There are limited data regarding the use of remimazolam in infants and children and its use in this population remains off label as it does not hold FDA-approval in pediatric-aged patients. Analgesic Techniques for Managing Orthopedic Injuries: A Review for the Emergency Clinician.

BACKGROUND: Orthopedic injuries are commonly managed in the emergency department (ED) setting. Fractures and dislocations may require reduct … , intra-articular injections, IVRA, and periosteal blocks, which provide adequate analgesia without procedural sedation or opioid administration. When used in isolation, these analgesic techniques decrease complications from sedation and the need for other medications, such as opioids, while decreasing ED length of stay. Emergency clinicians can also use these techni …

Disturbance of Key Cellular Subproteomes upon Propofol Treatment Is Associated with Increased Permeability of the Blood-Brain Barrier.

BACKGROUND: Propofol is a short-acting anesthetic, which is often used for induction and maintenance of general anesthesia, sedation for mechanically ventilated adults and procedural sedation. Several side effects of propofol are known and a substantial number of patients suffer from post-operative delirium after propofol application. In this study, we analyzed the effect of propofol on the function and protein expression profile on a proteome-wide scale. METHODS: We cultured human brain microvascular endothelial cells in absence and presence of propofol and analyzed the permeability of the b…

Effectiveness of premedication agents administered prior to nitrous oxide/oxygen.

BACKGROUND AND OBJECTIVE: In paediatric dentistry, when anxiety, fear of dental procedures or behavioural impairment precludes the conduct of dental treatments, sedation procedures are required. However, sedation at the desired level might not be achieved despite administration of various agents. The present study aimed to evaluate the effectiveness of oral premedication with different agents on children scheduled for dental treatment under nitrous oxide/oxygen (N2O/O2) sedation. METHODS: The present research was approved by the Ethics Committee of Gazi University Faculty of Medicine. Sixty c

Conscious Sedation in Dentistry.

Invasive dental procedures can be performed only with local anesthesia; in some cases, it may be useful to combine the administration of drugs to obtain anxiolysis with local anesthesia. Sedation required level should be individually adjusted to achieve a proper balance between the needs of the patient, the operator, and the safety of the procedure. Surgical time is an important factor for post-operative phases, and this could be greatly increased by whether the patient interrupts the surgeon or if it is not collaborative. In this manuscript some dentistry-used methods to practice conscious se

Tachycardia and muscle tremors following intravenous administration of detomidine to a horse undergoing standing surgery.

Detomidine hydrochloride is an alpha2 adrenoceptor agonist that is commonly used in equine practice to provide sedation and analgesia for horses undergoing standing procedures. Reported effects following intravenous administration of detomidine in horses include bradycardia, bradyarrhythmias, transient hypertension, sweating, piloerection and dose-dependent sedation, ataxia and analgesia. This case report describes a novel response characterised by tachycardia and profound muscle tremors following the administration of intravenous detomidine to a horse undergoing a standing procedure.

Midazolam as intravenous sedative for electrocardioversion.

Of the various agents which have been employed for sedation in patients undergoing electrocardioversion, diazepam has had the most extensive use. However, this agent possesses several disadvantages including pain and venous complications at the site of injection and a lower incidence of amnesia. Midazolam, a benzodiazepine derivative, is being increasingly used in general and local anesthesia as well as for procedures requiring conscious sedation, eg, endoscopy. We used intravenous midazolam for conscious sedation in 12 patients undergoing a total of 17 cardioversions. All of the patients expe

Efficacy and Safety of the Remimazolam-Alfentanil Combination for Sedation During Gastroscopy: A Randomized, Double-blind, Single-center Controlled Trial.

PURPOSE: Propofol infusion is a popular single drug of choice for sedation in the gastrointestinal endoscopy suite. Drug combinations are more beneficial than single-drug regimens in gastroscopy sedation. However, the cardiopulmonary complications of propofol sedation raise concern. Remimazolam is a novel, ultra-short-acting benzodiazepine sedative, and alfentanil is a weak opioid. During endoscopic procedures, remimazolam is an effective and safe sedative procedure. No synergistic effect has been reported when remimazolam was combined with alfentanil in gastroscopy sedation. Here, we evaluate

Patient Safety and Anesthesia Considerations for Office-Based Otolaryngology Procedures.

