Accuracy of surgeon-performed ultrasound in parathyroid localization.

Ultrasound is one of the preferred modalities for localization of abnormal parathyroids. Accuracy of ultrasound is technician-dependent. This study was undertaken to determine the accuracy of surgeon-performed ultrasound (SPU) for the localization of parathyroid tumors in comparison to radiology-performed ultrasound (RPU) and nuclear scintigraphy (NS). In this series 74 consecutive patients with untreated primary hyperparathyroidism underwent SPU at the initial clinic visit; 21 of these patients did not undergo surgery and are excluded from the analysis. Of the 53 patients remaining, RPU was o

Optimizing patellofemoral tracking during total knee arthroplasty.

Fifty-seven patients who underwent 65 primary TKAs between 1993-1994 were retrospectively studied to identify the technical challenges and pitfalls associated with patellar resurfacing and to improve patellar tracking during total knee arthroplasty (TKA). Average patient age was 69 years. All surgeries were performed by a single surgeon (J.N.I), and the same prosthesis was used in all patients. Intraoperatively, attention was paid to avoid femoral and tibial component malrotation and prevent overstuffing of the patellofemoral joint. Preoperative limb alignment was varus in 42 knees, neutral in

[Intramedullary abscess of the spinal cord--a rare pathology. Case report, therapeutic regimen and review of the literature].

Although the presence of an intramedullary abscess of the spinal cord is extremely rare, it is most important to be aware of its existence in the differential diagnosis of neurological diseases. Existing neurological deficits with progressive symptoms of paraplegia should always be regarded as suspect and a differential diagnosis of an intramedullary abscess of the spinal cord should then be included in the therapeutical regimen. A correct diagnosis using MR-tomography followed by an early surgical treatment strategy are essential for the affected patients, simply because an early diagnosis an

Aortic injury during laparoscopic fundoplication: an underreported complication.

BACKGROUND: Advances in video equipment, instrumentation, and laparoscopic skills have enabled the performance of an increasing variety of procedures using minimally invasive techniques. Additionally, the public is more aware of the benefits of laparoscopic surgery, including decreased postoperative pain and shortened recovery period. Surgical treatment of gastroesophageal reflux disease (GERD) is blossoming as a result. As with all surgical procedures, complications can occur. This case report describes a complication of laparoscopic fundoplication not previously reported. Also summarized is

Anaesthesia for coronary artery surgery--a plea for a goal-directed approach.

The purpose of the current literature review was to examine whether changes in current anaesthetic techniques are warranted for patients undergoing coronary artery surgery in light of recent information presented in the literature. The objectives of a cardiac anaesthetic technique are to maintain haemodynamic stability and myocardial oxygen balance, minimize the incidence and severity of ischaemic episodes, be aware of cardiopulmonary bypass-induced pharmacokinetic changes, and facilitate early tracheal extubation if appropriate. Many techniques have been utilized. Provided attention is paid t

Appendico-Ileal Knotting (Appendiceal Tourniquet Syndrome): A Systematic Review and Clinicopathological Classification.

Appendico-ileal knotting (AIK), also known as appendiceal tourniquet syndrome, is an exceptionally rare and frequently overlooked cause of small bowel obstruction in which the appendix encircles the ileum, creating a closed-loop obstruction that rapidly progresses to ischemia and bowel gangrene. Despite advancements in cross-sectional imaging, AIK remains a formidable diagnostic enigma. Its clinical masquerade as common small bowel obstruction, coupled with the absence of pathognomonic radiographic markers, results in a persistently low preoperative diagnostic rate that necessitates a high ind

Recurrent Cushing's disease due to postoperative seeding of tumor cells in the sphenoid sinus: a case report with review of the literature.

Recurrent Cushing's disease is most commonly caused by residual or newly developed pituitary neuroendocrine tumor (PitNET) tissue within the sellar region. Extrasellar recurrence due to postoperative tumor cell seeding is exceedingly rare. We report the unique case of a 30-year-old woman who achieved endocrine remission after gross total resection of an ACTH-secreting PitNET but later developed biochemical and clinical recurrence without radiological evidence of a sellar lesion. Despite long-term medical therapy including cabergoline, metyrapone, and pasireotide, the hypercortisolism gradually

Recurrent Vasovagal Syncope Triggered by Procedural Pain and Post-operative Analgesia during Spinal Anesthesia for High Tibial Osteotomy: A Case Report.

INTRODUCTION: Vasovagal syncope is a common reflex-mediated condition characterized by transient loss of consciousness due to autonomic imbalance resulting in hypotension and bradycardia. Although it is frequently encountered in response to pain or emotional stress, its occurrence during the perioperative period under regional anesthesia is relatively uncommon but potentially alarming. CASE REPORT: We report the case of a 39-year-old female with no significant comorbidities who presented with pain, swelling, instability, and locking of the right knee for 1.5 months. Magnetic resonance imaging

Refractory Hypoxemia After Left Ventricular Assist Device Implantation: Underestimated Patent Foramen Ovale and Venovenous Extracorporeal Membrane Oxygenation-Associated Sump Effect on Bubble Study.

Hypoxemia after left ventricular assist device (LVAD) implantation is a life-threatening complication with a wide differential diagnosis. One potentially catastrophic cause is right-to-left shunting through an unmasked or underestimated patent foramen ovale (PFO). We describe a 63-year-old man in whom an intraoperatively identified PFO with left-to-right flow was initially judged mild and unlikely to be clinically significant. However, on postoperative day 1 in the intensive care unit, profound refractory hypoxemia developed due to massive right-to-left shunting, requiring rescue venovenous (V

Delayed Pseudoaneurysm of the Subclavian Artery Following Clavicle Fracture Fixation: A Case Report.

