State of the Art of IDH Inhibitors: Emerging Questions and Perspectives.

Isocitrate Dehydrogenases (IDH) are ubiquitous enzymes essential for cellular metabolism, including the Krebs cycle, glutamine metabolism, lipogenesis, and redox balance. Mutations in IDH1 and IDH2 are implicated in several tumors - gliomas, Acute Myeloid Leukemia (AML), cholangiocarcinoma - altering enzyme activity and causing the overproduction of 2-hydroxyglutarate (2-HG). This oncometabolite disrupts α-KGdependent proteins, impairing key processes such as differentiation, division, and DNA repair. Understanding these genetic, biochemical, and clinical aspects has made IDH enzymes promising

Serum pyruvate and lactate predict immunotherapy efficacy in advanced gastric cancer: a prospective biomarker study.

BACKGROUND: While immunotherapy has redefined clinical paradigms for advanced gastric cancer, reliable efficacy prediction remains a critical unmet challenge. Unlike invasive tissue-based predictors, circulating biomarkers offer non-invasive monitoring potential. This study investigated serum energy metabolites, whose dysregulation drove immune evasion, as predictors of therapeutic efficacy in advanced gastric cancer receiving first-line chemoimmunotherapy. METHODS: We conducted a prospective observational study involving 52 patients with advanced gastric cancer receiving first-line chemoimmu

Exploration of Natural Products for Targeting IDH1/2 Mutations in Acute Myeloid Leukemia Through Ligand-Based Pharmacophore Screening, Docking, ADME-T, and Molecular Dynamic Simulation Approaches.

BACKGROUND: Mutations in isocitrate dehydrogenase 1 (IDH1) and 2 (IDH2) are prevalent drivers of acute myeloid leukemia (AML). While targeted therapies exist, resistance can emerge. This study explored the potential of natural products to identify novel dual IDH inhibitors. METHODS: In silico screening of the COCONUT database was performed using Lipinski's Rule of Five. Pharmacophore modeling identified crucial features for IDH binding. Docking simulations with Glide (Schrödinger) assessed binding affinity, followed by MM-GBSA calculations for free energy estimation. The most promising candid

Favorable Response to Conventional Chemoradiotherapy in Radiation-induced Glioma Harboring Coamplification of PDGFRA, KIT, and KDR: A Case Report and Literature Review.

One of the most serious complications of cranial radiotherapy is the development of radiation-induced glioma, which is estimated to occur in 1%-4% of patients who have received cranial irradiation and has a worse prognosis than sporadic glioblastoma. Although comprehensive genetic analysis has recently uncovered the molecular characteristics of radiation-induced glioma, the full picture remains unclear due to its rarity. A 45-year-old man presented with generalized seizures caused by multiple brain tumors involving the right frontal lobe, thalamus, and brainstem. The patient had a history of w

New and Emerging Therapies for Patients with Low-Grade Glioma.

Both pediatric and adult patients can develop low-grade glioma (World Health Organization [WHO] grade 2), a type of primary brain tumor that can impact neurologic function and limit one's ability to thrive and survive. Traditionally, the treatment of low-grade gliomas mirrored recommendations for patients with higher-grade gliomas, such as glioblastoma. The diagnosis and categorization of primary brain tumors, including low-grade gliomas, were transformed in 2021 with an update of the World Health Organization classification system for pediatric and adult diffuse gliomas. In the pediatric popu

Large-scale bulk and single-cell RNA sequencing combined with machine learning reveals glioblastoma-associated neutrophil heterogeneity and establishes a VEGFA+ neutrophil prognostic model.

BACKGROUND: Neutrophils play a key role in the tumor microenvironment (TME); however, their functions in glioblastoma (GBM) are overlooked and insufficiently studied. A detailed analysis of GBM-associated neutrophil (GBMAN) subpopulations may offer new insights and opportunities for GBM immunotherapy. METHODS: We analyzed single-cell RNA sequencing (scRNA-seq) data from 127 isocitrate dehydrogenase (IDH) wild-type GBM samples to characterize the GBMAN subgroups, emphasizing developmental trajectories, cellular communication, and transcriptional networks. We implemented 117 machine learning co

Dexamethasone-associated regression of glioblastoma.

BACKGROUND: Dexamethasone-induced regression of an intracranial space-occupying lesion is commonly characteristic of primary central nervous system lymphoma (PCNSL). However, dexamethasone does not have an established chemotherapeutic role in glioblastoma multiforme (GBM). This is a report on dexamethasone-induced regression in GBM with the aim of exploring the mechanisms behind the phenomenon. CASE DESCRIPTION: We present the case of a 63-year-old male presenting with status epilepticus. Initial imaging showed a high-grade glioma with significant vasogenic edema. Following 23 days of dexamet

Absolute quantification of TCA cycle intermediates in mouse ocular tissues reveals distinct tissue- and sex-specific mitochondrial metabolism.

