Annular abscesses are serious complications of infectious native and prosthetic valve endocarditis. In this patient, we isolated Stenotrophomonas maltophilia, a rare cause of subaortic abscess with high mortality/morbidity rates although virulent gram-positive cocci, S. Aureus in particular, have been the most commonly isolated agents. We treated this case of endocarditis and the subannular abscess observed 1 year after the initial operation by aortic root replacement with resternotomy in addition to appropriate antibiotics.

BACKGROUND AND PURPOSE: Robotic-assisted total knee arthroplasty (rTKA) has been proposed to improve component positioning but evidence on its effect on implant survival is limited. We aimed to compare implant survival between rTKA and conventional total knee arthroplasty (cTKA) overall and due to peri-prosthetic joint infection (PJI).
METHODS: We included all rTKAs performed in Finland from April 2021 until May 2023, all implanted with Triathlon TKA using MAKO robotic-arm assist. Propensity score matching was used to form a control group with similar characteristics for the same time period.

Managing chronic periprosthetic infections in patients who have undergone limb-salvage surgery following a malignant bone tumor with megaprosthesis often involves a two-stage revision surgery with the use of a cement-spacer. This paper show details the preparation of a self-made intramedullary metal-stabilized mega-cement spacer for patients needing a two-stage revision surgery due to infection after oncologic bone tumor resection and limb-salvage surgery with megaprosthesis and present 2clinical cases treated with this technique. The report provides a practical surgical technique to create a

Aortic root reconstruction during aortic root replacement for a patient with prosthetic valve endocarditis and aortic root abscess can be a difficult procedure with many possible complications. In this video case report, we describe our novel technique using a single bovine pericardial patch that avoids deep stitches or external sutures to support the friable annulus. Compared with more standard methods, this approach has shorter cross-clamp and cardiopulmonary bypass times and is less demanding technically.

BACKGROUND Infective endocarditis (IE) is a severe, life-threatening, and relatively common complication after valve replacement operations, with incidence rates varying between 1.8% and 5.8%, with an in-hospital mortality rate of up to 20%. Common microorganisms are (listed by decreasing incidence) Streptococci, Staphylococcus aureus, Enterococci, bacteria of the HACEK group, and fungi. Treatment of IE is complex, typically involving prolonged courses of antibiotics. However, in cases of aortic prosthetic valve endocarditis, root abscess formation with involvement of the aorto-mitral skeleton

OBJECTIVE: Despite the improvements in xenogeneic grafts and surgical techniques, management of aortic graft infection has remained challenging. The optimal graft material has remained controversial, with high rates of reinfection using prosthetic grafts and a limited time for venous harvest in an emergent setting. Recent studies have highlighted an increase in the use of Omniflow II biosynthetic vascular grafts (LeMaitre Vascular, Burlington, MA) for aortic reconstruction. The primary aim of the present study was to review the key outcomes for the Omniflow II graft in terms of reinfection and

BACKGROUND: Graft infections are one of the most serious complications in vascular surgery, with high mortality rates. Few studies addressed risk factors associated with a higher susceptibility to infection. The aim of this study is to identify perioperative factors associated with aortic graft infections (AGI).
METHODS: We designed a retrospective, case-control study from patients subjected to open aortic repair between 2013 and 2019. Cases of AGI were defined according to the management of aortic graft infection collaboration (MAGIC) criteria and matched to controls without proven infection

BACKGROUND: The selection of the appropriate implant material, stainless steel or titanium, is still the decision of the surgeon and/or the affiliated institution. Additionally, remarkable international differences can be found between the different markets, which cannot really be explained.
OBJECTIVE: A systematic literature search was performed to verify whether there is clinical evidence for the preference of one material over the other.
MATERIAL AND METHODS: The systematic literature search was performed utilizing the internet databases PubMed, Cochrane and Web of Science. Comparative st

INTRODUCTION: Inflatable penile prosthesis (IPP) surgery has been performed for more than 40 years. IPP infection rates have decreased owing to advances in manufacturing and surgical technique but have remained a devastating complication.
AIMS: To describe the pathophysiology of infections, examine evidence associating clinical risk factors with IPP infection, assess the benefit of techniques aimed at preventing and managing infection, and discuss future directions.
METHODS: PubMed and Google Scholar were searched for studies relating to IPP infections.
MAIN OUTCOME MEASURES: A comprehensiv

A case is described in which a new treatment was taken to eradicate an aortic root abscess in a 56-year-old man with aortic prosthetic valve endocarditis. Debridement of all apparently infected tissue created left ventricular-aortic discontinuity, involving the orifice of the right coronary artery. A conduit of a diameter of 23 mm was made by hand with a glutaraldehyde preserved xenopericardial graft. A 21 mm St. Jude Medical prosthetic valve was sewn in it at 2 cm to its edge. The hand-made valved conduit was used to reconstruct the left ventricular outflow tract and aortic root. A saphenous

Replacement valve endocarditis occurred in 3.7% of 2443 patients who underwent primary or redo aortic valve replacements at The Prince Charles Hospital between December 31, 1969 and January 1, 1992, based on a cross-sectional follow-up in 1992 which was 98.8% complete. Because some patients had re-replacements during the study period, a total of 2686 operations were considered for analysis. A variety of replacement devices were used, including 571 allografts (21%), 1152 xenografts (43%), and 880 mechanical valves (36%). Insertion of an allograft valve resulted in a constant risk of endocarditi

The emergence of a Japan-intrinsic community associated methicillin-resistant Staphylococcus aureus strain (CA-MRSA/J) has been reported. A 70-year-old man with recurrent colon cancer and a history of mitral valve replacement was admitted to the hospital in a state of shock. He was diagnosed with prosthetic valve endocarditis (PVE) caused by MRSA and underwent cardiac surgery. The MRSA isolates belonged to multilocus sequence type 8 and carried staphylococcal cassette chromosome mec IVl and the genes of toxic shock syndrome toxin-1, enterotoxin C, and enterotoxin L. These characteristics indic

Development of pseudoaneurysm from an infected carotid prosthetic patch after carotid endarterectomy is extremely rare and is very challenging from the perspective of surgical and medical treatment. This article describes the case of a 65-year-old patient presenting with the signs of wound infection and recurrent bleeding in the region of the right carotid artery. Three years earlier the patient underwent coronary artery bypass grafting and bilateral replacement of carotid arteries with prosthetic tube grafts. Magnetic resonance imaging confirmed the presence of an infected pseudoaneurysm in t

Sixteen cases of infective endocarditis requiring valve replacement with a prosthesis are described. The indication for surgery was intractable congestive heart failure in all. Fourteen patients underwent aortic valve replacement, with five hospital deaths secondary to cardiac decompensation and one late death related to recurrent periprosthetic insufficiency. Two patients who required mitral valve replacement did well, with no early or late deaths. A collected review of 293 cases of left-sided primary infective endocarditis reported in the English literature confirms the feasibility, advisabi

Management of infected prosthetic arterial grafts has traditionally included total graft excision especially when gram-negative bacteria were cultured. Between 1973 and 1991 we treated 42 patients with infected prosthetic grafts (33 polytetrafluoroethylene (PTFE), 9 Dacron) by complete graft preservation when the graft was patent, the anastomoses were intact, and the patient did not have sepsis. The infection involved the anastomosis (36 cases) or the body (6 cases) of 33 peripheral grafts and the distal segment of five aortofemoral and four iliac-distal grafts. Cultures of the 42 infected gra
