pearls and pitfalls …

A modified deltoid splitting approach with axillary nerve bundle mobilization for proximal humeral fracture fixation.

INTRODUCTION: The deltopectoral and the deltoid splitting approach are commonly used for the treatment of proximal humeral fractures. While the deltopectoral approach requires massive soft tissue devascularization, the deltoid splitting approach needs an additional skipped incision to avoid axillary nerve injury. The purpose of this study was to describe a modified anterolateral deltoid splitting approach with axillary nerve bundle mobilization in the treatment of proximal humeral fractures and to assess its radiologic and clinical outcomes. PATIENTS AND METHODS: Twenty-two consecutive patien…
