Motoneuron death in normal and spinal muscular atrophy-affected human fetuses.

The onset and sequence of motoneuron death in the lumbar part of spinal cord of seven control and five affected fetuses carrying deletion of exon 7 in the SMN gene were studied by light and electron microscopy. Naturally occurring motoneuron death in control fetuses was detected between 8 and 13 weeks of gestation. In affected fetuses motoneuron death was prolonged and also observed at 16 and 20 weeks of gestation. In addition, motoneurons in the affected fetuses displayed nuclear abnormalities as early as 16 weeks of gestation and the changes progressed as the affected fetuses developed and c

Molecular genetic analyses of five Vietnamese patients with spinal muscular atrophy.

Most patients with spinal muscular atrophy (SMA) have been reported to show homozygous deletion of the gene responsible for SMA, SMN1. However, whether SMA patients homozygous for the SMN1 deletion exist in Southeast Asian countries, including Vietnam, remains to be determined, because molecular genetic analyses of SMA patients from these countries have not been reported. In this preliminary study, we analyzed five Vietnamese SMA patients and found that SMN1 gene exons 7 and 8 were completely absent in one of them, a 6-month-old girl with hypotonic muscles. Thus, SMN1 deletion can be a cause o…

Progressive muscular atrophy variant of familial amyotrophic lateral sclerosis (PMA/ALS).

Twelve cases of adult-onset progressive muscular atrophy variant of amyotrophic lateral sclerosis (PMA/ALS) were studied in a small rural population of 1500 in the Republic of Belarus (former Soviet Union). The patients were members of three apparently related kindreds, each showing autosomal dominant pattern of disease inheritance. The average age at clinical onset ranged from 26 to 57 years (mean, 40 years). Each patient suffered from skeletal muscle weakness and wasting, starting in the limbs and spreading to the trunk and neck, with very limited bulbar and no upper motor neuron involvement…

Role of anti-apoptotic Bcl-2 protein in spinal muscular atrophy.

Apoptosis is an essential physiological cell death for selective elimination of cells, involved in a variety of biological events including morphogenesis, cell turn over and removal of harmful cells. Disruption of the regulation of apoptosis may result in various diseases, including cancer and autoimmune diseases both associated with inhibition of apoptosis and various degenerative disorders associated with enhancement of apoptosis, and therefore, apoptosis is an important theme in the medical field. Apoptosis is driven by a family of cysteine proteases, called caspases and regulated by a Bcl-

Non-familial, spinal segmental muscular atrophy in juvenile and young subjects.

Thirty-two cases of juvenile and adult onset, non-familial, spinal, segmental muscular atrophy seen in two widely separated geographical regions of India were studied over the last 15 years. The characteristic features were: the non-familial, non-endemic nature, the strictly segmental distribution from the clinical and EMG point of view, which involved either distal or proximal segments of one upper or lower extremity, the absence of involvement of cranial nerves of the pyramidal, sensory and autonomic nervous system, the very high male predilection and the very insidious progression. No defin…

Hexosaminidase-A deficiency presenting as atypical juvenile-onset spinal muscular atrophy.

Three patients from two families had an unusual phenotypical variant of late-onset hexosaminidase-A deficiency. The clinical picture was dominated by spinal motor neuron involvement mimicking juvenile-onset spinal muscular atrophy. Atypical features included prominent muscle cramps, postural and action tremor, recurrent psychosis, incoordination, corticospinal and corticobulbar involvement, and dysarthria. The presence of these atypical features in patients whose lower motor neuron involvement would otherwise be consistent with juvenile-onset spinal muscular atrophy should raise the suspicion …

Machine learning of connectome-wide functional connectivity in spinal muscular atrophy.

While traditional view posed SMA as a pure motor neuron disorder, accumulating evidence pointed to the broader involvement of central nervous system. This study aimed to combined connectome-wide resting-state functional connectivity with machine learning algorithms to investigate the underlying brain mechanisms in SMA and capture the potential neuroimaging biomarker. Resting-state functional magnetic resonance imaging were acquired from 43 SMA (26 SMA type 2 and 17 type 3) and 45 healthy controls (HCs). Connectome-wide functional connectivity, defined based on the Automated Anatomical Labeling

Treatment evolution in spinal muscular atrophy: insights from the SMArtCARE registry.

Real-world treatments for 5q-spinal muscular atrophy (SMA) have evolved rapidly following the sequential approval of three disease-modifying treatments (DMTs): nusinersen, onasemnogene abeparvovec and risdiplam. The aim of this study was to map the sequence and timing of SMA treatments accurately using the SMArtCARE registry, a disease-specific registry for patients with SMA across 84 participating centres in Germany, Austria and Switzerland. All patients registered in SMArtCARE were included in the analysis. Patients were grouped based on their treatment regimen: those who remained on the fir…

Liver Steatosis in Induced Hepatocytes From Carriers of Spinal Muscular Atrophy.

