Paraneoplastic syndromes and the kidney.

A paraneoplastic syndrome is defined as a group of symptoms that develop when substances released by some cancer cells disrupt the normal function of the surrounding cells and tissue. Paraneoplastic renal syndromes are diseases that indirectly compromise tubular and glomerular function by electrolyte imbalance, hormone-producing tumors or deposition of antigen-antibody complexes in the glomeruli. In order to describe the most common paraneoplastic syndromes, which may compromise the renal function, an extensive review was performed of papers, including case reports, guidelines, meta-analysis a

Kidney transplantation in the elderly.

There is an increase in the older incident end-stage renal disease population that is associated with an increasing prevalence of end-stage renal disease in the United States. This trend is paralleled by an increasing rate of kidney transplantation in the elderly. Although patient survival is lower in older versus younger kidney recipients, the elderly benefit from a reduction in mortality rate and improved quality of life with transplantation compared with dialysis. Immunologic, physiologic, and psychosocial factors influence transplant outcomes and should be recognized in the care of the eld…

Epithelioid angiomyolipoma of the kidney.

Epithelioid angiomyolipoma (eAMLoma) is an uncommon renal mesenchymal tumor with malignant potential and is frequently associated with tuberous sclerosis (TSC). It is composed of polygonal large-sized tumor cells arranged in an epithelioid manner. Differential diagnosis from renal cell carcinoma (RCC) is often challenging because of its epithelioid morphology. Herein is reported three cases of eAMLoma, involving one in a 28-year-old man with TSC and two in women without TSC (34 and 62 years of age, respectively). The male TSC patient had microscopic conventional AMLomas in the same kidney. All…

[Epithelioid angiomyolipoma of the kidney].

A 37-year-old female was referred to our department for a renal mass which was incidentally found during a medical check-up. She had no history of tuberous sclerosis. Computed tomography and magnetic resonance imaging showed a 1.5-cm enhancing mass at the lower pole of the left kidney. An open partial nephrectomy was performed for a pre-operative diagnosis of renal cell carcinoma. Pathological examination revealed sheets of large polygonal epithelioid cells that showed positive immunostaining for HMB-45, which confirmed the diagnosis of an epithelioid angiomyolipoma. The patient remained free …

In search for the troponin of the kidney.

The prevalence of chronic renal disease is increasing dramatically in industrialized nations. Recent surveys in Europe and the US show that about 10% of the total population are affected. Patients with end stage disease, necessitating dialysis or transplantation, are only the tip of the iceberg. Even with treatment these subjects suffer from a massive reduction in quality and quantity of life despite consuming a large proportion of healthcare expenditures. As quite effective treatment strategies are available to prevent progression there is a pressing clinical need for early detection, differe

[The kidneys and old age].

With more detailed knowledge on renal function at advanced age it became clear that age-related changes of renal function are less severe than previously thought. However, these changes are significantly aggravated by classical cardiovascular risk factors such as hypertension, diabetes and smoking. As a consequence, in elderly with such co-morbidity the appearance of a primary kidney disease or acute renal failure may cause a serious deterioration of renal function. In addition, impaired homeostasis with respect to salt (and fluid) balance has important consequences for the management of the e…

Iron transport and the kidney.

Over the last decade there has been an explosion in our understanding of the proteins that modulate iron homeostasis. Much research has focused on the tissues classically associated with iron absorption and metabolism, namely the duodenum, the liver and the reticulo-endothelial system. Expression profiling has highlighted that many of the components associated with iron homeostasis, are also expressed in tissues which hitherto have received relatively little attention in terms of iron research. These include, testis, lung and, the subject of this review, the kidney. The latter is of great inte…

Animal toxins and the kidney.

Envenomation or poisoning by toxins from animals poses an important health hazard in the tropics. Animal toxins are complex mixtures of prot … fects. All renal structures can be affected by animal toxins, and tubular necrosis is common. Acute kidney injury is attributed to decreased renal blood flow (associated with intravascular hemolysis or rhabdomyolysis), disseminated intravascular coagulation or direct tubular toxicity. Immunologic mechanisms have a minor role in the pathophysiology of nep …

Parvovirus B19 and the kidney.

Infection with parvovirus B19 causes several clinical syndromes (fifth disease, transient aplastic crisis, pure red cell aplasia, and hydrops fetalis) and may contribute to other illnesses. B19 has been linked to renal disease in three settings: As a cause of acute glomerulopathy and as a cause of anemia in ESRD and kidney transplantation. Case reports implicate parvovirus in the pathogenesis of proliferative glomerulonephritis and collapsing glomerulopathy, but a causal relationship has not been established. A proposed role for B19 infection is based on the temporal association of renal findi…

Intrathoracic kidney in an adult.

An intrathoracic kidney, although rare, should be considered in a patient with a mass at the base of the lung on a chest radiograph. Excretory urography is diagnostic and may eliminate the need for further extensive investigation and operation.

The kidney in liver transplantation.

Early recognition and determination of the cause of renal failure in patients with ESLD can be difficult because of the potential interplay … the latter. Because of the ever-increasing shortage of donor organs, CLKT must be used judiciously. Kidney biopsy may resolve diagnostic dilemmas. Management of renal complications post-OLT remains a challenge for the physician caring for transplant patients. Modification of nephrotoxic immunosuppressive regimens to avoid postoperative ARF/CRI has met wi …

[Angiotensin II and the kidney].

Angiotensin II (AngII) plays a central role for maintenance of GFR and Na balance particularly in volume depletion, when AngII preferentially increases the resistance of efferent arterioles as compared to afferent arterioles, enhancing the glomerular perfusion pressure. In addition, AngII enhances tubular reabsorption of sodium in proximal tubules directly and indirectly as a consequence of glomerulotubular balance. AngII also stimulates Na reabsorption in the collecting ducts by stimulating the release of aldosterone from the adrenal cortex. AngII augments tubuloglomerular feedback. Recently,

Inherited carcinomas of the kidney.

Studies of families with inherited carcinomas have provided powerful tools to identify the genes involved in the pathogenesis of human cancers. In this review, we summarize the clinical, pathological, and genetic characteristics of the inherited carcinomas of the kidney. We emphasize the observation that different genes predispose to histologically different types of renal carcinoma. Hereditary papillary renal carcinoma, a recently described inherited disorder, is discussed in detail along with the predisposing gene, the MET protooncogene. The data support a classification of renal carcinomas …
