[Calcification and contamination as complications in the use of double-J catheters: incidence and correlation].

We present a series of cases of complex renal lithiasis, in which we carried out temporary internal urinary bypassing, prior to extracorporeal lithotripsy, by means of the retrograde insertion of a double-J urethral catheter, studying the time of stay, the presence or not of contamination in the catheter and its relationship with the calcifications found in them. We also carried out a metabolic-functional study of each patient, determining the urinary pH and the serum and urinary calcium, phosphorous and uric acid concentrations, and observing their possible influence on the above-mentioned co…

[Odditis. Pathologic reality or tactics necessity].

In 14 patients with secondary odditis, a biopsy of the papilla was studied, in one case encountering moderate peri and intrafascicular fibrosis and in another, erosion of the papillary mucosa with impaction of biliary material. In 6 biopsies obtained from patients with perforated liver cysticerocosis, submucosal edema of the biliary tree was observed in only 3. On the basis of these studies an analysis is made of the different percentages published, which are highly variable, and results are compared to those reported in larger series. Papillary affectation in lithiasis was related to trauma p…

[Acute calculous cholecystitis (author's transl)].

The authors studied 333 acute cholecystitis out of 2,200 operated on for lithiasis of the gall-bladder, 186 bacteriological tests were carried out on the vesicular liquid and wall. They insist on the main anatomo-pathological characteristic: early, partial or total destruction of the mucous together with acute inflammatory lesions of the wall and even sometimes necrosis. This irreversible lesion is due to the acute obstruction of the gall-bladder by blockage of a calculus. Acute cholecystitis are practically always aseptic at the beginning. Sepsis is a secondary complication. These anatomo-pat…

[Etiologic diagnosis of hypercalcemia. A study of 100 cases].

On the basis of 100 cases of hypercalcemia, the authors attempt to elucidate the criteria of the etiologic diagnosis. Kidney lithiasis or nephrocalcinosis suggested a primary hyperparathyroidism (HPT I) or an intoxication due to vitamin D. X rays of the skeleton and quantitative histological exams of the bone were not useful in the diagnosis of HPT I. The level of parathormone in the plasma is the best parameter to be used in distinguishing HPT I from other diseases. In the absence of renal insufficiency or severe intestinal disorders, a phospharemia below 2.6 mg/100 ml, a chloremia above 103 …

[A Case of Intrahepatic Ectopic Pancreas Diagnosed by Surgical Resection Suspecting Intrahepatic Cholangiocarcinoma].

The patient was a 64-year-old male. During a health checkup, dilation of the intrahepatic bile ducts and intrahepatic stones were found. Abdominal ultrasound revealed a space-occupying lesion in segment 3 and significant dilation of the peripheral bile duct of B3. ERCP showed approximately 1 cm stenosis at the root of B3 and dilation of the peripheral bile ducts. Additionally, EUS revealed a stone in the proximal bile duct of B3. Based on these findings, a diagnosis of intrahepatic lithiasis and intrahepatic bile duct dilation was made, and the possibility of intrahepatic cholangiocarcinoma wa…

Unfavorable factors in accessing the pelvicalyceal system during retrograde flexible ureteroscopy (fURS).

Flexible ureteroscopy (fURS) is a well-established procedure for treating multiple upper-urinary tract pathologies, particularly renoureteral lithiasis. Endoscopes have undergone significant advancements, including miniaturization, improved optics, and increased maneuverability. In addition, advancements in accessory instruments, such as the performance of laser fibers, guidewires, and extraction probes, have played a significant role in improving the overall performance of flexible ureteroscopy procedures. However, despite these advancements, unique circumstances can make achieving optimum re…

[Cholelithiasis and cholecystectomy in infants].

Cholelithiasis is rare in children and even more so in infants. We report the case of a 3-month-old patient with cholestatic jaundice secondary to an obstruction of the terminal portion of the bile duct. The treatment applied in this patient was a cholecystectomy with trans-cystic cholangiography and common bile duct clearance. The evolution was excellent. The current literature on biliary lithiasis in children and infants is poor in large cohort studies. The various treatments proposed, if necessary, include biliary lavage by percutaneous puncture, endoscopic retrograde cholangiopancreatograp…

A submucosal bladder stone in a 65-year-old woman.