INTRODUCTION/AIMS: Although classically characterized as a motor neuron disease, spinal muscular atrophy (SMA) is increasingly recognized as a multisystem disorder. We previously showed hepatocyte-intrinsic steatosis in SMA, raising the question of whether SMA carriers, who are typically asymptomatic, may also exhibit subclinical hepatic abnormalities. METHODS: We generated induced hepatocyte-like cells (iHeps) from induced pluripotent stem cells (iPSCs) derived from an SMA Type 2 proband, his isogenic wild-type (Iso-WT) line, and both carrier parents, comprised of three carrier lines from th…

Spinal muscular atrophy type 2 with severe kyphoscoliosis: a case report.

BACKGROUND: Spinal muscular atrophy (SMA) is a rare autosomal recessive neuromuscular disorder caused by homozygous deletion or mutation of the SMN1 gene. Scoliosis is a common complication in patients with SMA type 2, often leading to impaired sitting balance, reduced respiratory function, and poor quality of life. We report a case of SMA type 2 with severe kyphoscoliosis successfully treated with posterior spinal fusion. CASE PRESENTATION: A 16-year-old Chinese male presented with progressive spinal deformity and a 14-year history of lower limb weakness. He had never walked independently an…

Fatal outcomes following onasemnogene abeparvovec in advanced-stage spinal muscular atrophy.

Supported by encouraging trial outcomes, onasemnogene abeparvovec (OA) was authorized for spinal muscular atrophy (SMA). Nevertheless, efficacy of OA in advanced SMA patients remains underexplored. This investigation assessed clinical effectiveness and adverse effects of OA in a cohort including advanced SMA, and compared to historical survival data for SMA type 1 patients in Thailand. We conducted observational cohort study at Siriraj Hospital, Thailand, from May 2019 to April 2022. The study enrolled eight SMA patients receiving OA therapy. The cohort comprised five SMA type 1 patients treat…

Global Economic Burden of Spinal Muscular Atrophy: A Systematic Literature Review.

Spinal muscular atrophy (SMA) is a rare inherited neuromuscular disease classified into four main subtypes and characterized by severe muscle weakness and loss of motor function. Its high mortality rates, high treatment costs, and lengthy care requirements place a heavy burden on patients, caregivers, and the healthcare system. This study aims to explore the economic burden of SMA subtypes by analyzing costs, healthcare resource use, and loss of productivity for patients and their caregivers. We conducted a systematic literature review, searching for studies published since 2010 via Medline, E…

Autophagy in spinal muscular atrophy: from pathogenic mechanisms to therapeutic approaches.

Spinal muscular atrophy (SMA) is a devastating neuromuscular disorder caused by the depletion of the ubiquitously expressed survival motor neuron (SMN) protein. While the genetic cause of SMA has been well documented, the exact mechanism(s) by which SMN depletion results in disease progression remain elusive. A wide body of evidence has highlighted the involvement and dysregulation of autophagy in SMA. Autophagy is a highly conserved lysosomal degradation process which is necessary for cellular homeostasis; defects in the autophagic machinery have been linked with a wide range of neurodegenera…

Beyond Contractures in Spinal Muscular Atrophy: Identifying Lower-Limb Joint Hypermobility.

Background: The natural history of spinal muscular atrophy (SMA) is well understood, with progressive muscle weakness resulting in declines in function. The development of contractures is common and negatively impacts function. Clinically, joint hypermobility (JH) is observed but is poorly described, and its relationship with function is unknown. Methods: Lower-limb ROM (range of motion) assessments of extension and flexion at the hip, knee, and ankle were performed. ROMs exceeding the published norms were included in the analysis. The functional assessments performed included the six-minute w…

KLF13 restrains Dll4-muscular Notch2 axis to improve the muscle atrophy.

BACKGROUND: Muscle atrophy can cause muscle dysfunction and weakness. Krüppel-like factor 13 (KLF13), a central regulator of cellular energy metabolism, is highly expressed in skeletal muscles and implicated in the pathogenesis of several diseases. This study investigated the role of KLF13 in muscle atrophy, which could be a novel therapeutic target. METHODS: The effects of gene knockdown and pharmacological targeting of KLF13 on skeletal muscle atrophy were investigated using cell-based and animal models. Clofoctol, an antibiotic and KLF13 agonist, was also investigated as a candidate for re

Macrostructural Brain Abnormalities in Spinal Muscular Atrophy: A Case-Control Study.

BACKGROUND AND OBJECTIVES: Most individuals with spinal muscular atrophy (SMA) on disease-modifying therapies continue to have chronic motor impairment. Insights into brain involvement in SMA may open new pathways for adjunctive therapies to optimize outcomes. We aimed to characterize macrostructural brain abnormalities detected by MRI in individuals with SMA compared with peer controls. METHODS: We conducted a cross-sectional case-control study of children and adults with a confirmed genetic diagnosis of 5q SMA, and peer controls matched by age and sex. Brain MRIs acquired on a 3T MRI scanne…
