Neuromonitoring in the operating room: why, when, and how to monitor?

This review considers the main principles and indications of EEG and evoked potential (EP) neuromonitoring in the operating room. Neuromonitoring has a threefold purpose: to warn the surgeon that he has to adjust his strategy, to confirm his decision, and to help him improve subsequent procedures. The pathophysiology of intraoperative events liable to alter the EEG or the EPs is first considered. The usefulness of neuromonitoring in preventing neurological complication relies on its ability to detect neurological dysfunction at a reversible stage. This applies especially to ischemia and compre

Genetics, natural history, surveillance, management, and gene mapping in the Lynch syndrome.

Hereditary nonpolyposis colorectal cancer (HNPCC), also termed Lynch syndrome, may account for as much as 10-15 percent of the total colorectal burden. Lynch syndrome I is characterized by site-specific colorectal cancer (CRC) with early (congruent to age 45) onset, predilection to the proximal colon (congruent to 70%), and a marked susceptibility to metachronous CRC (45% following hemicolectomy or segmental resection). Lynch syndrome II shows the same colonic features but includes an excess of extra-colonic CRCs, namely carcinoma of the endometrium, ovary, small bowel, stomach, pancreas and t

Informed choices for long-term benefits. The management of catheters in continence care.

1. Non-invasive methods of managing urinary dysfunction should be considered as alternatives to long-term catheterisation and reviewed on a regular basis. 2. Nurses must match choice of catheter and drainage equipment with the individual patient, to enhance self-care and independence. 3. Monitoring of catheter life can establish a pattern of recurrent blockage and allow pro-active, planned recatheterisation. 4. Good handwashing practice is important in catheter management to reduce the risk of cross-infection.

[Atypical early infantile epileptic encephalopathy with suppression-burst].

We report an 8-month-old infant with tonic spasms that started at the age of 38 days and occurred in clusters. He had no myoclonic seizures, or erratic myoclonus. Repeated simultaneous video-EEG monitoring revealed abrupt flexion of the neck and extremities associated with high voltage slow waves preceded by multiple spikes and/or sharps, which resembled spasms seen in West syndrome. Interictal EEGs, before and after the onset of epilepsy, showed suppression-burst. The bursts consisted of irregular high voltage slow waves mixed with poorly developed spike before the onset of epilepsy. On the o

Anesthetic considerations for the patient with Huntington's disease.

Huntington's disease (HD) is an autosomal, dominantly inherited disease with symptoms manifesting late in life. Patients suffer from motor dysfunction and progressive mental deterioration. HD patients present a number of anesthetic challenges as they are usually elderly, malnourished, and at risk for aspiration. Previous reports on anesthesia for patients with HD have warned that sodium pentothal and succinylcholine cause prolonged apnea in this patient population. In this article, a 78-year-old female presented for cataract extraction. Although her movement disorder was under good control wit

Narcolepsy in childhood and adolescence.

The objective of this study was to evaluate clinical, polysomnographic and the multiple sleep latency test (MSLT) features in young narcoleptics. We evaluated 14 patients with mean age of 13.6 years old (ranging from 6 to 18 years); 11 were males and 3 females. Daytime sleepiness was the main complaint in all cases and started at the ages of 6 to 17 years. Cataplexy was described in 10 cases and it was considered mild to moderate in all but one case. Sleep paralysis was present in 6 cases and hypnagogic hallucinations in 7 cases. The main polysomnographic characteristics were the short sleep l

Liquid crystal temperature monitoring for microsurgery.

Postoperative monitoring of free tissue transfers remains a problem for the microsurgeon. Liquid crystal temperature probes (LCT) are used by anesthesiologists to monitor patient core temperature and to indicate changes in temperature trends as an indicator of pending malignant hyperthermia. By placing an LCT monitor on the flap and adjacent tissue at the completion of surgery, temperature differentials can be reliably monitored. If the temperature differential exceeds 2 degrees C, the flap is re-explored. The LCT readout resembles a standard thermometer and can easily be recorded by even inex

[Early recognition of malignant hyperthermia using capnometry].

Although malignant hyperthermia is still a potentially fatal disease that was marked by a high mortality until recently, lasting damage to the patient can now be prevented by early diagnosis and treatment. The following case demonstrates the special value of capnometry in diagnosing this condition. A 34-year-old man admitted for oral surgery showed symptoms of malignant hyperthermia 5 h after induction of anesthesia. Neuroleptanalgesia had been conducted. The patient had received thiopental and fentanyl for induction of anesthesia and alcuronium and succinylcholine for intubation. The first sy

Parameters influencing the response time of volatile anesthetics monitors.

In a given inhalational anesthetic analyzer, response (RT) is usually thought to be a constant value, however, several factors may influence RT. RT's measured under ideal conditions for the Beckman LB 2, the Normac (Datex), the Servo S 120 (Siemens) and the Irina (Dräger) were 107 +/- 5, 589 +/- 14, 538 +/- 30, and 166 +/- 15 msec, respectively. In addition, we investigated the RT of a Beckman LB 2 analyzer under conditions which may occur in clinical practice (non ideal conditions). Increasing aspirating flow (AF) resulted in shorter RT's, the effect being most pronounced when AF was below 20

[Usefulness of intracranial pressure monitoring and EEG analysis in a child with acute encephalitis].

The usefulness of continuous intracranial pressure (ICP) monitoring and the correlation between ICP and electroencephalographic findings were investigated in a 6-year-old boy with acute encephalitis. On admission he was comatose in a decerebrate posture. ICP was continuously monitored by the epidural transducer and controlled with hyperventilation, fluid restriction and administration of mannitol, glycerol and pentobarbital to keep below 20 mm Hg. The inotropic agents such as dopamine and dobutamine were also given to maintain the cerebral perfusion pressure (CPP) at 50 mm Hg or greater. At th

Which is the most accurate method of measuring blood pressure?

A blood pressure (BP) recording is, strictly speaking, only relevant for the circumstances under which it is measured. A reading that is truly representative of the average pressure is difficult to obtain because of the great variability of pressure in any person. The use to which BP readings are put should be defined, whether for characterizing populations, assessing antihypertensive agents, or managing individual patients. The limitations of any technique must be taken into account. Intra-arterial measurement of BP is the most accurate method, capable of giving a continuous picture. Indirect

Respiratory-dependent pacing: a dual response from a single sensor.

Five patients with respiratory dependent rate responsive pacemakers (Biorate RDP3, Biotec) were studied using ambulatory telemetry to evaluate the sensitivity of this pacing system to nonrespiratory signals. In each case the pacemaker was implanted in the left infraclavicular position with an impedance sensing electrode inserted into the subcutaneous tissues of the anterior chest wall. The pacing rate was 73 +/- 2 beats per minute at rest and rose by 42 +/- 5 beats per minute when the patients were walking with both arms swinging (mean +/- SEM). Three of the five patients had sensing electrode

Effects derived from long-term low-level chromium exposure in ferro-alloy metallurgy. Study of absorption and renal function in workers.

Ferro-chromium production is based on the use of chromium oxide (III) and it is generally accepted that chromium in this form is not able to cross biological barriers. However, the data on the toxic and carcinogenic effects of hexavalent chromium in man are now firmly established. Some studies have questioned these data, calling for a clarification as to whether exposure to trivalent chromium can also produce human effects, perhaps with different latency time. A study was carried out on the exposure conditions (type and degree) in a ferro-chromium foundry that had been in operation since 1972.
