Change in soft tissue in relation to that in hard tissue following orthognathic surgery was evaluated. Twenty-five patients were enrolled in the study. The diagnosis in all was jaw deformity (maxillary retrusion and mandibular protrusion) and all underwent a Le Fort I osteotomy and bilateral sagittal splitting ramus osteotomy. Three-dimensional (3D) computer-aided design (CAD) models (polygon models) of the hard and soft tissue of the maxilla and mandible were constructed and superimposed. Reference points were established on the pre- and postoperative hard and soft tissues. Specific elements

Interdisciplinary treatment for patients with Treacher Collins syndrome is challenging because of the rarity of the condition and the wide variety of phenotypic expression. A 23-year-old male was diagnosed with Treacher Collins syndrome with a history of severe obstructive sleep apnea. He presented with a Pruzansky-Kaban classification grade I mandible, skeletal type II pattern with a hyperdivergent mandibular plane, severe convex profile, and Class II malocclusion with a missing mandibular incisor. Improvement of facial esthetics was achieved by a combination of orthodontics, mandibular distr

BACKGROUND: Mandibular prognathism is a common dentofacial deformity in Asia. Treatment of such condition may vary from orthodontic camouflage to surgical orthodontics with orthognathic surgery depending on the severity of the condition. Because of the prominent position of the mandible, fractures involving different locations of the mandible commonly occur in maxillofacial trauma. Anatomical reduction of maxillofacial fractures and restoration of the pretraumatic occlusion are the primary goals of acute management of facial fractures. In patients with dentofacial deformity, simultaneous surgi

Orbital hypertelorism (OR.H) is defined as an abnormal increase in the distance between the two orbits secondary to a skeletal anomaly, and it occurs in association with numerous congenital craniofacial malformations. Since its description by Greig in 1924, OR.H and the associated corrective procedures have captivated many surgeons. Here we present a discussion of the historical evolution of surgery for OR.H and highlight its future prospects. In the mid-twentieth century, only cover-up techniques simulating approximation of the eyes via an optical illusion were used, such as frontonasal skin

BACKGROUND: Endoscopic dacryocystorhinostomies (eDCRs) show patency rates between 81% and 94%. However, dacryocystorhinostomy (DCR) failure and the need for revision remain a significant challenge. One of the principal challenges in revision eDCR is the need to surgically identify the correct osteotomy site and maintain long-term patency in the setting of previously instrumented and potentially scarred tissue. At the same time, the surgeon must assume that the blood supply to the commonly described anterior and posteriorly pedicled flaps has been compromised.

OBJECTIVE: To investigate the effect of new bone formation in sinus augmentation with guided bone regeneration (GBR) using collagen membranes.
METHODS: The first maxillary molars of 18 adult female Beagle dogs were extracted and the sinus floors of both sides were lifted with simultaneous implantation. A combination of autografts and Bio-Oss in a 2:1 ratio was placed in the space under the membrane. On the experimental side in each dog, the collagen membrane was folded at the lateral osteotomy window, the apex of the implants and a certain part of palatal bone. On the contralateral control si

PURPOSE: To evaluate the ratio of soft tissue to hard tissue in bilateral sagittal split setback osteotomy with rigid internal fixation or wire fixation.
MATERIALS AND METHODS: A literature search was performed using PubMed, Medline, CINAHL, Web of Science, the Cochrane Library, and Google Scholar Beta. From the original 766 articles identified, 8 articles were included. Two articles were prospective and 6 retrospective. The follow-up period ranged from 1 year to 12.7 years for rigid internal fixation. Two articles on wire fixation were found to be appropriate for inclusion.
RESULTS: The dif

PURPOSE: The purpose of the study was to obtain information on the possibility of using distraction osteogenesis (DO) to increase the height of posterior atrophic maxillary alveolar ridges by distracting very small alveolar crest bone segments and, secondly, to determine if it is possible to distract the bony antral floor itself, producing new bone between the sinus membrane and the alveolar crest.
MATERIALS AND METHODS: Three adult Papio anubis baboons were rendered edentulous in the posterior maxilla. Bilateral osteotomies were performed to produce a thin margin of crestal alveolar bone and

This study aims to compare the incidence of sinus membrane perforation between osseodensification and osteotome crestal sinus lift techniques when 5 mm of residual bone is present beneath the sinus floor. Additionally, it seeks to identify when the perforation most commonly occurs-during instrumentation, bone grafting, or implant insertion. A split-mouth design using 20 sinuses from 10 fresh human cadaver heads were employed. One side underwent a bone-added osteotome crestal sinus lift, while the other side received an osseodensification lift. Membrane perforation was recorded through video an

PURPOSE: Using panoramic radiograph and CBCT to compare the measurement errors of the residual bone height in the same site before implant placement in the posterior maxilla and endo-sinus bone gain after osteotome sinus floor elevation without grafting, and to evaluate endo-sinus bone augmentation before stage-two surgery with CBCT. Also, some related factors of new bone formation in the sinus were analyzed, such as small bone block elevated by osteotome at the implant apex intraoperatively.
MATERIALS AND METHODS: Patients were enrolled in the retrospective study; dental implants were placed

Juvenile ossifying fibroma (JOF) is an uncommon, benign, bone-forming neoplasm seen in the craniofacial bones. It is distinguished from other fibro-osseous lesions primarily by its age at onset, clinical presentation, and potential aggressive behavior. Occasionally JOF may grow aggressively and extend to involve the orbits and skull base, resulting in serious cosmetic and functional problems. A radical surgery is not advisable in a pediatric age group. We describe a case of JOF of the maxilla removed by sublabial approach conservatively, but completely without compromising the aesthetic looks

Odontomas are mixed odontogenic tumors of the jaws with both epithelial and mesenchymal origins. These tumors usually are small and at times are disregarded; however, there are times when dentists will need to remove such tumors (for example, when they are symptomatic, when they enlarge or impede tooth eruption, when orthodontic treatment is required, or when they are associated with teeth that require removal, such as impacted third molars). Although odontomas are not uncommon and are familiar to practitioners, postsurgical complications may result if dentists are not aware of the potential p

Two examples are presented of osteotomies performed simultaneously with surgical recontouring in cases of fibrous dysplasia of the jaws. Osteotomies are necessary when a coexistent discrepancy in jaw relationship is present. This provides a functional as well as aesthetic result. Rigid miniplate fixation was utilised, and dysplastic bone healing was uneventful, and found clinically not to differ from that of normal bone. Osteotomy is thus feasible in dysplastic bone, and may complement recontouring in achieving a better result.

James Sidman, MD, and Sherard A. Tatum, MD, address the following questions for discussion and debate. Is neonatal distraction osteogenesis (DO) better than lip-tongue adhesion or tracheotomy for micrognathic airway compromise? What role does DO have in adult orthognathic surgery situations? In monobloc and Le Fort III procedures, are internal or external devices preferable? What role does DO play in craniofacial microsomia? Is endoscopic DO better than open procedures for synostosis management? How has your technique changed or evolved over the past 5 years and what has doing this technique t

Placing implants in the posterior region of the maxilla may require expanding the floor of the maxillary sinus. A major surgical procedure developed 30 years ago used lateral window access to the sinus cavity to achieve this. In the intervening years, less invasive techniques using a crestal approach were introduced; while they have reduced the complication and morbidity risks, they require micro-surgical skills. This review of the progression of maxillary sinus augmentation from major surgery to a minimally invasive procedure explores the various surgical approaches, discussing the advantages
