Desmoplastic fibroma (DF) is a rare benign bone tumor adopting an aggressive behavior, representing a challenge for clinical and radiographic diagnosis. This case report focused on a 31-year-old man with a large mandibular lesion with severe displacements of the mandibular teeth. Only a combination of paraclinical findings allows a definitive diagnosis to be made. Cervicofacial MRI revealed a low T1 signal intensity with peripheral enhancement after Gadolinium, and T2 hyperintense signal, while PET scan showed a moderate metabolism. Bone biopsy with immunohistochemical analysis allowed for def

Following a brief discussion of etiology, pathogenesis, and diagnosis of congenital facial asymmetries, a new interdisciplinary treatment concept encompassing the disciplines of orthodontics, maxillo-facial surgery, and pedaudiology is set forth. This concept calls for the surgical reconstruction of the mandibular ramus and of the temporo-mandibular joint already in childhood in those cases in which functional jaw orthopedics has already begun at an early age. Thereafter the occlusal plane can be corrected orthodontically. Six patients were treated according to this concept. Two years nine mon

This study is an intermediate evaluation of early surgical-orthodontic treatment in 7 patients with uni- or bilateral hypoplastic mandibular growth due to juvenile chronic arthritis. Resection of the condylar heads and reconstruction with costochondral grafts was undertaken between 10 and 14 years of age and followed by orthodontic treatment. At the latest follow-up, 2-4.5 years postoperatively, all patients showed mandibular growth. Some even showed a tendency towards progenia. The scheduling of the surgery in relation to completion of growth seemed to be of importance in relation to the magn

Facial penetrating gunshot wounds (GSWs) are seen in an assault, suicide, and accidental injury. They often carry high mortality given the important anatomical structures located within the neck. The foundations of maxillofacial GSWs are rooted in data from military combat, specifically the last world war. This type of injury is complex for reconstructive surgery due to significant soft tissue and bone loss. Management of maxillofacial GSWs is often challenging and has trended from serial debridement, immediate reconstruction, local tissue flaps, and distant free flap transfers depending on bu

We present the case of an 11-year-old girl with McCune-Albright syndrome associated with severe fibrous dysplasia. In addition to bone lesions, she has apparent manifestations of precocious puberty. In examination, a mass at the mentum spreading to mandibular corpus bilaterally was seen. This mass has affected the mandibular teeth. It was nearly 20 x 20 x 15 cm in size and had local necrotic regions on it. The lower lip was expanded too much by the mass. Another mass filled the left maxillary sinus, expanding the left zygomatic region outwardly and closing the left nasal fossa completely. A re

BACKGROUND: Post-traumatic mandibular malocclusion is a complex condition that poses a significant challenge to reconstructive surgeons. The malocclusion that ensues from bilateral condylar and parasymphseal fractures presents a particular challenge as it leads to bilateral posterior shortening and lingual tilting of dental arch leading to a combination of open anterior bite, crossbite, overbite, underbite, and/or facial asymmetry. The complexity of such malocclusion requires intricate freedom of movement of the mandibular arch that can be achieved by performing a 5-segment mandibular osteotom

Vascularized fibula free flap (FFF) grafts are frequently used to reconstruct mandibular defects. However, the current planning methods for osteotomy, splicing, and fibula placement present challenges in achieving satisfactory facial aesthetics and restoring the original morphology of the mandible. In this study, we propose a novel two-step framework for automated preoperative planning in FFF mandibular reconstruction. The framework is based on mandibular shape completion and morphometric descriptors. Firstly, we utilize a 3D generative model to estimate the entire mandibular geometry by incor…

Osteomyelitis is an infection and inflammation associated with the bone structures: bone marrow, cortical bone, periosteum, blood vessels and nerves. Although it does not have a frequent presentation, it can sometimes lead to complications such as pathological fractures or even septicemia. We present a clinical case study, to assess the relationship between osteomyelitis of the jaw and pathological fractures, after the extraction of an impacted wisdom tooth. This case highlights a rare complication following the surgical removal of mandibular third molar; a slow evolution of the pathology foll

There is limited information on the biological status of bone regenerated with microvascular fibula flap combined with biomaterials. This paper describes the clinical, histological, ultrastructural, and molecular picture of bone regenerated with patient-customized plate, used for mandibular reconstruction in combination with microvascular osteomyocutaneous fibula flap. The plate was virtually planned and additively manufactured using electron beam melting. This plate was retrieved from the patient after 33 months. Microcomputed tomography, backscattered-scanning electron microscopy, histology,…

