Evaluating the efficiency of one stop crisis centres in managing domestic violence cases.

INTRODUCTION: As domestic violence (DV) poses a critical threat to public health worldwide, this prompts the need for efficient and effective intervention. In Malaysia, although One Stop Crisis Centres (OSCCs) have been offering multisectoral services to DV victims for many decades, an evaluation of the efficiency of these centres has yet to be conducted. This study aimed to assess the efficiency and effectiveness of three Malaysian OSCCs using a two-stage Data Envelopment Analysis (DEA) approach. MATERIALS AND METHODS: A total of 153 adult DV victims were recruited from OSCCs in Sarawak Gene

Review of evolution of clinical, training and educational services and research program for autism spectrum disorders in Hong Kong.

The evolution of a local fragmented model of services for children with autism in Hong Kong emerged gradually over the past three decades with lack of government funding or support. This had been due to increasing number of children with autism being detected and referred for earlier assessment. With increasing pressure from parents due to long waiting time for assessment and training services and the increasing polarization by mass media there had been a gradual increasing public awareness over the past five years. Though still highly fragmented in the availability of services, there is a gro

Inhaled corticosteroid use and associated outcomes in elderly patients with moderate to severe chronic pulmonary disease.

OBJECTIVE: This study was undertaken to examine the impact of select demographic, clinical, and medication-related factors on elderly patients' medication adherence to inhaled corticosteroid therapy. It also examined the impact of medication adherence on health outcomes and on health care service utilization and costs. BACKGROUND: Asthma in the elderly is a growing concern because of its increasing prevalence and the increase in asthma-related deaths. Poor medication utilization could be contributing to this trend in the elderly population and should be examined. METHODS: This retrospective,

Multidisciplinary teams, efficient communication, procedure services, and telehealth improve cirrhosis care: A qualitative study.

BACKGROUND: Cirrhosis care and outcomes are improved with access to subspecialty gastroenterology and hepatology care. In qualitative interviews, we investigated clinicians' perceptions of factors that optimize or impede cirrhosis care. METHODS: We conducted 24 telephone interviews with subspecialty clinicians at 7 Veterans Affairs medical centers with high- and low-complexity services. Purposive sampling stratified Veterans Affairs medical centers on timely post-hospitalization follow-up, a quality measure. We asked open-ended questions about facilitators and barriers of care coordination, a

Maintaining Continuity of Care for Expectant Mothers in Kenya During the COVID-19 Pandemic: A Study of MomCare.

In Kenya, early coronavirus disease (COVID-19) modeling studies predicted that disruptions in antenatal care and hospital services could increase indirect maternal and neonatal deaths and stillbirths. As the Kenyan government enforced lockdowns and a curfew, many mothers-to-be were unable to safely reach hospital facilities, especially at night. Fear of contracting COVID-19, increasing costs of accessing care, stigma, and falling incomes forced many expectant mothers to give birth at home. MomCare, which primarily serves communities in remote areas and urban slums, links mothers-to-be with pay

Acute Opiate Overdose: An Update on Management Strategies in Emergency Department and Critical Care Unit.

BACKGROUND: Opioids are natural, semisynthetic, or synthetic substances that act on opioid receptors in the central nervous system. Clinically, they are prescribed for pain management. Opioid overdose (OOD) occurs when the central nervous system and respiratory drive are suppressed because of excessive consumption of the drug. Symptoms of OOD include drowsiness, slow breathing, pinpoint pupils, cyanosis, loss of consciousness, and death. Due to their addictive potential and easy accessibility opioid addiction is a growing problem worldwide. Emergency medical services and the emergency departme

[Interdependent islands: health policies and medical practices in the care, management, and research of a neglected disease in the Global South].

This study analyzes medical practices related to hydatidosis in the province of Santiago del Estero, Argentina. This parasitic zoonotic disease is recognized as a significant public health problem in South America. Nevertheless, it is included among the neglected diseases as classified by the World Health Organization - those that primarily affect the poorest populations living in rural, remote, or marginalized areas with limited access to health services. The objective is to examine how hydatidosis is configured as a health issue through an analysis of medical practices involved in its care,

What Happens Next? Traumatic Brain Injury in the Community.

Traumatic brain injury (TBI) continues to substantially impact the lives of millions of people around the world annually. Community-based prevention and support of TBI are particularly challenging and underresearched aspects of TBI management. Ongoing cognitive, emotional, and other effects of TBI are not immediately obvious in community settings such as schools, workplaces, sporting clubs, aged care facilities, and support agencies providing homelessness or domestic violence support. This is compounded by a lack of guidance and support materials designed for nonmedical settings. Connectivity

Preventing escalation in customer service: immediate motivation and 4-week outcomes in a workplace deployment of LLM role-play training.

Many customer support employees encounter aggressive customer attitudes, including overt dissatisfaction, unreasonable demands, and verbal abuse. Such attitudes do not always stem solely from customers: inadequate employee responses can amplify dissatisfaction and escalate initially manageable issues, making skills that prevent escalation critical. Recent advances in large language models (LLMs) enable role-play training with virtual interlocutors. However, role-play systems in which virtual customer attitudes shift in response to the trainee's utterances under pedagogically grounded rules rem

Understanding functional diversity in public primary health care: a cluster analysis of utilization patterns.

BACKGROUND: Primary health care (PHC) systems are typically organized using uniform administrative models that assume functional similarity across facilities. In practice, however, PHC units differ substantially in how services are delivered. In Greece, despite ongoing reforms, empirical evidence on the functional heterogeneity of public PHC units remains limited. This study aims to classify PHC units based on observed service utilization patterns and to identify the organizational and geographical determinants shaping these functional typologies. METHODS: This cross-sectional study analyzed

Determinants of the incidence of clinical consequences of untreated dental caries among underprivileged schoolchildren: a two-year prospective cohort study.

OBJECTIVES: To investigate the influence of structural and intermediary determinants on the incidence of clinical consequences of untreated dental caries in schoolchildren. METHODS: A two-year prospective cohort study was conducted with 329 twelve-year-old students from public schools in [Text blinded for peer review]. Socioeconomic status (family income, parents' schooling, number of goods, and number of rooms), psychosocial factors (oral health beliefs, sense of coherence [SOC-13], social support [SSA]) and number of decayed teeth were assessed at baseline. Identifying and understanding how critical landscapes for carbon sequestration respond to development for low carbon energy production: Insight to inform optimal land planning and management strategies.

Wind farms can mitigate increasing CO2 emissions by fossil-fuel free energy generation. However, landscape disturbance during development must not have lasting impacts on C sequestration, an ecosystem service. To understand how a critical carbon landscape responds to wind farm development, we monitored for 10 years dissolved organic carbon (DOC) export in five catchments draining Europe's second largest onshore wind farm, Whitelee, UK. The DOC flux trend and seasonality were modelled using Generalised Additive and Mixed Models, novelly using first derivatives of trends to identify responses to
