Mapping the gendered dynamics of cesarean section: A scoping review.

Cesarean section (CS) is a lifesaving obstetric intervention when medically indicated, yet global rates have risen sharply over recent decades, with substantial variation across and within countries. While underuse persists in many low-resource settings, overuse is increasingly documented in higher-income and private-sector facilities, raising concerns about inequitable access, unnecessary surgical intervention, and divergent quality of maternity care. Existing explanations have largely focused on clinical indications and biomedical risk profiles, often neglecting the social and gendered struc

Learning from Ebola Alerts in the Pandemic Agreement Era: Preparedness, Trust, and Shared Responsibility.

Ebola alerts reveal the truth about preparedness. They show whether a health system can recognize danger early, protect health workers, isolate and care for patients, trace contacts, communicate clearly, and sustain public trust before fear replaces cooperation. In the Pandemic Agreement Era, this truth has become harder to ignore. The 2026 Bundibugyo virus disease outbreak in the Democratic Republic of the Congo and Uganda is a sharp reminder that the principles of pandemic prevention, preparedness, response, equity, and cooperation become meaningful only when they work where the first cases

The Costs of Parkinson's Disease in Europe: Results of the Costs of Illness in Neurology Initiative.

BACKGROUND: Parkinson's disease (PD) is the second most common neurodegenerative disease. It imposes substantial and growing burden on patients, families, and caregivers and reduces quality of life. The disability and care needs associated with PD carry economic ramifications. OBJECTIVE: The objective of the study was to estimate the socioeconomic costs of PD across 47 European countries in 2019. METHODS: Estimation is based on four steps: (1) determination of prevalence, (2) systematic review of costs, (3) data preparation and imputation, and (4) expert validation. Prevalence data were obta

From Safety Science to Operational Reality: Competing Institutional Logics in the Translation of National Patient Safety Recommendations.

BACKGROUND: National patient safety investigation bodies conduct system-level investigations and produce recommendations intended to drive improvement. Evidence that such recommendations translate into meaningful improvement is weak and the reasons for this are poorly understood. This paper examines how recommendations produced by the Health Services Safety Investigations Body (HSSIB), England's independent national patient safety investigation body, are produced by investigators and received by the national organisations to whom they are directed. METHODS: A qualitative multi-method design w

Getting Back on the Path to Eliminating HIV in Children.

INTRODUCTION: The global community proved remarkably resilient in protecting the progress in and commitment to eliminating HIV in children in the face of the COVID-19 pandemic. However, sudden disruptions in 2025 in donor support for HIV assistance have posed new and existential threats to the hard-fought gains in global paediatric HIV control. DISCUSSION: Since the early 2000s, the reductions in paediatric HIV acquisitions, in deaths in children with HIV and in the number of children orphaned by HIV/AIDS have been breathtaking, but in recent years, progress has stalled, and gaps remain. The

Towards a new psychiatric ontology in the era of artificial intelligence.

OBJECTIVES: Psychiatric classifications remain indispensable clinical tools, yet they do not define mental disorders as natural entities with stable boundaries. This article examines whether artificial intelligence may simply refine existing nosologies or contribute to the emergence of a new psychiatric ontology. METHODS: We conducted a conceptual and integrative analysis combining philosophical ontology, principles of computational ontology engineering, and recent literature on artificial intelligence, digital phenotyping, knowledge representation and psychiatric classification. The aim was

Trust in government, science, and vaccine confidence in Southeast Asia: A latent profile analysis.

Global immunization efforts have saved millions of lives, with vaccines playing a crucial role in mitigating disease transmission. However, the COVID-19 pandemic highlighted both increased vaccine uptake and rising skepticism, fueled in part by misinformation on social media. This trend extends beyond COVID-19, contributing to overall vaccine hesitancy and a resurgence of vaccine-preventable diseases. Southeast Asia presents a diverse context for examining vaccine confidence, with varying political systems, economic development, and information ecosystems shaping public trust in science, autho…

Global health and the dialectics of solidarity through Ubuntu and European perspectives.

The concept of solidarity plays a central role in shaping both African and European cultural responses to social and political challenges, although its interpretations diverge significantly. In African societies, solidarity is rooted in the relational philosophy of ubuntu, where individual identity is understood as inseparable from the community, emphasising collective well-being and mutual care. This is reflected in practices, such as community-based caregiving for the sick and elderly, traditional healing networks and shared child-rearing. In parts of West Africa, for example among the Igbo,

Exploring 'failure to fail' behaviour among examiners of undergraduate medical programs.

BACKGROUND: Even though a lot has been written on 'failure to fail' behaviors of educators in developed and resource-rich countries, there have been no studies from developing and resource-constrained countries, which are among the major contributors of international medical graduates to the healthcare systems worldwide. Understanding such behaviors of examiners has significant implications for professionalism, patient safety, and healthcare worldwide. Using game theory, the current study explores the determinants of 'failure to fail' underperforming medical students from examiners of six medi

Two Years of Revolutionizing Oral Health in Tanzania: Evaluation of a Multicomponent Government-Led Model.

Background: In 2022, the Tanzanian government launched a comprehensive multicomponent reform aimed at revolutionizing the national oral health care system. This initiative is part of a broader commitment to align with global health standards and address the significant oral health challenges faced by the population. Prior to the reforms, Tanzania's oral health infrastructure was marked by outdated practices and high prevalence rates of oral diseases, including dental caries and periodontal disease. Methods: We employed a qualitative case study design, guided by Walt and Gilson's Policy Triangl

Strengthening the mpox response: how do we balance pragmatism and equity in resource-constrained settings?