We describe a case of a 65-year-old woman with a submucosal bladder stone with no apparent underlying cause. She presented with lower abdominal pain, dysuria, and a history of recurrent cystitis. Ultrasound and computed tomography of the lower abdomen revealed the presence of lithiasis nearby to the left ostium. During ureterorenoscopy, stone was found neither in the bladder nor in the left ureter. Instead, we observed a macroscopic bulge close to the left ureter. After opening of the mucosa, a stone became visible and could be removed. Mostly, imaging of the calculus was done some time before…

Acute pancreatitis: an opportunity for gastroenterology hospitalists?

Acute pancreatitis (AP) is the third most frequent cause of hospital admissions for digestive disorders in the US and Europe after digestive bleeding and cholelithiasis/cholecystitis. The incidence of AP ranges from 15 to 100 cases per 100,000 inhabitants per year, and has been steadily increasing in recent years. In Spain, the reported incidence is 72 patients per 100,000 inhabitants per year. The most frequent cause is biliary lithiasis (50 %-60 % of cases); fortunately, 80 % of patients have only mild symptoms-as defined by the revised Atlanta Classification-and progress favorably, although…

Fully laparoscopic pancreaticojejunostomy, Puestow procedure (with video).

Chronic pancreatitis usually requires medical treatment and, in rare cases, surgical intervention. The most frequent cause of consultation i … olution with endoscopy was attempted, but the pancreatic duct could not be accessed due to impacted lithiasis. After discussion in a multidisciplinary committee, surgical treatment was recommended, and a fully laparoscopic Puestow surgery was performed.

[Urinary stone imaging tests.].

Imaging in urolithiasis has a wide group of techniques, with different characteristics, limitations, and indications, which together allow the management of this prevalent pathology. Plain abdominal radiography and ultrasound are very accessible and in expensive techniques that combined present an acceptable sensitivity and specificity. They are widely used for monitoring the evolution of stone disease and for evaluation after treatments (surgery or SWL). Ultrasound is the primary radiological diagnostic tool in the pediatric population and in pregnant women. CT has prevailed over IVP in the a…

[Mirizzi's syndrome: a rare cause of biliary tract obstruction: about a case and review of the literature].

Mirizzi's syndrome is a rare complication of chronic vesicular lithiasis with prevalence ranging from 0.7% to 1.4% among patients who have undergone cholecystectomy. It is characterized by cholestatic icterus associated with compression of the common bile duct due to lodged calculus in the vesicular neck or in the cystic duct. The disease can evolve toward the erosion through the common hepatic duct wall and, therefore, it can cause the formation of a gallbladder-biliary fistula. We here report a case of Mirizzi's syndrome type I in order to highlight the role of preoperative diagnosis which i…

Microureteroscopy in Children: Two First Cases.

BACKGROUND: Urinary stones disease is becoming more common not only in adults but also in children. Most cases are resolved with extracorporeal shock wave lithotripsy, but miniaturization of endoscopes has increased the use of ureteroscopy in resolving ureteral stones, most notably in children. CASE PRESENTATION: This presentation focuses on two cases of microureteroscopy. In both cases, the presence of lithiasis in the pelvic ureter was suspected to be the cause of ureter hydronephrosis, and a microureteroscopy was performed for treatment purposes. MicroPerc set 4.85F sheath was used to expl…

Diffuse Adenomyomatosis of the Gallbladder in a Child.

Adenomyomatosis of the gallbladder (ADMG) is characterized by proliferation of the mucosal epithelium and hypertrophy of the muscularis. ADMG is predominantly diagnosed by using ultrasonography. Although ADMG is benign in nature, lithiasis, and chronic inflammation secondary to it may lead to dysplastic changes and cancer. Mucosal invagination through the hypertrophied muscularis results in large intramural diverticula or sinus tracts which are visible at radiology, known as Rokitansky-Aschoff sinuses. Histologically, ADMG manifests with hyperplasia of the muscular layer and proliferation of m…

Phosphate handling: new genes, new molecules.

BACKGROUND: Inappropriate renal phosphate transport may alter serum phosphate concentration and bone mineralization, and it may also increase the risk of renal lithiasis or soft-tissue calcifications. Molecular identification of renal phosphate transporters and regulatory proteins has improved our knowledge of the mechanisms that control phosphate balance. This summary reviews recent findings regarding the consequences of mutations affecting several human genes encoding for phosphate transporters or regulatory proteins. Further, it describes the role played by the fibroblast growth factor 23-K…
