[Acute ischemia, autonomic reflexes, and ventricular fibrillation].

The effects of the autonomic nervous system on malignant arrhythmias, particularly in the setting of ischemic heart disease, have been widely investigated and described. Specifically, while sympathetic hyperactivity is arrhythmogenic, an increased vagal activity often exerts a beneficial effect. New insights on the relationship between autonomic activity and sudden cardiac death have been obtained in conscious dogs in which a healed myocardial infarction, acute myocardial ischemia and exercise are combined. In this chronic animal model myocardial infarction reduces baroreflex sensitivity and h…

Influence of vagal afferents in the sneeze reflex in cats.

We studied the effects of bilateral vagotomy and step pulmonary inflations (5, 10, 15 mmHg, i.e., 0.66, 1.33, 2 kKPa) on sneeze reflex in ketamine-anaesthetized cats. Bilateral vagotomy lengthens the duration of preparatory inspiration and diminishes the amplitude of expiratory activities in sneeze. In contrast, 5 mmHg pulmonary inflation facilitates the sneeze. It shortens the inspiratory preparation and increases the frequency of sneeze attacks. At 10 mmHg pulmonary inflations, inspiration is inhibited and only expiratory thrust occurs. At 15 mmHg pulmonary inflations, vagal afferent stimula…

[Changes in the deglutition reflex during the perioperative period].

The swallowing reflex is one of the major mechanisms protecting the lungs against aspiration of foreign material. Its complex time course requires a perfect coordination with ventilation, as the aerodigestive crossroads are common for both functions. Physiological factors as ageing, as well as mechanical and pharmacological agents, may contribute to swallowing reflex alterations. Moreover, endotracheal intubation and most anaesthetic agents may impair the swallowing function and favour the occurrence of aspiration. This review discusses the normal swallowing process and its alterations by agen…

The pathophysiology and pharmacology of photic cortical reflex myoclonus.

We studied 6 patients with myoclonus elicited by flash stimulation (1-15 Hz). Multichannel electromyographic recording showed a rostrocaudal recruitment order for the generalized myoclonic jerk. In all patients, each flash induced a large (36.9 +/- 5.7 microV) frontal biphasic wave with an onset latency of 42 msec that preceded the earliest muscle response in the face by at least 4 msec (range, 4-7 msec), the activity in the biceps by 11 to 14 msec, and the activity in tibialis anterior by 26 to 34 msec. Occipital potentials evoked by the same flash stimulation had a latency of 33.7 msec and w

Detection of hearing impairment with the acoustic stapedius reflex.

The acoustic stapedius reflex (ASR) is the sound-evoked contraction of the stapedius muscle. The ASR is mediated by a neural network, receiving its afferent input from the VIIIth cranial (auditory) nerve and sending its efferent output to the VIIth cranial (facial) nerve. Several centers in the brain stem comprise the central portion of the reflex pathway. The stapedius muscle in each ear responds to ipsilateral, contralateral, and binaural stimulation. The normal ASR threshold is 90 to 95 dB SPL for tones and 70 to 75 dB SPL for wideband noise. In ears with sensorineural hearing loss, ASR thr…

An excitatory body-antral reflex in the sheep abomasum.

This paper describes an abomasal body-antral excitatory reflex in acute experiments in sheep anaesthetized with chloralose. The abomasum was either surgically intact or acutely transected to form separate compartments of the abomasal body and antrum. Inflation of a balloon situated in the abomasal body and active contraction of the abomasal body increased the amplitude of antral contraction. In the intact preparation, inflation of the abomasal body increased the amplitude of antral contraction before and after extrinsic denervation. The antral response was reduced by 75% after extrinsic denerv…

Perioral reflexes in orofacial dyskinesia and spasmodic dysphonia.

In order to assess the clinical utility of trigemino-facial reflexes in lower facial muscles, we studied perioral reflexes to mechanical and electrical stimulation in 13 patients with spasmodic dysphonia and orofacial dyskinesia and in 7 healthy subjects. Mechanical stimulation of the upper lip of all patients and electrical stimulation of the infraorbital nerve of patients with orofacial dyskinesia elicited larger perioral reflexes than in controls. In the majority of patients, hyperexcitable perioral reflexes were accompanied by increased gain of the blink reflex. In 4 patients, however, tri…

Opioidergic inhibition of flexor and extensor reflexes in the rabbit.

1. Recordings were made from gastrocnemius medialis (GM), semitendinosus (ST) and tibialis anterior/extensor digitorum longus (TA/EDL) motor … stimulation at this site. 3. The opioid antagonist naloxone caused a dose-dependent increase in all reflexes evoked by electrical stimulation of the skin. The heel-GM, toes-ST and toes-TA/EDL reflexes all increased to more than 3 times control levels with naloxone, which also caused significant decreases in the latencies of these reflex responses. On the …

Differences of cortical activation in spontaneous and reflex myoclonias.

Frontal and parietal components of somatosensory evoked potentials (SEPs) following median nerve stimulation and scalp potentials preceding myoclonic jerks (jerk-locked averaging, JLA) were compared in 6 patients with cortical reflex myoclonus. Giant potentials were found over the parietal cortex in both conditions. Prominent frontal activity was detected following median nerve stimulation which, however, was absent in jerk-locked averages. Therefore an identical generator of the giant SEP and the JLA is unlikely. As the frontal component is lacking in jerk-locked averaging, the spontaneous je…

Pudendal reflexes and effects of conditioning stimuli in cats.

