A retrospective study of end-to-side venous anastomosis for free flap in extremity reconstruction.

INTRODUCTION: The success of end-to-side (ETS) venous anastomosis primarily depends on several mandatory factors including recipient vessel selection and reconstruction site. This retrospective study was designed to analyze the success rate of ETS venous anastomosis for free flap extremity reconstruction. METHODS: Between 2009 and 2011, ETS venous anastomosis was performed in 117 patients to reconstruct soft tissue defects of extremity at Nanjing Drum Tower Hospital. The patients were divided into three groups: (1) only one large recipient vein left, (2) two deep venas, (3) one deep vein and

New composite tissue allograft transplantation model in mouse with intravital microscopic evaluation of microcirculation.

A new mouse composite tissue allograft (CTA) transplantation model was developed to study the microcirculatory changes during acute allograft rejection and ischemia/reperfusion (I/R) injury. The donor cremaster muscle allografts were prepared as a tube flap, harvested on the common iliac vessels, transplanted to the neck region of the recipient, and anastomosed to the recipient's ipsilateral carotid artery and external jugular vein using standard end-to-end microsurgical technique. In Group 1 (n=6), the hemodynamics of cremasteric muscle microcirculation was measured in C57BL/6N mice without t

Outcome of cadaver kidney transplantation in small children.

Small children have often been reported to have poor outcome after kidney transplantation (KT). Recent reports from North America have shown that the use of living-related donors improves patient and graft survival. We report the experience in one centre of primary cadaveric KT using sequential immunosuppression in nine children aged 8-30 months and weighing 5.4-9.8 kg; donors were 0.7-12.3 years old. Four patients had pre-emptive KT and the other five were on peritoneal dialysis; the mean +/- SD waiting time was 2.0 +/- 2.4 months. Perioperative care has been published previously. The surgica

Vascularized anterolateral thigh free flap for salvage reconstruction of complex anterior skull base and nasion defects after failed conventional reconstruction- how I do it.

BACKGROUND: Sinonasal inverted papilloma (SNIP) is histologically benign but locally aggressive, with high recurrence rates and a relevant r … p was used. The pedicle was routed transcranially through an enlarged skull base defect to temporal recipient vessels, enabling three-layer reconstruction from dura to skin and restoration of nasal dorsum contour. RESULTS: Definitive reconstruction was completed in 7 h. The flap survived without vascular compromise. During the postoperative course and early follow …

Lymph-interpositional flap transfer promotes lymphatic flow restoration: A rabbit hindlimb model supports the lymph axiality concept.

BACKGROUND: Lymph-interpositional flap transfer (LIFT) is a novel lymphatic reconstruction technique that maintains alignment between donor and recipient lymphatic vessels-termed lymph axiality-without requiring lymph node harvest. Despite its conceptual rationale and growing clinical application, experimental validation remains limited. We aimed to evaluate the mechanistic impact of lymphatic interposition on lymphatic regeneration using a rabbit hindlimb model. METHODS: A rabbit hindlimb model was developed. Limbs were assigned to either a LIFT (+) group (lymph axiality maintained via lymph

Mesenchymal stem cells and exosomes in ischemic brain injury: a review.

Stroke poses a serious threat to human health and life, serving as a leading cause of death and disability in adults. The incidence rate of stroke continues to rise annually. Following the onset of ischemic stroke, most patients experience a period of spontaneous recovery. Neural repair after cerebral ischemia is closely associated with neurovascular plasticity, which facilitates the regeneration and repair of nerves and blood vessels in the ischemic injury area. Mesenchymal stem cells (MSCs), adult stem cells isolated from bone marrow or other tissues, can differentiate into various cell type

Sequential Free Flaps in Lower Extremity Reconstruction.

INTRODUCTION: Sequential free flaps are often utilized for complex defects, particularly for recurrent head and neck tumors. However, their … complex scenarios, it is often prudent to utilize preoperative vascular imaging and to use distinct recipient vessels from those used in the first flap. Overall, sequential free flaps can be used in complex cases of limb salvage; however, they are associated with a substantial risk of complications. LEVEL OF EVIDENCE: Therapeutic-IV.

Techniques for covering soft tissue defects resulting from plantar ulcers in leprosy: Part V. Use of the flap in the inguinal region and latissimus dorsi musculocutaneous flap.

The area of distribution of the superficial circumflex iliac, superficial epigastric and superficial external pudental arteries is large and flaps based on them can meet the requirement of different recipient sites. We have transplanted free flaps based on the superficial epigastric artery for repairing plantar soft tissue defects in six leprosy patients. During the follow-up examination 58 to 118 months later there has been no recurrence of ulceration in any of these cases. The latissimus dorsi muscle, is mainly nourished by the thoracadorsal artery and the latissimus dorsi musculocutaneous f

Determining factors in relation to lymphovascular characteristics and anastomotic configuration in supermicrosurgical lymphaticovenous anastomosis - A retrospective cohort study.

INTRODUCTION: Supermicrosurgical lymphaticovenous anastomosis (LVA) can be performed in different configuration such as end-to-end (LVEEA), end-to-side (LVESA), and side-to-end (LVSEA). Each configuration has its own advantages and disadvantages. However, it has remained ambiguous as to which anastomotic o configuration to choose. The aim of this study is to analyze and compare the relative sizes of lymphatic vessel (LV) and recipient vein (RV), in attempts to provide the basis for proper selections of the anastomotic configuration. METHODS: From March 2016 to October 2018, 100 lymphedema pat

Preoperative Vascular Interventions to Improve Donor Leg Perfusion: A Report of Two Fibula Free Flaps Used in Head and Neck Reconstruction.

PURPOSE: For reconstruction of head and neck defects, the fibula free flap is the first choice at many institutions. The main contraindication for fibula harvest is the lack of 3-vessel runoff, which leads to postoperative vascular compromise of the lower extremity. Atherosclerosis is the most common disease, which can limit the use of this donor site. In general, vascular interventions, which include angioplasty, atherectomy, and stenting, have been used to fix arterial supplies using endovascular methods. The purpose of this study was to report on the outcome of a preliminary cohort of patie
