A Novel Framework for Network-Targeted Neuropsychiatric Deep Brain Stimulation.

Deep brain stimulation (DBS) has emerged as a promising therapy for neuropsychiatric illnesses, including depression and obsessive-compulsive disorder, but has shown inconsistent results in prior clinical trials. We propose a shift away from the empirical paradigm for developing new DBS applications, traditionally based on testing brain targets with conventional stimulation paradigms. Instead, we propose a multimodal approach centered on an individualized intracranial investigation adapted from the epilepsy monitoring experience, which integrates comprehensive behavioral assessment, such as th

Median nerve compression in the carpal tunnel--functional response to experimentally induced controlled pressure.

Controlled external compression was applied to the medium nerve of 16 volunteer subjects. Tissue fluid pressure in the carpal canal was monitored with a wick catheter and pressures of 30, 60 and 90 mm Hg were induced for periods varying from 30 to 90 minutes.l Sensory and motor conduction and two-point discrimination were continuously monitored. Tissue compression at 30 mm Hg caused mild neurophysiological changes and symptoms of hand paresthesias. Compression at both 60 and 90 mm Hg induced a rapid, complete sensory conduction block which consistently preceded a motor block by 10 to 30 minute

A novel approach for monitoring writing interferences during navigated transcranial magnetic stimulation mappings of writing related cortical areas.

BACKGROUND: It has recently been shown that navigated repetitive transcranial magnetic stimulation (nTMS) is useful in preoperative neurosurgical mapping of motor and language brain areas. In TMS mapping of motor cortices the evoked responses can be quantitatively monitored by electromyographic (EMG) recordings. No such setup exists for monitoring of writing during nTMS mappings of writing related cortical areas. NEW METHOD: We present a novel approach for monitoring writing during nTMS mappings of motor writing related cortical areas. COMPARISON WITH EXISTING METHOD(S): To our best knowledg

Diagnostic criteria and therapeutic implications of rapid-onset demyelinating polyneuropathies.

Guillain-Barré syndrome (GBS) and acute-onset chronic inflammatory demyelinating polyneuropathy (A-CIDP) are the most common autoimmune polyneuropathies. Their aetiology is unclear. The pathomechanism includes damage mainly to the myelin sheath and, in the long-term process, secondary axonal loss. Both inflammatory polyneuropathies involve different combinations of motor, sensory and autonomic fibres in the peripheral nerves. The differential diagnosis should be based on clinical and neurophysiological features, and laboratory tests. Numerous studies aim to demonstrate the most common errors i

Multimodal Neuroimaging and Electrophysiological Markers in Multiple Sclerosis: An Integrative Review of fMRI, EEG, and EMG Approaches : fMRI, EEG, EMG in Multiple Sclerosis.

Multiple sclerosis (MS) is a chronic neurological disease marked by demyelination, neurodegeneration, and widespread network dysfunction. While conventional MRI remains central to diagnosis, advanced techniques such as functional MRI (fMRI), electroencephalography (EEG), and electromyography (EMG) are increasingly recognized for their ability to capture dynamic functional changes that underlie clinical symptoms. This review explores the individual and combined applications of fMRI, EEG, and EMG in MS, emphasizing recent clinical findings from 2019 to 2024. fMRI provides high-resolution mapping

Axonal-demyelinating differences in functional change in pediatric Charcot-Marie-Tooth and related neuropathies.

BACKGROUND: Charcot–Marie–Tooth disease and related neuropathies (CMTR) are heterogeneous inherited neuropathies with variable progression, yet data on pediatric disease progression by neurophysiological subtype are limited. We evaluated 1-year clinical and functional changes in children with CMTR and compared axonal versus demyelinating trajectories. METHODS: We conducted a prospective cohort study of 20 Thai children with CMTR who were aged 3–20 years. Participants were assessed at baseline and after 1 year using the Rasch-modified Charcot–Marie–Tooth Examination Score (CMTES-R), Charcot–Mar

Brainwaves under medication: revealing class-specific neural signatures of psychotropic medication from 24,000 EEGs.

BACKGROUND: Psychotropic medications remain foundational in psychiatric care, yet the neurophysiological mechanisms through which they exert therapeutic and adverse effects are still poorly characterised, limiting the field's ability to optimise treatment selection and monitoring. Electroencephalography (EEG) offers a non-invasive, real-time window into brain function that could support more precise, mechanism-informed prescribing; however, progress has been constrained by the absence of sufficiently large and systematically analysed pharmaco-EEG datasets. METHODS: In this cross-sectional obs

Excessive daytime sleepiness in narcolepsy and central nervous system hypersomnias.