OBJECTIVE: Mitochondrial tricarboxylic acid (TCA) cycle is central to energy production and redox balance in the eye, which must sustain high metabolic activity to support vision. Retinal neurons, the retinal pigment epithelium (RPE), cornea, and lens each have distinct physiological roles and metabolic demands, yet the absolute concentrations of key TCA intermediates and their variation by tissue, sex, and time of day are not well-defined. METHODS: Targeted gas chromatography-mass spectrometry was employed to quantify the absolute concentrations of TCA cycle metabolites in mouse ocular tissu

Human Herpesvirus 6A (HHV-6A) Encephalitis in an Immunocompetent Patient and Its Association With Glioblastoma: A Case Report.

Human herpesvirus 6 (HHV-6) is a neurotropic virus capable of establishing latency in the central nervous system. While its reactivation is well-documented in immunocompromised individuals, its role in immunocompetent hosts remains unclear. Additionally, growing but inconclusive evidence suggests a potential association between HHV-6 and glioblastoma. We present the case of a 65-year-old immunocompetent male who developed HHV-6A-associated meningoencephalitis, followed by the diagnosis of high-grade glioblastoma within months. The patient initially presented with altered consciousness, seizure

Comparative Analysis of Recurrent Glioblastoma Target Contours via 11C-Methionine, 68Ga-Prostate-Specific Membrane Antigen Positron Emission Tomography, and Magnetic Resonance Imaging: Implications for Precision Radiotherapy Planning.

PURPOSE: Glioblastoma (GBM) recurrence poses challenges in radiation therapy treatment planning because reirradiation has limited leeway needed for precise target delineation. Although effective radiotracers are emerging for treatment planning, comparisons of 11C-methionine positron emission tomography (MET-PET), 68Ga-prostate-specific membrane antigen PET (PSMA-PET), and magnetic resonance imaging (MRI) for contouring recurrent GBMs are lacking in the literature. This case study aimed to highlight the differences and similarities in target contours delineated from 3 examinations, aiming to ra

Itaconic acid inhibits nontuberculous mycobacterial growth in pH dependent manner while 4-octyl-itaconic acid enhances THP-1 clearance of nontuberculous mycobacteria in vitro.

Increasingly prevalent, nontuberculous mycobacteria (NTM) infections affect approximately 20% of people with cystic fibrosis (CF). Previous studies of CF sputum identified lower levels of the host metabolite itaconate in those infected with NTM. Itaconate can inhibit the growth of M. tuberculosis (MTB) in vitro via the inhibition of the glyoxylate cycle enzyme (ICL), but its impact on NTM is unclear. To test itaconic acid's (IA) effect on NTM growth, laboratory and CF clinical strains of Mycobacterium abscessus and Mycobacterium avium were cultured in 7H9 minimal media supplemented with 1-10 m

13C-SpaceM: Spatial single-cell isotope tracing reveals heterogeneity of de novo fatty acid synthesis in cancer.

Metabolism has emerged as a key factor in homeostasis and disease including cancer. Yet, little is known about the heterogeneity of metabolic activity of cancer cells due to the lack of tools to directly probe it. Here, we present a novel method, 13C-SpaceM for spatial single-cell isotope tracing of glucose-dependent de novo lipogenesis. The method combines imaging mass spectrometry for spatially-resolved detection of 13C6-glucose-derived 13C label incorporated into esterified fatty acids with microscopy and computational methods for data integration and analysis. We validated 13C-SpaceM on a

GPR68 supports AML cells through the calcium/calcineurin pro-survival pathway and confers chemoresistance by mediating glucose metabolic symbiosis.

Accumulating evidence demonstrates that the "Warburg effect" that glycolysis is enhanced even in the presence of oxygen existed in hematopoietic malignancies, contributing to extracellular acidosis. G-protein coupled receptor 68 (GPR68), as a proton sensing GPCR responding to extracellular acidosis, is expected to play a critical role in hematopoietic malignancies. In the present study, we found that GPR68 was overexpressed in acute myeloid leukemia (AML) cells, and GPR68 deficiency impaired AML cell survival in vitro and cell engraftment in vivo. Mechanistic studies revealed that unlike GPR68

Metabolic adaptation in epithelial ovarian cancer metastasis.

Epithelial ovarian cancer (EOC) is highly lethal due to its unique metastatic characteristics. EOC spheroids enter a non-proliferative state, with hypoxic cores and reduced oncogenic signaling, all of which contribute to tumour dormancy during metastasis. We investigated the metabolomic states of EOC cells progressing through the three steps to metastasis. Metabolomes of adherent, spheroid, and re-adherent cells were validated by isotopic metabolic flux analysis and mitochondrial functional assays to identify metabolic pathways that were previously unknown to promote EOC metastasis. Although s
