Long-term neurodevelopmental and psychiatric risks after general anesthesia in early childhood: a nationwide population-based cohort study.

PURPOSE: Pre-clinical evidence suggests that general anesthesia (GA) may have neurotoxic effects on the developing brain, but clinical findings remain inconsistent. Most studies have had limited follow-up or focused on selected neurodevelopmental outcomes, and residual confounding related to surgery remains difficult to address. We investigated the association between GA exposure before age 3 and a broad spectrum of neurodevelopmental disorders (NDDs) diagnosed through adolescence. METHODS: Using the South Korean National Health Insurance Service database, we identified 1,116,195 children bor

Perioperative red blood cell transfusion in gallbladder cancer surgery: Trends and determinants from nationwide data.

BACKGROUNDS/AIMS: Gallbladder and biliary tract cancers (GBCs) are rare malignancies with poor prognoses, and surgical resection remains the only curative intervention. This study examined perioperative red blood cell (RBC) transfusion patterns among GBC patients in Korea and explored clinical and sociodemographic determinants associated with transfusion. METHODS: Utilizing National Health Insurance Service (NHIS) data from 2012 to 2020, we identified 20,564 patients with GBC. Associations between perioperative RBC transfusion and various sociodemographic and clinical characteristics were ass

Standardization of pure laparoscopic extended cholecystectomy with en-bloc lymphadenectomy of the hepatoduodenal ligament for gallbladder cancers.

BACKGROUND: Application of laparoscopic liver resection (LLR) for gallbladder cancers (GBC) has been approved by the Japanese national health insurance system since 2022. However, there are few reports describing LLR techniques for GBCs. We herein report pure laparoscopic extended cholecystectomy with en-bloc lymphadenectomy of the hepatoduodenal ligament for clinical T2 GBC patients. MATERIALS AND SURGICAL TECHNIQUE: We performed this procedure for five clinical T2 GBC patients from September 2019 to September 2022. Under general anesthesia and usual set-up for LLR, the caudal line of the he

Microsurgical Clipping of Low-Riding Basilar Bifurcation Aneurysm.

Basilar bifurcation is a challenging site for aneurysm clipping. Anatomic factors, such as size and projection of the aneurysm, distance between the aneurysm neck and the dorsum sellae, and location of the basilar bifurcation, contribute to surgical complexity. Endovascular treatment has been used more frequently than microsurgical clipping, especially for posterior circulation lesions. Thus, the upcoming generation of neurosurgeons will have increasingly less access to the microsurgical treatment of such lesions. We present the case of a 45-year-old female patient who presented sudden mental

What Factors Are Associated With Revision Cubital Tunnel Release Within 3 Years?

BACKGROUND: It has been shown that patient demographics such as age, payer factors such as insurance type, clinical characteristics such as preoperative opioid use, and disease grade but not surgical procedure are associated with revision surgery to treat cubital tunnel syndrome. However, prior studies evaluating factors associated with revision surgery after primary cubital tunnel release have been relatively small and have involved patients from a single institution or included only a single payer. QUESTIONS/PURPOSES: (1) What percentage of patients who underwent cubital tunnel release unde

Effectiveness of Preoperative Antibiotics in Preventing Surgical Site Infection After Common Soft Tissue Procedures of the Hand.

BACKGROUND: Antibiotic prophylaxis is a common but controversial practice for clean soft tissue procedures of the hand, such as carpal tunnel release or trigger finger release. Previous studies report no substantial reduction in the risk of surgical site infection (SSI) after antibiotic prophylaxis, yet are limited in power by low sample sizes and low overall rates of postoperative infection. QUESTIONS/PURPOSES: Is there evidence that antibiotic prophylaxis decreases the risk of SSI after soft tissue hand surgery when using propensity score matching to control for potential confounding variab

Editorial Commentary: Cost-Effective, Until It Isn't: Variability in Lateral Extra-articular Tenodesis Techniques Challenges Cost Analyses for Primary Anterior Cruciate Ligament Reconstruction.

Lateral extra-articular tenodesis (LET) is an adjunct to anterior cruciate ligament reconstruction (ACLR) designed to improve rotational stability and reduce graft failure, particularly in young, high-risk athletes returning to pivoting sports. With growing clinical evidence supporting reduced failure rates, recent cost analyses suggest lateral extra-articular procedures at the time of primary ACLR may be cost-effective. In this context, routine LET begins to appear not only reasonable but financially responsible. Surgeons, patients, hospital systems, and insurers are understandably drawn to t

Surgery of the rheumatoid foot: preferable procedures.

Management of patients who have rheumatoid arthritis is best accomplished by a team approach utilizing the help of the family physician, the rheumatologist, the physical therapist and the orthopaedist. Such an approach insures careful screening and proper selection of those who can be helped by surgical procedures. Those who have painful feet welcome surgical intervention if nonoperative methods do not furnish relief. The forefoot is often involved and severe deformities of the metatarsophalangeal joints with painful plantar calluses are frequent. The deformities can be corrected and the callu
