Anterior cervical spine fusion and sleep disordered breathing.

The authors reviewed 12 patients who developed obstructive sleep apnea (OSA) syndrome in association with anterior cervical spine fusion. Four subsequent patients were studied prospectively before C2 to C4 anterior fusion and documented to have OSA by questionnaire, visual analogue scales, polysomnography, and multiple sleep latency tests. The authors found that placement of the anterior cervical plates reduced the size of the upper airway. Symptoms and objective findings were controlled with nasal continuous positive airway pressure.

Evaluation of the adult with sleep apnea.

Twenty-seven cases of adult sleep apnea are reported. The patients were studied with all-night polysomnography. Seven (26%) had predominately obstructive sleep apnea, while another seven (26%) had predominantly central sleep apnea. Thirteen patients (48%) were found to have a mixed pattern. Obesity was not a reliable guideline for predicting differential etiology. We conclude that sleep studies with specific monitoring are essential in the evaluation of the adult with sleep apnea prior to therapeutic intervention.

Not what it seems to be: Depression versus periodic limb movement disorder.

Sleep disorders often disturb sleep. Daytime symptoms of disturb sleep mimic that of depression, somatoform disorder, fibromyalgia and chronic fatigue syndrome. We are presenting a case of depression who was not responding to antidepressant therapy. Based on clinical history, diagnosis was changed to chronic fatigue syndrome and in view of prominent sleep disturbances, polysomnography was done. Based on sleep study data, diagnosis of periodic limb movement disorder was made and he was started on ropinirole, that improved his symptoms.

[Diagnosis and treatment of a case of obstructive sleep apnea syndrome with AutoSet].

The author presents the case of a male patient with obstructive sleep apnoea syndrome, in which the use of classic CPAP therapy after performing an uvulo-pharyngoplasty (UVPP) led to very good results. The pressure levels determined initially after a polysomnography with Apnoe Check were compared with the results of the measurements with another device, AutoSet, used for the first time in our country. This allowed the tapering of pressure levels for CPAP and also revealed other advantages of the device, reviewed here.

[Isolated respiratory insufficiency in Arnold-Chiari malformation].

We report on a 45-year old woman with daytime sleepiness, polycythemia, hypoxemia and hypercapnia, admitted to hospital on three occasions in a 10 month period for acute respiratory failure. Polysomnography demonstrated apneas of central type, testing of the respiratory drive suggested central alveolar hypoventilation and magnetic resonance imaging showed an Arnold-Chiari malformation with syringomyelia. The originality of this case is the absence of any neurologic sign, respiratory failure being the sole manifestation of the Arnold-Chiari malformation.

Sleep-disordered breathing.

Obesity is a critical factor in the development of sleep-disordered breathing (SDB). Snoring is the most frequent nocturnal symptom suggesting a diagnosis of SDB. Other common nighttime symptoms include snorting, gasping, choking, coughing, and witnessed apneas. The most frequent diurnal symptom in SDB is excessive daytime sleepiness. Patients suspected of having SDB should undergo a full night of in-laboratory polysomnography or in-home oligosomnography. SDB includes a spectrum of disorders; the most common are obstructive sleep apnea and central sleep apnea.

[Management of restless legs and periodic movements of sleep].

INTRODUCTION: Restless legs syndrome and periodic leg movements in sleep are frequently observed, often in association. Misdiagnosis of these two conditions is common. STATE OF ART: Physical examination can provide the diagnosis of restless legs syndrome but polysomnography recording must be performed to confirm the presence of periodic leg movements. Dopaminergic pathways are implicated in the pathophysiology of these two syndromes and the same treatment is given: dopaminergic agents, benzodiazepines, opioids and/or antiepileptic drugs. PERSPECTIVES AND CONCLUSION: Treatment provides an imp…

Diagnosis and pathophysiology of obstructive sleep apnea in children.

Factors that decrease pharyngeal size or increase pharyngeal compliance increase an individual's risk for OSA. In children, these factors include craniofacial disorders, central nervous system problems, or simply adenotonsillar hypertrophy. Although children with OSA may present with nocturnal symptoms of snoring, respiratory pauses, secondary enuresis, and/or nightmares, their initial complaint may be poor school performance, hyperactivity, or other symptoms of sleep fragmentation. The diagnosis is confirmed by overnight polysomnography in a laboratory with expertise in the care of infants an…

Obstructive sleep apnea.

