PURPOSE: Severly increased femoral anteversion is an important risk factor for patellofemoral instability. Recurrent dislocations cause a traumatic disruption of the medial patellofemoral ligament. Therefore a procedure that combines femoral derotation osteotomy and patellofemoral ligament reconstruction should be considered for patients with severely increased femoral anteversion. The aim of the study was to evaluate the subjective and objective outcomes after combined femoral derotation osteotomy and anatomical reconstruction of the MPFL.

On the basis of review of results of 60 kinematic condylar knee replacements in 49 patient and the literature the principles of surgical techniques in varus and valgus deformities of the knee and patellar subluxation have been presented. The authors substantiated their opinion of contraindication for patellar replacement. Tibial components (AJ, PCR, Total Condylar) have been discussed and data on frequency of their use provided. We present a case of superior dislocation of the patella trapped by interlocked osteophytes. Unlike previous reports, in which the mechanism resulted from a blow to the inferior pole, it is postulated that increased load on the extensor mechanism, combined with patella alta and patellofemoral osteophytes, caused locking of the knee in extension.

Tibial plateau fractures or knee fracture-dislocations often result from trauma or sporting injuries. Given the subcutaneous position of the knee and upper tibia, without much intervening soft tissue, any such injury can be associated with significant soft tissue damage. Thus, consideration of the degree of soft tissue injury must be part of the treatment plan. Specific management decisions regarding blisters, open wounds, and compartment syndrome are discussed, as well as the timing of surgery, role of provisional external fixation, and placement of incisions for definitive internal fixation.

In 1,377 patients who had severe injuries about the knee joint, vascular complications were present in 28 (2%). Injuries to the popliteal artery or its branches present serious problems. Amputation was necessary in 13 patients, and 12 patients had a Volkmann's syndrome. In fractures of the lower end of the femur or upper end of the tibia, and in knee dislocations, the importance of early diagnosis of the acute ischemic syndrome cannot be overemphasized. Tibial plateau fractures produced the highest percentage of vascular complications and indicate immediate application of therapeutic measures.
