Food for thought: re-emergence of tardive dyskinesia amid nutritional rehabilitation of atypical anorexia nervosa; a case report.

BACKGROUND: Atypical anorexia nervosa is an eating disorder that involves significant restriction of food intake leading to clinically significant weight loss despite remaining at a normal or above normal weight for age or height. Part of rehabilitation involves increasing calorie intake for nutritional rehabilitation and weight gain to an appropriate percentage of ideal body weight, determined based on patient weight history. This report highlights the importance of considering how increased food intake in a previously malnourished patient can affect the metabolism of medications they are tak

Pseudo-obstruction, enteric dysmotility and irritable bowel syndrome.

New diagnostic techniques have advanced our knowledge about the irritable bowel syndrome. The majority of patients that we believed to have a psychosomatic disorder have received other diagnoses explaining their symptoms. Endoscopy makes it possible to diagnose celiac disease before it leads to malnutrition and allows the detection of microscopic colitis as a cause of watery diarrhea. At the severe end of the symptom spectrum enteric dysmotility marks the border at which IBS ceases to be a functional disorder and becomes a genuine motility disorder. Joint hypermobility or Ehlers-Danlos syndrom

Longitudinal multimorbidity trajectories shape personalized glycaemic patterns.

Older individuals often live with diverse combinations of chronic diseases. However, whether multimorbidity contributes to glycaemic dysregulation remains unclear. Here we show that cumulative disease trajectories shape interindividual glycaemic variability throughout the ageing process. Tracking 1,398 participants in the Guangzhou Nutrition and Health Study cohort over 12 years, we develop a systemic multimorbidity index (MMI-system) that reflects the cumulative burden of chronic disorders. We also measure individual glycaemic dynamics and responses to dietary challenges using continuous gluc

[The treatment of intolerance for enteral nutrition of unknown origin with parenteral nutrition].

In recent years, there has been a notable rise in referrals to specialised chronic intestinal failure centres of patients with self-reported intolerance to enteral feeding. Despite incomplete or inconclusive diagnostic procedures and no convincing clinical evidence for the presence of intestinal failure, patients often urge physicians to start with total parenteral nutrition (TPN). The frequent use of opioids in these patients is a complicating factor concerning the diagnostic workup for motility disorders and the interpretation of clinical symptoms. Despite TPN administration, these patients

Impact of Transgenerational Nutrition on Nonalcoholic Fatty Liver Disease Development: Interplay between Gut Microbiota, Epigenetics and Immunity.

Nonalcoholic fatty liver disease (NAFLD) has emerged as the most prevalent pediatric liver disorder, primarily attributed to dietary shifts in recent years. NAFLD is characterized by the accumulation of lipid species in hepatocytes, leading to liver inflammation that can progress to steatohepatitis, fibrosis, and cirrhosis. Risk factors contributing to NAFLD encompass genetic variations and metabolic disorders such as obesity, diabetes, and insulin resistance. Moreover, transgenerational influences, resulting in an imbalance of gut microbial composition, epigenetic modifications, and dysregula

[Small bowel transplantation].

Intestinal transplantation is the logical alternative to definitive total parenteral nutrition in patients with chronic intestinal failure. It has become a lifesaving procedure for patients with intestinal failure who cannot be treated using conventional therapies. In children (over 50% of the recipients) indications include short gut syndrome, primary disorders of intestinal motility and mucosal diseases. In adults, the major indication for intestinal transplantation is inadaptable short bowel syndrome after total or subtotal resection. Patients with irreversible intestinal failure and total

"At Least the Second Plate": rethinking nutritional care in the geriatric unit.

Adequate nutritional care is important to reduce adverse outcomes in hospitalized older adults. However, achieving adequate nutritional intake in geriatric units is challenging due to appetite-related disorders, chronic diseases, anabolic resistance, and limitations associated with oral nutritional supplementation. In this context, healthcare professionals often encourage patients to consume "at least the second plate," which is usually centered on protein-rich foods, as an attempt to preserve muscle mass and support recovery. However, it remains unclear whether this strategy alone is sufficie

Nutritional state of elderly women on admission to mental hospital.

Since nutritional deficiencies might worsen the severity of symptoms and prolong the length of illness in non-nutritional disorders, particularly in the elderly, we examined the nutritional status of 216 elderly women newly admitted to a mental hospital. Compared to healthy elderly women, they had lower values for plasma prealbumin, vitamin C, and B vitamins. This was particularly common in senile dementia, and appeared to be the result of inadequate intake of protein or vitamins. Regular hospital diet for one month corrected the very low levels of prealbumin, but supplements were essential to

Serial Measurements of Refractive Index, Glucose and Protein to Assess Gastric Liquid Nutrient Transport-A Proof-of-Principal Study.

Delayed gastric emptying contributes to complications as aspiration or malnutrition. Among patients suffering from acute neurological diseases, motility disorders are prevalent but poorly understood. Thus, methods to measure gastric emptying are required to allow for appropriate adaptions of individual enteral nutrition algorithms. For enterally fed patients repetitive concentration measurements of gastric content have been proposed to assess gastric emptying. This approach can be used to calculate the gastric residual volume (GRV) and transport of nutrition formula (NF), but it has not yet be

Effects of multimodal nutritional intervention on nutritional status and clinical outcomes in patients with systemic sclerosis: study protocol for a randomised controlled trial.

