Kidney stone disease: risk factors, pathophysiology and management.

Nephrolithiasis is the most common health condition affecting the kidney and urinary tract and constitutes a major global health-care problem. The prevalence of nephrolithiasis has increased substantially over the past five decades, irrespective of age, sex or ethnicity. Kidney stones cause substantial morbidity, reduced quality of life and enormous health-care expenditure, largely due to their frequent recurrence. Furthermore, nephrolithiasis is now recognized as a systemic condition associated with increased risks of chronic kidney disease, cardiovascular disease, metabolic syndrome and low

The "Tri-Glide" Technique: A Case Report on a Novel Intraoperative Approach for Removal of Retained and Encrusted Ureteral Stents.

BACKGROUND: Retained ureteral stents can result in significant morbidity and can be surgically challenging to urologists. A multimodal approach is often necessary for removal, potentially including retrograde and antegrade procedures performed over multiple anesthetic sessions. We describe the novel "Tri-Glide" technique for treating retained stents, particularly those with stent shaft encrustation prohibiting safe removal. Case Presentation. Two patients with nephrolithiasis and retained, encrusted ureteral stents were managed with the "Tri-Glide" technique. Patient #1 was a 58-year-old man w

Kidney Disease: Kidney Stones.

Kidney stones have a prevalence of 8.8% in the United States, with men affected more than women (10.6% versus 7.1%). Most stones are composed of calcium oxalate (61%). Calcium phosphate (15%) and uric acid (12%) stones are the second and third most common types. Risk factors include renal and ureteral anatomic abnormalities, family history, previous stones, older age, and various drugs. Factors that increase the risk of stone development include obesity, older age, metabolic syndrome, diabetes, and hypertension. Symptoms include renal colic, dysuria, urinary frequency, hematuria, fever, flank

Delayed emergence from propofol anesthesia in a patient with Lesch-Nyhan syndrome: A case report.

RATIONALE: Lesch-Nyhan syndrome (LNS) is an X-linked recessive disorder presenting with uric acid overproduction, neurocognitive disability, and behavioral disturbances. Inhalational anesthesia has been frequently used in LNS patients undergoing surgery. Characteristic compulsive self-injurious behavior and high risk of emesis may hinder inhalational induction. Propofol may be beneficial for these patients because of its easy and rapid titration for anesthetic depth during induction, early recovery from anesthesia, and antiemetic effect as well as uricosuric effect. PATIENT CONCERNS: A 16-yea

Retrograde intrarenal surgery in Nepal: an early experience.

With the advancement in technology and miniaturization of instruments, retrograde intrarenal surgery (RIRS) with flexible ureteroscope is gaining popularity. Flexible ureteroscope is introduced into renal collecting system through the urethra. Holmium YAG (Ho-YAG) Laser fiber of different sizes is introduced through the ureteroscope and renal stones are pulverized. Removal of renal stones less than 15 mm in size with RIRS has sharply reduced post operative morbidity. This is a Prospective study comprised of 58 RIRS performed from January 2013 to July 2013. Preoperative investigations like full
