Only recently the relationship between trauma and acute renal failure has been described on a rational basis. Shock is commonly a major pathogenetic factor in tubulo-interstitial damage often associated with other causes, such as toxic agents, toxins, immunologic and coagulation mechanisms. The Authors consider the various patterns of acute renal failure such as the "functional, tubulo-interstitial, cortical necrosis" and analyze the different etiopathogenesis. Finally they report the mortality rate of acute renal failure in S. Giovanni Hospital of Turin from 1969 to 1979.

An indirect haemaglutination method was developed for the immunochemical detection of myoglobin in human urine. Myoglobin was found in the urine of 84% of 44 patients with acute myocardial infarction. In contrast, it was detected in the urine of 2.7% of 147 control subjects and in none of a control group of 15 patients who had received intramuscular injections. A 13-year-old girl who had severe brain damage due to unknown prenatal cause presented rhabdomyolysis triggered by a mild viral infection. Her muscle biopsy revealed mild variation in fiber size and type 2 fiber atrophy without excess lipid storage. Biochemical analysis of the biopsied material showed decreased carnitine palmitoyltransferase (CPT) activity (15% of the control). Serum and urinary carnitine levels were normal. Trauma-induced rhabdomyolysis and myoglobinuric renal failure are well recognized. The mechanism of trauma in reported cases has been burns, crush injuries, or compression syndromes. We report a case of myoglobinuric renal failure in a 5-year-old boy who suffered primarily lacerations and penetrating wounds from a wolf attack. Rhabdomyolysis, a clinical syndrome resulting from the release of skeletal muscle cell contents into the plasma, is causally connected to up to 25 percent of all cases of acute renal failure. Critical care nurses need to be aware of precipitating factors and proper interventions in order to assure optimal function in this patient population.