INTRODUCTION: Systemic sclerosis (SSc) is an autoimmune disorder causing progressive fibrosis of internal organs and skin, leading to increased thickness and rigidity. It frequently results in malnutrition, which can decrease survival rates and physical performance, while also imposing financial burdens on healthcare systems and society. Multimodal nutritional interventions may encompass various approaches, including nutritional counselling, consumption of oral nutritional supplements and disease-specific dietary regimens based on nutritional needs. The present clinical trial is designed to ex

Nutritional bone disease in Indian population.

Syndromes of bone disease and deformities consequent to disorders of nutrition, bone and mineral metabolism constitute a serious national health problem. The studies on this subject are scanty. Data on nutritional bone disease are described and discussed. We had surveyed 337.68 million population residing in 0.39 million villages in 22 States of India during the period 1963 to 2005. Of the 4,11,744 patients identified with the disorders of bone and mineral metabolism, 2,13,760 (52%) had nutritional bone disease, 1,77,200 (43%) had endemic skeletal fluorosis and 20,784 (5%) had metabolic bone d

Psychiatric disorders and large interdialytic weight gain in patients on chronic haemodialysis.

AIMS: Psychiatric disorders have been considered in terms of non-compliant behaviour and low life quality in haemodialysis patients. The aim of this study is to investigate the potential association of psychiatric disorders with compliance of fluid restriction and nutritional status and to measure the effects of psychiatric disorders on the life quality in chronic renal failure patients on haemodialysis. METHODS: The study was conducted between April 2002 and December 2002 at a University hospital haemodialysis unit. The study population included 40 chronic renal failure patients (15 females/

Reassessment of albumin as a nutritional marker in kidney disease.

The decision by nephrologists, renal dietitians, federal agencies, health care payers, large dialysis organizations, and the research community to embrace serum albumin as an important index of nutrition and clinical performance is based on numerous misconceptions. Patients with analbuminemia are not malnourished and individuals with simple malnutrition are rarely hypoalbuminemic. With the possible exception of kwashiorkor, a rare nutritional state, serum albumin is an unreliable marker of nutritional status. Furthermore, nutritional supplementation has not been clearly shown to raise levels o

The role of intravenous hyperalimentation in intestinal disease.

This article has dealt briefly with intravenous nutrition in intestinal disorders. The indications for its use and techniques of nutritional assessment have been stressed. The use of intravenous hyperalimentation in a few of the more common diseases of the small bowel and colon has been discussed. Every patient with disease of the small or large intestine has some degree of dysfunction in the gastrointestinal tract. In many instances, this functional impairment interferes with normal ingestion or absorption of nutrients and predisposes the patient to malnutrition. The presence of malnutrition

[Nutrition and dietary supplements].

Nutritional problems occur very frequently in patients with cancer and different problems are associated with separate phases of the disease. Therefore, it is principally recommended to regularly screen all patients with cancer for nutritional disorders and in the case of conspicuous results meticulous diagnostics should be carried to clarify the underlying causes. The focus is on food intake and possible disturbing complaints, the physical performance index, nutritional status including weight change and body composition, the metabolic pattern and the presence of a systemic inflammatory react

A Narrative Review of Oral Hygiene and Pulmonary Health Amid Dysphagia: Implications for Feeding Route, Nutrition, and Quality of Life.

Oral health has significant implications for pulmonary outcomes, particularly among individuals with dysphagia who are at risk for aspiration. Moreover, oral health and condition affect nutrition accessibility and status. Inadequate oral hygiene promotes bacterial colonization, plaque accumulation, and aspiration-related respiratory complications. This narrative review aimed to explore current evidence and expert perspectives across palliative medicine, pulmonary and critical care, and dentistry on the role of oral hygiene in supporting pulmonary health and maintaining opportunities for oral n

Nutritional Status Differs by Prescription Opioid Use among Women of Reproductive Age: NHANES 1999-2018.

Prescription opioid use among pregnant women has increased in recent years. Prenatal exposure to opioids and poor nutrition can both negatively impact maternal-fetal outcomes. The objective of this study was to characterize the nutrition and health status of reproductive-age women taking prescription opioids, compared to women not taking opioids. Using NHANES 1999-2018 data, non-pregnant women aged 20-44 years were classified as taking a prescription opioid in the last 30 days (n = 404) or unexposed controls (n = 7234). Differences in anthropometric, cardiovascular, hematologic, and micronutri

Hospital Malnutrition in the Medicine and Neurology Departments: A Complex Challenge.

Hospital malnutrition is especially common among elderly patients with neurological deficits or dementia. These conditions can be exacerbated by unpalatable diets and issues such as dysphagia and presbyphagia. Our study aimed to investigate the prevalence of malnutrition in patients on a homogenized diet and to identify potential correlations with specific clinical variables. We conducted a retrospective observational study in compliance with the STrengthening the Reporting of OBservational studies in Epidemiology (STROBE) guidelines. The study encompassed 82 patients, mainly elderly and diagn