Office-based otolaryngology procedures provide a safe and efficient alternative to the traditional operating room. Physicians are responsible for knowing their state regulations and subspecialty guidelines. Although the clinic setting has fewer regulations than hospitals and ambulatory surgery centers, the clinic has the same standards as a hospital with respect to emergency equipment, trained personnel, protocols, and safety measures. Sedation occurs along a continuum; it is impossible to predict a patient's response to sedation. Otolaryngologists performing office-based sedation must be prep

Noninvasive Approaches for Anxiety Reduction During Interventional Radiology Procedures.

Periprocedural anxiety is a major cause of morbidity, particularly for interventional radiology procedures that often depend on conscious sedation. Management of anxiety and pain during image-guided procedures has traditionally relied on pharmacologic agents such as benzodiazepines and opioids. Although generally safe, use of these medications risks adverse events, and newer noninvasive, nonpharmacologic techniques have evolved to address patient needs. In this review, we explore the roles of hypnosis, structured empathic attention, anodyne imagery, music, video glasses, and mobile application

Gastric rupture after bag-mask-ventilation.

A 42 year old woman underwent bronchoscopy with procedural propofol sedation. During the procedure, the patient suffered respiratory arrest, and bag-mask ventilation was initiated. During forced mask ventilation, abdominal distention occurred. Even after correct placement of an endotracheal and a nasogastric tube, high inspiratory pressures persisted. The abdominal CT scan revealed a high amount of intraperitoneal free air. An emergent laparotomy confirmed a stomach rupture. Immediately after opening of the peritoneal cavity, peak ventilatory pressures decreased. In this case forceful bag-mask

Ultrasound-guided nerve blocks in the emergency department.

Peripheral nerve blocks preclude the need for procedural sedation and provide adequate anesthesia during painful procedures. This technique can be performed in the emergency department with the aid of ultrasound imaging to identify target nerves. We describe eight cases of upper and lower extremity nerve blocks performed under ultrasound guidance in the emergency department of the Jai Prakash Narayan Trauma Centre, All India Institute of Medical Sciences, New Delhi. Only two of the patients, both with extensive injuries, required any additional anesthesia during the subsequent procedures and a…

A retrospective analysis of the effects of moderate sedation on the degree of cardia exposure in overweight elderly patients.

BACKGROUND: Gastric cancer is a type of malignant gastrointestinal tumor that poses a serious threat to human life and ranks as the second leading cause of cancer-related mortality worldwide. Although cardia-related diseases are rarely reported in the literature on gastrointestinal disorders, cardia lesions are not uncommon in clinical practice. The development of esophageal cancer may be associated with risk factors for esophageal adenocarcinoma, such as reflux esophagitis, high-fat diet, advanced age, and obesity. During gastroscopy, it is important to note that inadequate visualization of t

[NON-CONVULSIVE EPILEPTIC SEIZURE IN PATIENT WITH DELAYED AWAKENING AFTER SUPRATENTORIAL TUMOR REMOVAL].

In some cases after elective neurosurgical procedures we use technique of delayed awakening of patients. Prolonged sedation however can hide the development of epileptic seizures and lead to the status epilepticus formation. This article is a demonstration and discussion of a clinical case of nonconvulsive status epilepticus during delayed awakening of the patient due to prolonged sedation after elective neurosurgical interventionsforsupratentorial brain tumor Presented case suggests the presence of significant risk of nonconvulsive status epilepticus during prolonged sedation. Superficial cervical plexus block for vocal cord surgery in an awake pediatric patient.

UNLABELLED: Medialization thyroplasty is a surgical procedure that decreases the incidence of dysphonia and dysphagia in patients who have vocal cord paralysis. We report a case of a pediatric patient who underwent this procedure with minimal sedation and bilateral superficial cervical plexus blockade. The use of a regional technique provided analgesia while allowing the patient to phonate at the request of the surgeon. IMPLICATIONS: Medialization thyroplasty is a surgical procedure that decreases the incidence of dysphagia and dysphonia in patients with vocal cord paralysis. This procedure i

Patient-reported tolerability of endoscopic retrograde cholangiopancreatography using conscious sedation in patients with underlying depression and anxiety.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is often performed under conscious sedation, which may increase pain and dissatisfaction. Mental health conditions may influence patient-reported experience measures (PREMs), including tolerability. This study explores the association between pre-existing anxiety and/or depression and ERCP tolerability using the validated Patient-Reported Scale for Tolerability of Endoscopic Procedure (PRO-STEP) to address this. METHODOLOGY: We performed a retrospective analysis of prospectively maintained data from an international observation
