Sarcoid and the nail: review of the literature.

Nail involvement in sarcoidosis is rare, but its presence is often associated with chronic systemic disease. We have reviewed all 33 reported cases of nail involvement with sarcoidosis, including our own case. Nail changes in sarcoidosis, while rare, can include dystrophy (n=10, 30%), onycholysis (n=7, 21%), subungual hyperkeratosis (n=7, 21%), nail hyperkeratosis (n=2, 6%) and longitudinal ridging (n=8, 24%). All reported cases of nail sarcoidosis occurred in the setting of systemic involvement. In almost half of the cases, bony cysts were noted on radiological examination. The presence of na

Dermatoscopy of nail lichen planus.

BACKGROUND: Nail lichen planus affects 10% of all patients with lichen planus. It is a severe disease that may lead to destruction of the nail plate. It affects fingernails more than toenails. Early diagnosis is important due to its aggressive behavior. Histopathology should be carried out, but in many occasions it is not enough to come to a conclusive diagnosis. Dermatoscopy, a complementary tool, has proven to be useful in its diagnosis, management, and prognosis. Currently, there is very little data regarding dermatoscopy of nail lichen planus. METHODS: Dermatoscopic photographic data of 1…

Drug-related nail disease.

Drug-induced nail abnormalities can present with a wide variety of clinical manifestations that are often dose-related and that disappear after drug withdrawal. Some nail changes are asymptomatic and only cause cosmetic problems, whereas others cause pain and discomfort and impair manual activities or deambulation. New side effects have been recently observed with different groups of anticancer agents. These include painful onycholysis and subungual abscesses caused by taxanes and anthracyclines (eg, doxorubicin) in addition to ingrown nails, paronychia, and pyogenic granuloma associated with …

[Electric blue finger nail].

Glomus tumors are benign tumors and are most often localized acral. They usually are diagnosed non-invasively by patient's history, clinical examination, duplex ultrasound and MRI-angiography. Need for intervention depends on the symptoms.

Verrucous carcinoma of the nail unit.

Verrucous carcinoma (VC) of the nail bed and matrix is a rare malignancy that is often misdiagnosed as a benign condition. We present a case of a 65-year-old woman with a 2-year history of a progressive right fourth fingernail verrucous plaque replacing and surrounding her entire fourth fingernail. Diagnosis of VC was confirmed by a longitudinal biopsy and patient underwent en bloc excision and skin graft placement. The patient remained disease free with acceptable function of the digit at 24 months of follow-up. Although uncommon, VC must be considered in all patients with a persistent and en

[Pleuritis in yellow nail syndrome].

A 76-year-old man presented clinically with coughing and shortness of breath and was diagnosed radiologically to have massive pleural effusion as a combined feature of yellow nail syndrome. A lung biopsy was taken and revealed histologically: chronic non-specific inflammation in the pleuropulmonary border, intrapleural edema with eightfold pleural thickening in comparison to normal, angiogenesis in both the nutritive and functional intrapleural blood vessels, no abnormalities of lymphatic vessels with normal topographical distribution as detected by immunohistochemistry for antibody D2-40, gra

A misdiagnosed nail bed melanoma.

Nail bed melanoma or subungual melanoma is frequently misdiagnosed compared to other melanoma in other anatomic sites. We report the case of a 48-year-old woman presenting with a dystrophic and pigmented lesion of her fourth finger nail. This initial presentation had been mistaken for onychomycosis, but biopsy of nail bed and nail matrix confirmed nail bed melanoma. This case is presented to help increase the awareness of atypical presentations of acral melanoma.

Nail involvement in epidermolysis bullosa.

Nail abnormalities are a common feature in most subtypes of epidermolysis bullosa (EB), and they recently have been included among the criteria for scoring EB severity. Trauma undoubtedly contributes to the development of nail dystrophy, and for this reason the great toenails often are affected more severely. The nail abnormalities may be the first or the only symptom of EB. Nail abnormalities observed in EB are not specific or pathognomonic, as they result from nail bed and matrix scarring. The spectrum of clinical severity is large, and nail abnormalities may cause severe disability or just …

The burden of nail psoriasis: an introduction.

Nail involvement is an extremely common feature of psoriasis and affects approximately 50% of patients. Despite this, it is an often overloo … ficant functional impairment and psychological stress. The often distressing appearance of affected nails impacts the patient tremendously in both work and social activities. Importantly, 80% of patients with psoriatic arthritis have nail psoriasis. This review discusses the incidence of nail psoriasis and the burden on the patient and illustrates when …

Nail infection by Aspergillus ochraceopetaliformis.

Aspergillus ochraceopetaliformis is a rare fungal species that has not yet been identified as a proven human pathogen. Here we report a case of a toenail infection in a healthy woman caused by this fungus. Species identification was performed by scrutinizing the phenotypic and genetic characteristics of distinct isolates obtained at different times during the course of the infection. Treatment with terbinafine plus ciclopiroxolamine was effective.

The red nail--always benign?

Red nail is a common disorder. However, a thorough examination of the patient's nails allows one to distinguish different diseases. For example, longitudinal erythronychia may be monodactylous or polydactylous. Only the former type can show histologically Bowen's disease or an amelanotic melanoma. Among the polydactylous type, the nosologic position of the acantholytic and dyskeratotic naevus versus Darier's disease is still controversial. Acantholytic epidermolysis bullosa looks like bullous Darier's disease, and acrokeratosis of Hopf may present also red longitudinal streaks. Subungual warty…

Management of psoriatic nail disease.

Nail involvement is common at some point in the life of the patient with psoriasis. Simple hand care, keeping nails cut short and avoiding nail trauma, will all help in management. Medical interventions include topical therapies used for psoriasis at other body sites, directed at the location of the disease within the nail unit. Individual digits may require focused intensive treatment, such as steroid injections. Systemic therapy for psoriatic nail disease can be justified when the disease presents in tandem with severe skin disease or where function and quality of life are sufficiently dimin…
