Fluid and electrolyte abnormalities.

Patients with cancer are at risk for developing a variety of fluid and electrolyte disturbances caused by the disease process or by complications from therapy. An understanding of the pathophysiology of these potential abnormalities allows the clinician to manage patients expectantly and to avoid severe metabolic disarray by correcting imbalances promptly.

[Hypernatraemia, increased protein intake and progression of renal insufficiency in a patient with polycystic renal degeneration].

The authors submit the case of a patient with polycystic degeneration of the kidneys where after haemorrhage from the anterior cerebral artery hypernatraemia developed and concurrently significant acceleration in the progression of renal insufficiency to failure developed. Hypernatraemia was caused by non-natrium osmotic diuresis conditioned by increased urea excretion. In chronological association with increased dietary protein intake renal insufficiency was hastened.

Coexistence of central diabetes insipidus and salt wasting: the difficulties in diagnosis, changes in natremia, and treatment.

Both central diabetes insipidus (DI) and a high rate of excretion of sodium (Na) and chloride (Cl) contributed to the development of polyuri … d volume was contracted. The tool to explain the basis for the dysnatremias was a tonicity balance. Hypernatremia, which developed before treatment of central DI, was primarily a result of a positive balance for Na rather than a large negative balance for water. Moreover, hyponatremia that developed once antidiuretic hormone acted was primarily a result of a ne …

Management of hypernatremic dehydration due to insufficient lactation.

Critical malnutrition and hypernatremic dehydration resulting from inadequate breast-feeding have previously been reported; the finding of elevated sodium concentration in expressed samples of mother's milk from these infants is found concurrently. This is the first case report describing successful relactation following management of hypernatremic dehydration in a breast-fed infant. Mother's milk was of inadequate volume and had a high sodium content. Treatment of the primary cause of this disorder, insufficient lactation, was effective in allowing successful relactation; a nursing supplement

Chronic hypernatraemia and hypothermia following subarachnoid haemorrhage.

We describe a 30 year old man who developed chronic adipsic hypernatraemia and hypothermia following a subarachnoid haemorrhage from an anterior communicating artery aneurysm. Anterior pituitary function tests were normal. Hypothermia was demonstrated over 4 years with loss of the ability to control heat conservation despite body temperatures as low as 30 degrees C. He failed to experience thirst despite plasma sodium concentrations of up to 187 nmol/l and plasma osmolalities of up to 397 mOsm/kg. The slope of the plasma vasopressin-plasma osmolality curve indicated loss of the osmoreceptor. T

Metabolic alkalosis with hypertonic dehydration in a patient with diarrhoea and magnesium oxide ingestion.

An eight-week-old infant with diarrhoea and dehydration became markedly alkalotic after administration of magnesium oxide powder. The literature does not substantiate the premise that significant magnesium deficiency may occur during acute gastroenteritis in an otherwise healthy infant. Physicians should be warned about this form of therapy as a possible cause of metabolic alkalosis. The possibility of congenital alkalosis with diarrhoea should be considered in differential diagnosis.

A case of 'essential' hypernatraemia due to resetting of the osmostat.

We describe a 24-year-old short, obese girl who has bizarre episodic neurological abnormalities related to hyperosmolality due to hypernatraemia. Investigation of osmoregulation by water loading and infusion of hypertonic saline revealed (i) hypodipsia with thirst onset raised to plasma osmolality of 332 mmol/kg and (ii) elevation of the threshold for vasopressin release (plasma osmolality 320 mmol/kg) but normal slope of the plasma vasopressin-plasma osmolality curve. Baroregulated vasopressin release was also grossly subnormal. Other hypothalamo-pituitary investigations showed deficiencies o

Hypernatraemia--special problems with angiocardiography in total anomalous pulmonary venous drainage.

Two children with unobstructed total anomalous pulmonary venous drainage and hyperkinetic pulmonary hypertension deteriorated with pulmonary oedema and cerebral symptoms following angiocardiography using large amounts of contrast medium. One patient died. The dangers of this with resultant hypernatraemia are discussed.

Chronic in utero plasma hyperosmolality alters hypothalamic arginine vasopressin synthesis and pituitary arginine vasopressin content in newborn lambs.

OBJECTIVE: Arginine vasopressin is synthesized in the hypothalamus and secreted by the posterior pituitary gland in response to plasma hyper … dium increased from 147 +/- 1.3 mEq/L to 156 +/- 1.2 mEq/L. The maternal plasma hyperosmolality and hypernatremia were maintained until normal term delivery. Neonatal stress hyperglycemia mimicking diabetes mellitus secondary to massive intraparenchymal brain abscess due to escherichia coli sepsis: A case report.

Stress hyperglycemia is a common metabolic response in critically ill neonates but may rarely present with extreme hyperglycemia mimicking neonatal diabetes mellitus. We report a term female neonate presenting on day 18 of life with lethargy, poor feeding, dehydration, and severe hyperglycemia (>1000 mg/dL), hypernatremia, and acute kidney injury. Insulin therapy was initiated due to suspicion of neonatal diabetes mellitus. Blood cultures grew Escherichia coli, confirming neonatal sepsis. Neuroimaging revealed a massive right fronto-parieto-temporal intraparenchymal abscess with significant ma…

Transient Postoperative Diabetes Insipidus With Hypovolemic Shock Temporally Associated With Dexmedetomidine, Propofol, and Sevoflurane: A Case Report.

Dexmedetomidine, propofol, and sevoflurane are widely used anesthetic agents, but reports of water metabolism disturbances-particularly drug-induced diabetes insipidus (DI)-associated with their use remain extremely rare. We report a case of a 51-year-old Chinese man who developed abrupt high-volume polyuria (> 600 mL/h; 24-h output of 8750 mL), hypotonic urine (specific gravity 1.003; osmolality 175 mOsm/kg), progressive hypernatremia (peak 156 mmol/L), hemoconcentration, lactic acidosis, and hypotension following transurethral seminal vesiculoscopy performed under general anesthesia. The pat…

Acute Sheehan Syndrome With Distinctive Arginine Vasopressin Secretion and Magnetic Resonance Imaging Findings.

Acute Sheehan syndrome is a rare condition that occurs hours to days post partum and causes hypopituitarism. It may cause hyponatremia due to adrenal insufficiency, and most patients improve with steroid therapy. However, hyponatremia is caused not only by adrenal insufficiency but also by inappropriate secretion of arginine vasopressin (AVP). We report the case of a 30-year-old Japanese primipara with massive postpartum hemorrhage and acute Sheehan syndrome. Hyponatremia developed following hypernatremia soon after the postpartum period; however, it did not improve despite adequate hydrocorti…

Prediction of Admission to Intensive Care Unit and 1-Year Mortality After Acute Pancreatitis With Walled-Off Pancreatic Necrosis: A Retrospective, Single-Center Cohort Study.

BACKGROUND AND AIMS: Pancreatic walled-off necrosis (WON) carries significant mortality and morbidity risks, often necessitating intensive c … rkalemia, hypoalbuminemia, and elevated urea were significant predictors of 1-year mortality, while hypernatremia, elevated serum creatinine, and hypoalbuminemia predicted ICU admission. Predictive models exhibited good discriminative ability, with an AUC of 0.84 (95% CI,0,75-0.93) for 1-year mortality and 0.86 (95%CI, 0.79-0.92) for ICU admission. CONCLUSIONS …