Laboratory and clinical studies have shown that mandibular distraction is an effective and powerful reconstructive surgical technique. It is safely performed without the need for blood transfusion or bone graft. Because of concomitant expansion of the functional matrix (the associated skin, subcutaneous fat, and muscles of mastication), there is a multidimensional expansion of the lower jaw with minimal, if any, evidence of relapse. Moreover, the surgical reconstruction can be commenced by 2 years of age, and the vector of distraction can be controlled by preoperative design. Finally, the incr

Maxillo Mandibular injuries are most common through road traffic accident. The defects and deformities are usually prominent in case of facial injuries. The ultimate aim of recontouring and reconstruction of the facial defect is to achieve the maximum functional as well as esthetic part of the face. Here we present a case of fracture in to multiple fragment of the infra orbital rim, reconstructed with rib graft.

BACKGROUND: Vascularized fibula transfer has become a preferred method of mandibular restoration after oncologic surgical ablation. In order to elucidate the long-term effect on fibular mass after mandibular reconstruction, change in fibular height was utilized as an indirect measure of change in bone mass over time. Other potentially influential factors in long-term bone mass preservation were evaluated; these included site of reconstruction (central, body, ramus), patient age, length of follow-up, adjuvant radiotherapy, and the delayed placement of osseointegrated dental implants.

INTRODUCTION: Autologous reconstruction of segmental craniomaxillofacial bone defects is limited by insufficient graft material, donor site morbidity, and need for microsurgery. Reconstruction is challenging due to the complex three-dimensional (3D) structure of craniofacial skeleton. Customized 3D-printed patient-specific biologic scaffolds hold promise for reconstruction of the craniofacial skeleton without donor site morbidity. The authors report a porcine craniofacial defect model suitable for further evaluation of custom 3D-printed engineered bone scaffolds.

PURPOSE: Osteochondroma is a benign tumor that is rare in the craniofacial region. When it does occur, the condyle and coronoid process have been the most commonly affected sites. The secondary progressive malocclusion and facial asymmetry will be common physical signs in most cases. The traditional surgical treatment of osteochondroma of the mandibular condyle has been condylectomy, with or without reconstruction of the condyle. We believe more attention should be paid to restoring joint function, improving the facial appearance, correcting malocclusion, and re-establishing harmony among them

Ankylosis of the temporomandibular joint (TMJ) is a condition in which the mandibular condyle fuses with the mandibular fossa through fibrous or bone tissue. It is a debilitating pathology that interferes with chewing, speaking, and oral hygiene. Currently, alloplastic reconstruction is considered the gold standard for treating severely compromised TMJs, such as in ankylosis. The article describes a patient with a history of facial trauma, with bilateral ankylosis of the TMJs, inability to open his mouth, and poor dental condition. Due to a long period of immobilization of approximately 40 yea

PURPOSE: We present a 17-year follow-up of an unusual patient with median cleft of the lower lip, mandible, and tongue with a flexion contracture.
CONCLUSION: Timing of the mandibular reconstruction needs to take into account growth of the mandible and masticatory function. In this case, fixation of the mandible performed at the age of 6 years 6 months and reconstruction carried out at 7 years 8 months of age resulted in satisfactory occlusion and masticatory function.

Mandibular reconstruction in pediatric patients has some unique considerations. The method of reconstruction has to factor in the growth potential of the neo-mandible, the native mandible, and the donor site. The condyle is considered the main growth center of the mandible. Current literature indicates that fibula, iliac crest, and scapula osseous flaps do not have the ability to grow. Costochondral grafts exhibit growth because of the costal cartilage component, although the growth is unpredictable. Preservation of the mandibular periosteum can result in spontaneous bone regeneration. Fibula …

BACKGROUND AND PURPOSE: Paediatric temporomandibular joint ankylosis (TMJA) is a debilitating condition that impairs mandibular function and disrupts normal craniofacial growth. Costochondral graft (CCG) reconstruction has been widely used due to its biological compatibility and presumed growth potential; however, its long-term behaviour remains controversial. Purpose was to systematically evaluate the functional outcomes and growth potential of costochondral graft reconstruction in paediatric TMJA.
METHODS: A systematic review was conducted in accordance with PRISMA guidelines. Electronic da