Mpox, previously neglected as an endemic zoonotic disease in West and Central Africa, has become a significant global threat following its second declaration as a public health emergency of international concern. With the growing number of cases, the region faces several challenges, particularly inadequate surveillance and diagnostics, and a lack of vaccine access. Drawing on lessons from public health emergency response in the Economic Community of West African States region, we aim to inspire a new vision with health emergency preparedness and response framework transformed for implementing

The SABER School Feeding policy tool: a 10-year analysis of its use by countries in developing policies for their national school meals programs.

Since its launch in 2011, 59 governments have used the World Bank's Systems Approach for Better Education Results (SABER) policy tool to design their national school-based health and nutrition programs. This tool guides governments to self-evaluate their education system policies against international benchmarks and identify actionable priorities to strengthen national programs. Thirty-two of the 49 countries in sub-Saharan Africa (65%) have undertaken a SABER review, and globally the approach has been adopted by 68% of the world's low-income countries and 54% of lower-middle-income countries.

Protocol to assess bottlenecks inhibiting the scaling up of evidence-based family planning practices in low-income and middle-income countries using mixed methods.

INTRODUCTION: Scaling up evidence-based practices (EBPs) in family planning (FP), as recommended by the WHO, has increasingly been accepted by global health actors as core to their mission, goals and activities. National policies, strategies, guidance, training materials, political commitment and donor support exist in many countries to adopt and scale up a range of EBPs, including postpregnancy FP, task sharing for FP and the promotion of social and behaviour change (SBC) for FP. While there has been some success in implementing these practices, coverage remains inadequate in many countries.

Evaluating policies and regulations used to control corruption among accounting officers in the public sector of South Africa: a systematic literature review.

Corruption, a global phenomenon, affects countries worldwide, transcending geographic and political boundaries, and continues to escalate, partly due to the inefficiency of financial and legal institutions and lack of enforcement. This has significant implications for Africa, hindering democracy, development, and poverty alleviation efforts. Defined as the intentional misuse of official authority for personal benefit, corruption is often fueled by public sector financial managers or accounting officers embezzling funds. The increasing pressure on these officers to justify public fund usage, co

Mapping Eastern (EEE) and Venezuelan Equine Encephalitides (VEE) among Equines Using Geographical Information Systems, Colombia, 2008-2019.

INTRODUCTION: Eastern equine encephalitis virus (EEEV) and Venezuelan equine encephalitis virus (VEEV) viruses are zoonotic pathogens affecting humans, particularly equines. These neuroarboviruses compromise the central nervous system and can be fatal in different hosts. Both have significantly influenced Colombia; however, few studies analyse its behaviour, and none develop maps using geographic information systems to characterise it. OBJECTIVE: To describe the temporal-spatial distribution of those viruses in Colombia between 2008 and 2019. METHODS: Retrospective cross-sectional descriptiv

The Chronic Cholera Situation in Africa: Why Are African Countries Unable to Tame the Well-Known Lion?

Seven years to the Global Taskforce on Cholera Control's target of reducing cholera cases and deaths by 90% by 2030, Africa continues to experience a high incidence of the disease. In the last 20 years, more than 2.6 million cases and 60 000 deaths of the disease have been recorded, mostly in sub-Saharan Africa. Case Fatality Ratio remains consistently above the WHO-recommended 1% with a yearly average of 2.2%. Between 1 January 2022 and 16 July 2023, fourteen African countries reported 213 443 cases and 3951 deaths (CFR, 1.9%) of the disease. In this perspective article, based on available li

An exploration of person-centred approach in end-of-life care policies in England and Japan.

BackgroundIncreasing evidence has suggested that a person-centred approach (PCA) is beneficial not only for improving care outcomes but also for mitigating the pressure on public health systems. However, policy implementation gaps have prevented the translation of this complex framework into useful practical, ethical and moral stances for end-of-life care (EOLC). This article aims to explore the meaning and implications of person-centredness in EOLC policy discourses.MethodsBy perceiving policy documents as a medium embodied with socio-political and cultural norms, we analysed how PCA in EOLC

Medication for opioid use disorder in the Arab World: A systematic review.

BACKGROUND: Opioid use disorder (OUD) is a global public health concern. The standard of care for OUD involves treatment using medications such as buprenorphine, methadone, or naltrexone. No known review exists to assess the contextual factors associated with medication for opioid use disorder (MOUD) in the Arab World. This systematic review serves as an implementation science study to address this research gap and improve the uptake of MOUD in the Arab World. METHODS: Systematic searches of Medline, PsycINFO, and EMBASE, and a citation analysis, were used to identify peer-reviewed articles w

The COVID-19 pandemic and the "Plague of Athens": comparable features 25 centuries apart.

Motivated by the up to now disastrous outcomes of the COVID-19 pandemic, we attempted a flashback to the so-called "Plague of Athens," which … while the ancient one started on the second year of a catastrophic civil war. Rivalry and different political systems between now (US/China) and then (Athens/Sparta) superpowers were the basis for conspiracy scenarios, concerning origins of the pandemics, which resulted to huge numbers of deaths, particularly in overcrowded and poor areas/cities. Sexual assault and fatal violence against women during the Irish War of Independence, 1919-1921: Kate Maher's murder in context.

At the height of the Irish War of Independence, 1919-1921, 45-year-old Kate Maher was brutally raped. She subsequently died of terrible wounds, almost certainly inflicted by drunken British soldiers. This article discusses her inadequately investigated case in the wider context of fatal violence against women and girls during years of major political instability. Ordinarily her violent death would have been subject to a coroner's court inquiry and rigorous police investigation, but in 1920, civil inquests in much of Ireland were replaced by military courts of inquiry. With the exception of med