INTRODUCTION: The clavicle is one of the most commonly fractured bones, and may benefit from operative fixation to restore alignment and function. Although rare, vascular complications are a known risk due to the clavicle's anatomical proximity to the subclavian vessels. A delayed subclavian artery pseudoaneurysm is a very uncommon but potentially life-threatening complication that surgeons must be aware of, as recognition may be challenging. CASE REPORT: A 17-year-old female sustained a displaced midshaft clavicle fracture while playing ice hockey and was treated with an open reduction and i

Unique Location of a Ganglion Cyst Following Shoulder Arthroscopy: A Case Report.

INTRODUCTION: Ganglion cyst formation following shoulder arthroscopy is an exceptionally rare complication, particularly when arising from an arthroscopy portal site. Most shoulder ganglion cysts are associated with labral pathology and are typically located in the spinoglenoid or suprascapular notch. Portal-site or periarticular cysts cause a diagnostic challenge and are seldom reported in the literature. CASE REPORT: We present the case of a 56-year-old female with a history of multiple prior shoulder arthroscopic procedures who developed a painful, localized swelling at the lateral arthros

A deep learning-based classification method for subclinical zonular laxity in AS-OCT images.

OBJECTIVE: In this study, we developed and validated a deep learning method for the detection and angular position identification of subclinical zonular laxity using anterior segment optical coherence tomography (AS-OCT). METHODS: A total of 600 curated AS-OCT images from 536 patients (600 images) undergoing cataract surgery were evenly stratified into subclinical zonular laxity (n = 300 images from 297 patients) and normal control (n = 300 images from 239 patients) groups. Data were partitioned at the patient level to prevent data leakage, with 60% for training, 15% for validation, and 25% f

Peritonitis following duodenal injury due to seatbelt trauma: A case manifesting immediately post-surgery for thoracolumbar fracture-dislocation.

INTRODUCTION: Although abdominal organ damage due to motor vehicle accident is often evident immediately after the injury and urgent operation is performed, it has been reported that minor injuries such as hollow viscus may become apparent during the course of treatment and require urgent surgery. CASE REPORT: The Authors present the case of a 42-year-old female who developed peritonitis immediately after undergoing surgery for thoracolumbar fracture-dislocation caused by a traffic accident. The patient exhibited no abdominal symptoms, such as nausea, vomiting, or abdominal wall rigidity, and

Post-traumatic Cervical Osteomyelitis Masquerading as Neck Strain Following Minor Trauma: A Case Report.

Osteomyelitis is an infection of the bone tissue, causing inflammation. Common causes and risk factors include trauma, surgical procedures, the presence of foreign bodies, and immunocompromised states such as Human Immunodeficiency Virus (HIV) infection and diabetes mellitus. Osteomyelitis occurring in the absence of risk factors is relatively rare. Herein, we present a case of a 41-year-old female dog groomer with a history of iron deficiency anemia and no other significant chronic medical conditions, who presented with persistent neck pain and bilateral hand numbness following a work-related

Delayed Facial Nerve Palsy Developing After Surgery for a Benign Parotid Gland Tumor.

Facial nerve palsy is the most serious complication to be aware of in surgery for parotid gland tumors. It occurs due to various factors including direct intraoperative injury, traction, thermal injury, ischemia, and anatomical variations. While most cases occur immediately after surgery, some may develop within the first 24 to 72 hours due to postoperative edema or hematoma. Delayed facial nerve paralysis occurring more than 72 hours after surgery is extremely rare. We report a case of a 60-year-old woman who developed delayed facial nerve palsy 16 days after parotid gland surgery. Mild paral

Risk factors for post-extraction infection of mandibular third molar: A retrospective clinical study.

Post-extraction infection is one of the most concerning complications of mandibular third molar extraction, which is the most common procedure in oral and maxillofacial surgery. We investigated risk factors for post-extraction infection by retrospectively analyzing 2,513 teeth/cases of mandibular third molar extraction (1,040 males, 1,473 females) performed at a single medical facility in Kobe, Japan from January 2014 to May 2022. The predictive variables were categorized as patient attributes, health status, and anatomic, pathological, and operative variables that may be associated with post-

Management of cervical bronchogenic cyst via minimally invasive video-assisted thyroidectomy: a report of two cases.

BACKGROUND: Bronchogenic cysts are rare congenital diseases, which are believed to occur through the separation of small diverticula buds from an abnormal budding process in the primitive foregut during the formation of the tracheobronchial tree. Ectopic types located in the thyroid region are extremely rare. CASE DESCRIPTION: In this study, we present two cases of cervical bronchogenic cysts identified during minimally invasive video-assisted thyroidectomy (MIVAT). The first case involved a 24-year-old male who presented with a persistent neck mass measuring 41 mm in the right cervical regio

The relationship between different bispectral index and the occurrence of dreams in elective surgery under general anesthesia: protocol for a randomized controlled clinical trial.

INTRODUCTION: Dreaming reported after anesthesia remains a poorly understood phenomenon. At present, there is a hypothesis that dreaming occurs intraoperatively and is related to light or inadequate anesthesia; thus, in order to further verify the hypothesis, we choose elective surgery under general anesthesia to observe whether the generation of dreams is related to the dose of general anesthetics maintenance. METHODS AND ANALYSIS: This randomized, double-blind controlled trial to observe whether the generation of dreams is related to the dose of general anesthetics maintenance in the electi
