An Unexpected Encounter With Miliary Tuberculosis in a Young Man With a Remote Exposure History.

Miliary tuberculosis (TB) represents a rare escalation of TB stemming from the hematogenous dissemination of Mycobacterium tuberculosis . Common extrapulmonary infection sites include the lymphatic system, musculoskeletal system, and central nervous system. The nonspecific motif of clinical symptoms involving joint pain, fever of unknown origin, and night sweats make the diagnosis of disseminated TB quite challenging. Long-term outcomes remain bleak. Untreated miliary TB is estimated to be fatal within one year. Here, we present a case of miliary TB in a 21-year-old male with a remote exposure…

Parental agricultural work and selected congenital malformations.

The authors conducted a case-control study in Comunidad Valenciana, Spain, to assess the relation between occupational exposure to pesticides, mainly as a result of agricultural work, and the prevalence of congenital malformations. A total of 261 cases and 261 controls were selected from those infants born in eight public hospitals during 1993-1994. The cases were those who were diagnosed with selected defects (nervous system, cardiovascular, oral clefts, hypospadias/epispadias, musculoskeletal, and unspecified anomalies) during their first year of life. Information on occupational exposures a

Study protocol for the Australasian Cerebral Palsy Musculoskeletal Health Network (AusCP MSK) prospective cohort study: early detection of musculoskeletal complications in young children with moderate to severe cerebral palsy (GMFCS III-V).

BACKGROUND: Cerebral palsy (CP) is the most common physical disability of childhood, affecting movement and posture, resulting from a neurological insult during pregnancy or the neonatal period. While the brain lesion is static, the musculoskeletal sequelae in CP are often progressive and lifelong, associated with pain and can impact the lives of children with CP, their families and the healthcare system. The Australasian Cerebral Palsy Musculoskeletal Health Network (AusCP MSK) study will conduct comprehensive, population-based surveillance of children with moderate to severe functional mobil

A prospective field study of U.S. Army trainees to identify the physiological bases and key factors influencing musculoskeletal injuries: a study protocol.

BACKGROUND: Musculoskeletal injuries (MSKIs) are common in military trainees and present a considerable threat to occupational fitness, deployability, and overall military readiness. Despite the negative effects of MSKIs on military readiness, comprehensive evaluations of the key known and possible risk factors for MSKIs are lacking. The U.S. Army Research Institute of Environmental Medicine (ARIEM) is initiating a large-scale research effort, the ARIEM Reduction in Musculoskeletal Injury (ARMI) Study, to better understand the interrelationships among a wide range of potential MSKI risk factor

Effects of hyperthyroidism in the development of the appendicular skeleton and muscles of zebrafish, with notes on evolutionary developmental pathology (Evo-Devo-Path).

The hypothalamus-pituitary-thyroid (HPT) axis plays a crucial role in the metabolism, homeostasis, somatic growth and development of teleostean fishes. Thyroid hormones regulate essential biological functions such as growth and development, regulation of stress, energy expenditure, tissue compound, and psychological processes. Teleost thyroid follicles produce the same thyroid hormones as in other vertebrates: thyroxin (T4) and triiodothyronine (T3), making the zebrafish a very useful model to study hypo- and hyperthyroidism in other vertebrate taxa, including humans. Here we investigate morph

Maximum-Entropy-Rate Selection of Features for Classifying Changes in Knee and Ankle Dynamics During Running.

This paper investigates deteriorations in knee and ankle dynamics during running. Changes in lower limb accelerations are analyzed by a wearable musculoskeletal monitoring system. The system employs a machine-learning technique to classify joint stiffness. A maximum-entropy-rate method is developed to select the most relevant features. Experimental results demonstrate that distance travelled and energy expended can be estimated from observed changes in knee and ankle motions during 5-km runs.

Imaging Spectrum of Extrathoracic Tuberculosis.

The incidence of extrathoracic tuberculosis (ETB) continues to increase slowly, especially in immunocompromised and multidrug-resistant tuberculosis (TB) patients. ETB manifests with nonspecific clinical symptoms, and being less frequent, is less familiar to most physicians. Imaging modalities of choice are computed tomography (lymphadenopathy and abdominal TB) and MR imaging (central nervous system and musculoskeletal system TB). ETB commonly involves multiple organ systems with characteristic imaging findings that permit accurate diagnosis and timely management.

[Hearing loss in childhood combined with abnormalities].

Hearing loss was diagnosed in 923 of 1712 children attending our paedaudiology outpatient department over a period of two years. 14% of patients with hearing loss had one or more associated malformations, affecting the musculoskeletal system, the heart, the eye and central nervous system. Metabolic disorders were also found. In addition to specific otologic procedures such as surgery and hearing aids a close cooperation with several specialists, in particular human geneticists, is necessary.

Kaposi Sarcoma mimicking pedal osteomyelitis in a patient with HIV.

Kaposi Sarcoma (KS) is an angio-proliferative mesenchymal neoplasm that typically affects the skin. In the setting of AIDS, it is usually disseminated, commonly involving noncutaneous sites like oral cavity, lymph nodes, pulmonary, and gastrointestinal systems. Musculoskeletal system involvement by KS is rare, and when encountered, it typically involves the axial skeleton (vertebrae, ribs, sternum, and pelvis) and/or maxillofacial bones. This report describes an unusual case of a 44-year old patient with HIV, who presented with a foot ulcer that fit the typical clinical features of osteomyelit…

The association of congenital spinal anomalies with imperforate anus.

Ninety-four patients with congenital imperforate anus were studied to investigate: (1) the incidence of congenital spinal anomalies; (2) the orthopedic significance of these anomalies; (3) the incidence of other musculoskeletal disorders; and (4) the incidence of anomalies in other organ systems in the anal-spine anomaly group. There was a 38% incidence of spinal anomalies in 94 patients with congenital imperforate anus. Most of these abnormalities were sacral, and there were no related or progressive spinal deformities. The incidence of other musculoskeletal anomalies was much lower than of t

Gene expression of Postn and FGF7 in canine chordae tendineae and their effects on flexor tenocyte biology.

Chordae tendineae, referred to as heart tendinous cords, act as tendons connecting the papillary muscles to the valves in the heart. Their role is analogous to tendons in the musculoskeletal system. Despite being exposed to millions of cyclic tensile stretches over a human's lifetime, chordae tendineae rarely suffer from overuse injuries. On the other hand, musculoskeletal tendinopathy is very common and remains challenging in clinical treatment. The objective of this study was to investigate the mechanism behind the remarkable durability and resistance to overuse injuries of chordae tendineae…

Characterization of PD-1/PD-L1 immune checkpoint expression in the pathogenesis of musculoskeletal Langerhans cell histiocytosis: A retrospective study.

Recent data suggest that programmed cell death -1 (PD-1) and programmed cell death ligand-1 (PD-L1) are involved in the pathogenesis of Langerhans cell histiocytoma (LCH); however, their contributions are not well established. Also, the involvement of PD-1/PD-L1 molecules in musculoskeletal LCH remains particularly unclear. The current study aims to characterize the involvement of PD-1/PD-L1 immune checkpoint system in the pathogenesis of musculoskeletal LCH. PD-1/PD-L1 expression was evaluated in 6 patients, 3 men and 3 women with a mean age of 13.5 years, with musculoskeletal LCH who were tr
