Three-year follow-up (1983-1986) of children undergoing bilateral grommet insertion in Sheffield.

A retrospective study with review, after 3 years, of 43 children between the ages of 4 and 13 years, who had bilateral otitis media with effusion treated surgically purely by the insertion of grommets is described. The results shows that the younger the child at initial grommet insertion, the greater the likelihood of more than 1 grommet insertion being required (P less than 0.05). A younger patient is likely to make a greater number of outpatient visits (P less than 0.05), and the number of outpatient visits correlates with the number of grommets inserted (P less than 0.01). At 3 years, 49% o

Allergic rhinitis and asthma: drivers of otitis media and associated tympanostomy tube placement.

PURPOSE: The purpose of this study is to examine the influence of allergic rhinitis (AR) and asthma on the incidence of otitis media (OM), which in turn serves as a mediator for the need for tympanostomy tube placement (TTP). METHODS: A comprehensive logistic regression analysis was performed, incorporating multiple variables to assess the relationship between AR, asthma, and the likelihood of TTP. OM was considered the mediating variable in this relationship. The analysis adjusted for a range of demographic and health-related covariates to ascertain the independent effects of the allergic co

Aseptic surgical technique and postgrommet otorrhoea.

Myringotomy and grommet insertion is one of the most common operations performed in Europe today, with an estimated 70,000 being carried out annually in Britain alone [Lancet 1 (1984) 835]. Otorrhoea is a common postoperative complication. There is controversy about the degree to which surgical asepsis should be maintained during the operation. This prospective study of 35 patients (70 ears) undergoing grommet insertion was carried out to evaluate the efficacy of two differing degrees of surgical asepsis. Using each patient as their own control a grommet was inserted into one ear with the surg

Tympanic membrane perforation following the removal of ventilation tubes in the presence of persistent aural discharge.

This study considers the effect of the removal of a ventilation tube from the tympanic membrane of an ear which has been affected with persistent mucopurulent discharge for at least 3 months. The records of 332 patients were reviewed. Thirty-three patients with 34 ears satisfied the entry criteria. Tube removal cured 27 out of 34 ears (79%) of aural discharge within 1 month. The tympanic membrane healing rates were: at 1 month, 14 (41%); at 3 months, 23 (68%); at 6 months, 24 (71%); and at 1 year, 28 (82%). A table is presented comparing perforation rates from different ventilation tube studie

Secretory otitis media in the adult.

Middle ear effusion secondary to Eustachian tube obstruction may complicate nasal and nasopharyngeal conditions. It may be the presenting feature of a nasopharyngeal tumour. A series of 94 adult cases of middle ear effusion was reviewed. Examination under anaesthetic of the postnasal space was performed in each case, but did not result in the diagnosis of a tumour in any patient in whom it was not strongly suspected clinically on the basis of non-otologic features. Patients having grommet insertion had better hearing for 9 months postoperatively than those having myringotomy alone. Treatment o

Expression of surfactant Protein-A in the Haemophilus influenzae-induced otitis media in a rat model.

The objective of this study was to investigate the expression and the role of surfactant protein A (SP-A) in the middle ear (ME) mucosa in response to bacterial infection in a rat model. Otitis media (OM) was induced by surgical inoculation of non-typeable Haemophilus influenza (NTHi) into the ME cavity of Sprague-Dawley rats. The rats were divided into an NTHi-induced OM group and a phosphate-buffered saline-injected control group. The NTHi-induced OM and control groups were subdivided into sets of 6 rats, one for each of the 6 time points (0, 1, 2, 4, 7, and 14 days post-inoculation), at whi

Pediatric tympanostomy tube assessment via deep learning.

PURPOSE: Tympanostomy tube (TT) placement is the most frequently performed ambulatory surgery in children under 15. After the procedure it is recommended that patients follow up regularly for "tube checks" until TT extrusion. Such visits incur direct and indirect costs to families in the form of days off from work, copays, and travel expenses. This pilot study aims to compare the efficacy of tympanic membrane (TM) evaluation by an artificial intelligence algorithm with that of clinical staff for determining presence or absence of a tympanostomy tube within the TM. METHODS: Using a digital oto

Otogenic lateral sinus thrombosis in children: A review of 7 cases.

Otogenic lateral sinus thrombosis (LST) is a rare but serious intracranial complication of acute or chronic otitis media. Reported mortality rates have ranged from 8 to 25%; the pediatric mortality rate might be as low as 5%. Controversy still exists over the medical and surgical management of this condition. We conducted a retrospective chart review of 7 cases of pediatric otogenic LST that were treated at our institution over a period of 8 years. We hypothesized that good outcomes in very sick patients can be achieved by aggressively managing the mastoid cavity and without the need for a thr

Factors related to persisting perforations after ventilation tube insertion.

OBJECTIVE: Over a million ventilation tubes are placed annually in the United States, making this one of the most commonly performed procedures in the field of medicine. Certain factors increase the risk of persistent tympanic membrane perforation following the extrusion of short term ventilation tubes. Persistent perforations may fail to heal on their own, necessitating surgical closure to avoid conductive hearing loss. It is important to detect factors that may predict children who are at increased risk for persistent perforations. METHODS, OUTCOMES DATA AND STATISTICAL ANALYSIS: This study