Obstructive sleep apnea is diagnosed with the use of polysomnography by documenting repetitive periods of upper airway closure during sleep. Apneic episodes can compromise gas exchange and disrupt the sleep pattern. The challenge to the clinician is to suspect this diagnosis when evaluating the awake patient. The history of loud snoring plus daytime hypersomnolence or multiple awakenings during sleep justifies ordering a polysomnogram. The importance of diagnosing obstructive sleep apnea is underscored by the resolution of its sequelae with effective medical or surgical treatment.

Effects of DSIP on narcolepsy.

Repeated injections of delta sleep-inducing peptide (DSIP) were given to a 35-year-old male narcoleptic. Effects on wakefulness and sleep were evaluated by self-reports, performance tests, multiple sleep latency test and all-night polysomnography. DSIP reduced the frequency of sleep attacks and increased activity, alertness and performance during day-time. The sleep period was compressed by DSIP with enhancement of REM sleep. The results suggest that the effects are due to an accentuation of circadian and ultradian rhythms by DSIP.

The use of the odds ratio product and self-reported data to detect comorbid insomnia and sleep apnea.

STUDY OBJECTIVES: To evaluate the utility of the odds ratio product (ORP) in differentiating comorbid insomnia and sleep apnea (COMISA) from obstructive sleep apnea (OSA) and chronic insomnia (CIN). METHODS: We retrospectively analyzed 9750 patients in four groups: (1) 1152 controls, (2) 2395 with CIN, (3) 2297 with OSA, and (4) 3906 with COMISA. CIN was defined as difficulty initiating/maintaining sleep with daytime fatigue/sleepiness occurring "often"/"always." OSA was defined as an apnea-hypopnea index ≥5 on polysomnography. ORP, computed every 3 s from polysomnography, was analyzed alongs…

Quantitative assessment of nocturnal neural respiratory drive in children with and without obstructive sleep apnoea using surface EMG.

NEW FINDINGS: What is the central question of this study? Recent studies have suggested potential utility of non-normalized respiratory muscle EMG as an index of neural respiratory drive (NRD). Whether NRD measured using non-normalized surface EMG of the lateral chest wall overlying the diaphragm (sEMGcw) recorded during nocturnal clinical polysomnography can differentiate children with and without obstructive sleep apnoea (OSA) is not known. What is the main finding and its importance? Non-normalized sEMGcw was increased in children with OSA and an additional group of snoring children without…

Obstructive sleep apnea and nocturnal enuresis in the pediatric population: a systematic review and meta-analysis.

BACKGROUND: The relationship between nocturnal enuresis (NE) and obstructive sleep apnea (OSA) among the pediatrics population is considered … uresis among children with OSA, with a pooled prevalence of 40% (95% CI: 30%–51%). In studies using polysomnography, prevalence was 43% (95% CI: 32%–53%). Eight studies assessed the association between OSA and enuresis, showing a significant relationship (OR = 2.30; 95% CI: 1.37–3.84), which remained consistent in the polysomnography subgroup (OR = 2.62; 95% CI: …

Thoracoabdominal Asynchrony in Healthy Children.

Measuring asynchrony between the thorax and abdomen during sleep provides information about inspiratory effort and upper airway obstruction in children, aiding in the diagnosis of obstructive sleep apnea (OSA). This study aimed to define a normative range for thoracoabdominal asynchrony (TAA) in healthy children by studying polysomnographies of 113 children aged 10-13 years. The percentage of total sleep time (TST) spent with elevated TAA, > 40°, was measured, as well as obstructive apnea-hypopnea index (OAHI), sleep position, sleep stage and snoring. Subjects without measurable OSA (N = 94) w

Feasibility and Acceptability of the Somnomat Casa Rocking Bed as a Stimulation Substitute for a Patient with Rhythmic Movement Disorder.

Rhythmic Movement Disorder (RMD) is a parasomnia characterized by repetitive, rhythmic movements such as headbanging or body rocking. It typ … ed to baseline, and temporal shifts in episode occurrence were observed during intervention phases. Polysomnography suggested potential changes in sleep architecture linked to vestibular stimulation. These findings support the Somnomat Casa as a promising non-pharmacological intervention for RMD, warranting further investigation in larger clinical trials.

Using the modified OSA-questionnaire to determine the prevalence of a high risk of severe obstructive sleep apnea in children with Down syndrome.

Children with Down syndrome often experience obstructive sleep apnea (OSA), which can be severe and, if untreated, leads to serious complications. Polysomnography, the gold standard for diagnosing OSA, has long waiting lists and poses challenges for these children. The modified OSA-18 questionnaire may help determine the severity of OSA in this population, facilitating the study of its prevalence and risk factors. A cohort of 180 children with Down syndrome, aged 2-12 years, was enrolled from August 2020 to January 2022. Participants completed the modified OSA-18 questionnaire, where a score o…
