Central pontine myelinolysis. A case report.

The first case of central pontine myelinolysis in Scandinavia is presented. A premortem diagnosis was established on the basis of the history of chronic alcoholism, the picture of severe electrolyte disturbance and the development of the neurological syndrome. The postmortem findings verified the clinical diagnosis. A possible aetiological mechanism is postulated. The cause is believed to be a damage to the energy dependent electrolyte transport across the cell membrane, resulting in a cytotoxic oedema and death of the oligodendroglial cells of the brainstem which are particularly vulnerable.

A trembling detour to the diagnosis of encephalitis: a neuroborreliosis case report.

In this Clinical Rounds, we present a case of neuroborreliosis in a man aged 59 years who initially complained of headache, tremor, and hiccups. Diagnosis was delayed, partly due to an atypical presentation but mainly because subtle MRI findings were overlooked. There is emerging recognition that radial and patchy hyperintensities in cerebral peduncles, mesencephalon, and pons is a distinct MRI feature of neuroborreliosis. Dissemination of this knowledge could contribute to more timely and efficient diagnoses in future patients.

Isolated medial longitudinal fasciculus syndrome: Review of imaging, anatomy, pathophysiology and differential diagnosis.

Isolated medial longitudinal fasciculus (MLF) syndrome due to infarction limited only to the midbrain is a rare occurrence. The MLF are a group of fiber tracts located in the paramedian area of the midbrain and pons. They control horizontal eye movements by interconnecting oculomotor and abducens nuclei in the brain stem. Such small infarcts can easily be overlooked by young neuroradiologists and trainees. In this review, we discuss the clinical and imaging characteristics, comprehensive review of the anatomy, pathophysiology, and differential diagnosis.

Central pontine myelinolysis in AIDS.

Central pontine myelinolysis (CPM) is an uncommon complication in sick patients with severe underlying disorders such as chronic alcoholism, malignancy, malnutrition and hyponatraemia. We report two patients with advanced HIV infection who developed CPM. In one case the diagnosis was not suspected in life, in the other the diagnosis was made just before death, on the basis of magnetic resonance (MR) imaging appearances. At post mortem there was a close correlation between the MR abnormalities and the anatomic changes in the pons.

Fourth ventricular ependymoma in a child with Duane retraction syndrome.

A child with unilateral Duane retraction syndrome developed signs and symptoms of increased intracranial pressure at 3 1/2 years of age. Neuroimaging disclosed a fourth ventricular ependymoma compressing but not invading the floor of the fourth ventricle. The coexistence of Duane syndrome and fourth ventricular ependymoma in a young child raises the possibility that both conditions could have resulted from a mutational event or focus of cellular disorganization (i.e. field defect) localized to the dorsal pons.

Central neurogenic hyperventilation in a patient with medulloblastoma.

Central neurogenic hyperventilation is a rare but important cause of the frequently observed phenomenon of hyperventilation. Its diagnosis demands that primary respiratory or metabolic causes, as well as cerebrospinal fluid abnormalities, be ruled out. We hereby describe a patient whose attacks of hyperventilation were the presenting sign of her disease and might be consistent with the central neurogenic mechanism. This patient suffered from a medulloblastoma which compressed the pons. After removal of the tumor the hyperventilation attacks ceased.

[Wide abdominothoracic approach for a tumor on the superior pole of a horseshoe kidney; supra and subumbilical median and anterolateral oblique thoraco-phrenic laparotomy (author's transl)].

The development of a tumor on the superior pole of a horseshoe kidney is an event which, although rare, still presents the surgeon very specific problems related to the anatomy of the malformation. The presence of a parenchymal pons, the multiplicity and anomalies in the origin of arteries are all factors to be taken into consideration in the selection of a surgical procedure. It is of particular importance that the approach be very wide in order to combine the advantages of the trans-peritoneal approach with that of the thoraco-phrenic laparotomy. One case is presented, and the surgical solut…

Large pontine tubercular abscess treated surgically.

Tuberculosis of the central nervous system (CNS) is a life threatening condition with 50% mortality in advanced disease and serious neurological deficits in those who survive. Tuberculous abscess is a rare manifestation of CNS tuberculosis, brainstem involvement being even rarer. The management of these conditions poses a great challenge to the treating physician. We report a case of large tubercular abscess of pons which increased in size on anti-tubercular treatment, but showed excellent improvement following craniotomy and aspiration.

Presumed metastasis of breast cancer to the abducens nucleus presenting as gaze palsy.

A 51-year-old woman with breast cancer presented with progressive diplopia. Neuro-ophthalmologic examination revealed right gaze palsy and peripheral facial nerve palsy. Brain magnetic resonance imaging (MRI) was normal. However, two months later a repeat brain MRI revealed an enhancing round nodular mass at the right facial colliculus of the lower pons, at the location of the abducens nucleus. Localized metastasis to the abducens nucleus can cause gaze palsy in a patient with breast cancer.

Positive feedback of efferent copy via pontine nucleus facilitates cerebellum-mediated associative learning.

The cerebellum is critical for motor coordination and learning. However, the role of feedback circuitry in this brain region has not been fully explored. Here, we characterize a nucleo-ponto-cortical feedback pathway in classical delayed eyeblink conditioning (dEBC) of rats. We find that the efference copy is conveyed from the interposed cerebellar nucleus (Int) to cerebellar cortex through pontine nucleus (PN). Inhibiting or exciting the projection from the Int to the PN can decelerate or speed up acquisition of dEBC, respectively. Importantly, we identify two subpopulations of PN neurons (PN

DIPG-like MYB-altered diffuse astrocytoma with durable response to intensive chemotherapy.

Pontine gliomas represent difficult to treat entity due to the location and heterogeneous biology varying from indolent low-grade gliomas to aggressive diffuse intrinsic pontine glioma (DIPG). Making the correct tumor diagnosis in the pontine location is thus critical. Here, we report a case study of a 14-month-old patient initially diagnosed as histone H3 wild-type DIPG. Due to the low age of the patient, the MRI appearance of DIPG, and anaplastic astrocytoma histology, intensive chemotherapy based on the HIT-SKK protocol with vinblastine maintenance chemotherapy was administered. Rapid clini

Recurrent haemorrhagic pontine cavernoma resection via a retrosigmoid microsurgical approach.

BACKGROUND: Brainstem cavernomas occasionally require surgical treatment. Appropriate patient selection and thorough understanding of the anatomy and technical nuances involved in microsurgical resection is a pre-requisite in undertaking these challenging cases. CASE DESCRIPTION: We present a video case of a patient with a recurrent haemorrhagic pontine cavernoma. A step-by-step commentary of surgical footage is provided along with clinical, anatomical and technical learning points pertinent to the safe surgical management of these lesions.