Evaluation of pudendal reflexes and effects of pudendal branch conditioning on those reflexes was carried out in 2 studies. In the first study of pudendal reflexes, 20 adult male and female mixed-breed cats underwent surgical isolation of the anal branch, urethral branch, and distal trunk (consisting primarily of the dorsal nerve of the penis/clitoris) of the pudendal nerve. Reflexes were tested in all possible ipsilateral and contralateral test-response combinations. Latency values and effects of increasing stimulus rate on response amplitude were recorded. Reflexes were detected in all combi…

Reversal of apnoea by aspiration reflex in anaesthetized cats.

Various flow and timing characteristics of breathing as well as electrocorticographic (ECoG) records were analysed in a model of reversible respiratory failure induced by N2 inhalation in 27 anaesthetized cats. During the first minute of respiratory arrest, nasopharyngeal stimulation by an elastic nylon fibre elicited a typical sniff- or gasp-like aspiration reflex (in 88.8% of cases), whilst similar tracheobronchial irritation evoked a weak cough reaction in one third of cases (34.2%). The aspiration reflex could also be evoked in progressive stages of apnoea characterized by very low and eve…

Cardiocardiac excitatory reflexes during myocardial ischemia.

Myocardial ischemia represents a stimulus capable of exciting the receptive endings of both the vagal and sympathetic sensory fibers which innervate the heart. As a consequence, multiple and simultaneous reflexes can occur, mediated by the vagal and the sympathetic afferent limbs which, in turn, can modify both the vagal and the sympathetic outflows. Reflexes mediated by vagal afferent fibers are predominantly inhibitory, while those mediated by sympathetic afferent fibers are largely excitatory. In the experimental laboratory the various reflexes can be artificially isolated and analyzed, whi…

Inhibition of spinal reflexes by paramedian reticular nucleus.

The inhibitory actions of the paramedian reticular nucleus (PRN), and its neighbouring structures, i.e., midline raphe nuclei (MRN) and dors … etely suppressed by simultaneous PRN stimulation. The EVRR, including mono- and polysynaptic spinal reflexes with transmission velocity from 10 to 60 m/sec or above, were also suppressed. MRN stimulation only inhibited the KnJ, CEM and polysynaptic spinal reflexes with transmission velocities between 25 and 60 m/sec, but facilitated spinal reflexes with …

Vestibulo-ocular reflex as a parameter of seasickness susceptibility.

The vestibulo-ocular reflex (VOR) is known to be modulated in response to changing vestibular and optokinetic stimuli. The purpose of this study was to investigate possible relationships between VOR and future susceptibility and habituation to seasickness. Thirty candidates for future maritime service were exposed to a series of yaw axis smooth harmonic accelerations before and after 6 months of regular sailing, and their VOR gain and phase responses were recorded. Seasickness severity was estimated after 1 and 6 months of service by a questionnaire. We conclude that VOR gain at 0.01 Hz may se…

[A method of determining bladder sphincter reflex in children].

A new method of recording the bladder sphincter reflux is suggested. It lies in carrying out urethra profilography followed by stimulation investigations. The data of profilography and stimulation investigations are compared. Standard parameters of the bladder sphincter reflux were defined in patients without derangement of urinary function and were compared to the findings obtained in patients suffering from disorders fo urinary function (hypotonia, hyporeflexia, reflux). The method can be used for diagnosing disordered interrelations between the urinary bladder and sphincter apparatus (ureth…

Evolution of skin reflexes in the first years of life.

The evolution of 28 skin reflexes was recorded. In a group of term infants (group A) the 14 limb reflexes disappeared by about 16 months of age and the 14 predominantly truncal reflexes persisted beyond that age. A second group of healthy infants (group B) were followed until the end of their third year. This study and the following cross-sectional study of healthy children from four to 14 years (group C) showed that the same 14 (limb) skin reflexes disappeared early at about the end of the first year. Another four limb reflexes disappeared during the second and third years. These 18 reflexes …

Monopolar recording of H reflexes at various sites.

Various monopolar recording electrode sites have been used to record H reflexes and M responses. This investigation revealed a decrease in maximum M response amplitude accompanied by an increase in the H/M amplitude ratio as the active recording electrode was positioned more distally, below the gastrocnemii muscle bellies. H and M latencies were also significantly longer at the most distal recording site, but the latency difference is relatively independent of recording site. Serial variation was least at the most proximal recording site for an immobilized ankle. For an unrestrained ankle, ser…

Effects of general anesthesia on the human blink reflex.

The blink reflex was studied in 10 patients undergoing elective procedures under general anesthesia. Anesthetic agents were isoflurane, halothane, nitrous oxide, methohexital, and thiopental in various combinations. At induction, blink reflexes were diminished by low-dose thiopental (1-1.5 mg/kg) and abolished by high-dose thiopental (4-8 mg/kg) and methohexital (1.5 mg/kg). Blink reflexes were absent during halogenated volitile inhalational anesthesia and did not return until patients were in the recovery room, well after end-tidal anesthetic levels were zero by mass spectroscopy. Recovery of…
