The malignant gastrointestinal endometriosis transformation is represented by endometriosis-associated intestinal tumors. Endometrioid adenocarcinoma and clear cell adenocarcinoma are most common among the endometrial cancers of all organs. Self expandable metal stents (SEMS) are a useful option to diverting colostomy for the palliation of malignant rectal obstruction. SEMS can be successfully placed in approximately 90% of cases with acceptable complication rates. Covered SEMS allow closure of malignant rectovaginal and rectovesical fistulae associated with rectal obstruction. The main drawback to these devices is the inability to palliate bleeding.

We reported 3 cases of rectal involvement by locally invasive prostate carcinoma which caused an urethrorectal fistula in case 1 and a rectal stenosis in case 2 and 3. Discriminating between primary rectal carcinoma and prostate carcinoma locally invasive to the rectum is very important because of distinct differences in treatment and prognosis. To confirm the tumor origin, tumor markers, proctosigmoidscopy, computerized tomography and biopsy with immunohistochemical staining for prostate specific antigen were useful.