PURPOSE: Excessive daytime sleepiness (EDS) is the core complaint of central nervous system (CNS) hypersomnias. In this mini-review, we summarized EDS features in CNS hypersomnias to provide a guide for differential diagnosis purposes. METHODS: A review of recent literature was performed to provide an update in CNS hypersomnias. RESULTS: At clinical evaluation, narcolepsy patients report a good restorative potential of sleep together with the frequent occurrence of dreaming even during short-lasting naps. These features are mirrored by the neurophysiological evidence of REM sleep at sleep on

An observational study comparing the performance of TOF-Cuff with TOF-Scan monitoring during anaesthetic induction in clinical routine.

INTRODUCTION: Neuromuscular monitoring by acceleromyography assesses the effects of non-depolarising neuromuscular blocking agents used during anaesthesia induction to optimise intubation conditions. A new type of neuromuscular monitor, TOF-Cuff, integrates electrode stimulation into a blood pressure monitoring cuff. Comparisons of this device with TOF-Scan, considered a clinical standard acceleromyography device, have not been published. MATERIAL AND METHODS: This prospective, observational study was approved by the Ethics Committee East Switzerland (BASEC-nr. 2016-02044), and patients' cons

Deep neuromuscular block does not improve surgical conditions in patients receiving sevoflurane anaesthesia for laparoscopic renal surgery.

BACKGROUND: Deep neuromuscular block is associated with improved working conditions during laparoscopic surgery when propofol is used as a general anaesthetic. However, whether deep neuromuscular block yields similar beneficial effects when anaesthesia is maintained using volatile inhalation anaesthesia has not been systematically investigated. Volatile anaesthetics, as opposed to intravenous agents, potentiate muscle relaxation, which potentially reduces the need for deep neuromuscular block to obtain optimal surgical conditions. We examined whether deep neuromuscular block improves surgical

Optimization of direct cortical stimulation using tibial versus median nerve sensory mapping during midline brain tumor resection: illustrative case.

BACKGROUND: During brain tumor resection, neurophysiological mapping and monitoring help surgeons locate, characterize, and functionally assess eloquent brain areas in real time. The selection of mapping and monitoring targets has implications for successful surgery. Here, the authors compare direct cortical stimulation (DCS) as suggested by median nerve (MN) with posterior tibial nerve (PTN) cortical sensory mapping (SM) during mesial lesion resection. OBSERVATIONS: Recordings from a 6-contact cortical strip served to generate an MN and a PTN sensory map, which indicated the strip was anteri

Case Report: Ictal Central Apnea as First and Overlooked Symptom in Temporal Lobe Seizures.

Ictal respiratory changes have been mainly described following generalized tonic-clonic seizures and recently considered to be a biomarker to assess the risk of sudden unexplained death in epilepsy (SUDEP). Nonetheless, modification of respiratory pattern can be related also to focal seizures, especially arising from the temporal lobe. Changes in cardiac function such as tachycardia or bradycardia could be often associated. We report a short case series of four patients with temporal lobe epilepsy admitted to our Epilepsy Monitoring Unit (EMU) presenting with an ictal central apnea as the firs

Narcissism and risky decisions: a neurophysiological approach.

Narcissists are prone to risky decision-making, but why? This study tested-via behavioral and event-related potential (ERP) measures-two accounts: deficiencies in error monitoring and deficiencies in action updating. High and low narcissists were engaged in a monetary gambling task by choosing between a high-risk and a low-risk option while the electroencephalogram (EEG) was being recorded. Two ERP components relevant to outcome evaluation-feedback-related negativity (FRN) and P3-were analyzed, with the FRN serving as an index of error monitoring and the P3 as an index of action updating. Mood congruent tuning of reward expectation in positive mood: evidence from FRN and theta modulations.

Positive mood broadens attention and builds additional mental resources. However, its effect on performance monitoring and reward prediction errors remain unclear. To examine this issue, we used a standard mood induction procedure (based on guided imagery) and asked 45 participants to complete a gambling task suited to study reward prediction errors by means of the feedback-related negativity (FRN) and mid-frontal theta band power. Results showed a larger FRN for negative feedback as well as a lack of reward expectation modulation for positive feedback at the theta level with positive mood, re

A multi-centre multi-national survey of anaesthetists regarding the range of anaesthetic and surgical practices for paediatric scoliosis surgery.

Institutional responses to a detailed multi-national survey were used to characterise the range of current anaesthetic and surgical practices for paediatric scoliosis surgery. Questions addressed surgical practice, anaesthetic agents, blood-sparing techniques, neurophysiological monitoring used and recalled major complications. Twenty-seven (87%) institutions responded. The median number of cases of these institutions was 40 per year (range 5 to 700). Common practices included inhaled volatile anaesthetic maintenance (80%), omission of nitrous oxide (81%), intravenous remifentanil (88% [range …
