Traumatic vocal fold avulsion injury in a newborn.

A full-term newborn developed respiratory compromise in the immediate postparturition period requiring urgent intubation. Evaluation of post-extubation stridor later the same day revealed an avulsion injury extending from the left vocal fold into the lateral glottic musculature. Primary repair was accomplished with anatomic realignment of the torn vocal fold and muscle. Endotracheal intubation was utilized for stenting and the patient was extubated following 3 days of paralysis with sedation. Follow-up examination revealed a reparative granuloma, which was lasered. Eight-week follow-up examina

Chronic ventilatory failure caused by abnormal respiratory pattern generation during sleep.

A 67-yr-old man presented with a 4-yr history of chronic ventilatory failure in the absence of any restrictive or obstructive ventilatory defect. Detailed neurologic investigations were unremarkable, and the response of minute volume of ventilation to inhaled CO2 was normal. During sleep there was no evidence of upper airway obstruction, and minute volume of ventilation averaged 6.7 L/min, compared with 7.2 L/min during quiet wakefulness. However, sleep was associated with a rapid and shallow pattern of breathing, resulting in high dead space ventilation, inadequate alveolar ventilation, hypox

Periodic breathing in infants with histories of prolonged apnea.

Periodic breathing (PB) during sleep was investigated in two groups of full-term infants with histories of apnea that were terminated by resuscitation. One group of infants had been reported to be asleep whereas the other group had been reported to be awake when apnea was noted. Electrophysiologic sleep recordings were made after the apneic incident. The infants with histories of prolonged apnea while asleep exhibited more PB during recorded sleep than infants with histories of apnea while awake. Increased PB during sleep in full-term infants, therefore, may not be associated with all prolonge

Investigating phase coupling of respiratory sinus arrhythmia and slow wave sleep in obstructive sleep apnea patients with and without depression.

Phase coherence (λ) between respiratory sinus arrhythmia (RSA) and respiration has emerged as a promising metric for assessing the role of autonomic nervous system (ANS) activity and slow wave sleep (SWS) activities in healthy subjects. This study aims to investigate how λ and SWS activity differ between Obstructive Sleep Apnea (OSA) patients with and without major depressive disorder (MDD) during overnight sleep and explore whether the correlation between λ and SWS activity exists among those OSA patients compared to healthy individuals. Overnight electroencephalogram (EEG), electrocardiogram

Dendritic alterations precede age-related dysphagia and nucleus ambiguus motor neuron death.

Motor neurons (MNs) within the nucleus ambiguus innervate the skeletal muscles of the larynx, pharynx and oesophagus, which are essential for swallow. Disordered swallow (dysphagia) is a serious problem in elderly humans, increasing the risk of aspiration, a key contributor to mortality. Despite this importance, very little is known about the pathophysiology of ageing dysphagia and the relative importance of frank muscle weakness compared to timing/activation abnormalities. In elderly humans and in aged Fisher 344 (F344) rats, a variety of motor pools exhibit weakness and atrophy (sarcopenia),

Sleep Apnea and Cardiovascular Disease: Lessons From Recent Trials and Need for Team Science.

Emerging research highlights the complex interrelationships between sleep-disordered breathing and cardiovascular disease, presenting clinical and research opportunities as well as challenges. Patients presenting to cardiology clinics have a high prevalence of obstructive and central sleep apnea associated with Cheyne-Stokes respiration. Multiple mechanisms have been identified by which sleep disturbances adversely affect cardiovascular structure and function. Epidemiological research indicates that obstructive sleep apnea is associated with increases in the incidence and progression of corona

Respiratory and non-respiratory panic disorder subtypes: clinical and quality of life comparisons.

AIM: to compare the scores of the WHOQOL quality of life domains with the clinical features of patients with respiratory and non-respiratory panic disorders (PD) treated at the UFRJ Panic and Respiration Laboratory. METHOD: cross sectional study. Thirty-two PD outpatients under treatment were consecutively selected and evaluated by the MINI v.4.4. They were divided into two different groups according to Briggs et al. classification of respiratory and non-respiratory PD subtypes. Twelve (37.5%) patients had the respiratory subtype (SR) and 20 (62.5%) the non-respiratory subtype (SN). RESULTS:

Artificial Intelligence in the Assessment of Heart Rate Variability as an Instrument to Understand the Connection Between Psychologic and Psychiatric Conditions and the Heart.

Heart rate variability (HRV) refers to variations in the time intervals between consecutive heart beats. Changes in HRV reflect changes in either sympathetic or decreased parasympathetic tone that can originate in the brain. This brain-heart connection has led to the proposal that HRV may have utility in the diagnosis of psychiatric conditions and/or be a predictor of the response to psychiatric medications. There have been attempts to improve the correlation between HRV and psychological and psychiatric conditions by using artificial intelligence or specific machine learning algorithms. The o

Mutations in the Key Autophagy Tethering Factor EPG5 Link Neurodevelopmental and Neurodegenerative Disorders Including Early-Onset Parkinsonism.

OBJECTIVE: Autophagy is a fundamental biological pathway with vital roles in intracellular homeostasis. During autophagy, defective cargoes including mitochondria are targeted to lysosomes for clearance and recycling. Recessive truncating variants in the autophagy gene EPG5 have been associated with Vici syndrome, a severe early-onset neurodevelopmental disorder with extensive multisystem involvement. Here, we aimed to delineate the extended, age-dependent EPG5-related disease spectrum. METHODS: We investigated clinical, radiological, and molecular features from the largest cohort of EPG5-rel

Description of three dysfunctional breathing patterns in post-COVID dyspnea.

INTRODUCTION: Dysfunctional breathing (DB) can be defined as a change in breathing pattern associated with respiratory and/or systemic symptoms, after ruling out underlying respiratory or cardiac disease. Recent evidence suggests that DB contributes to dyspnea in post-COVID-19 syndrome (PCS), as demonstrated by ventilation analysis during cardiopulmonary exercise testing (CPET). Nevertheless, the lack of a standardized classification for the different subtypes of DB poses challenges for accurate diagnosis and effective management. We hypothesized that analyzing the evolution of breathing param

Rbm20ΔRRM Mice, Expressing a Titin Isoform with Lower Stiffness, Are Protected from Mechanical Ventilation-Induced Diaphragm Weakness.

Diaphragm weakness frequently develops in mechanically ventilated critically ill patients and is associated with increased morbidity, including ventilator weaning failure, mortality, and health care costs. The mechanisms underlying diaphragm weakness are incompletely understood but may include the elastic properties of titin, a giant protein whose layout in the muscle's sarcomeres makes it an ideal candidate to sense ventilation-induced diaphragm unloading, resulting in downstream signaling through titin-binding proteins. In the current study, we investigated whether modulating titin stiffness

Successful treatment of respiratory dysfunction in cystinosis by nocturnal non-invasive positive pressure ventilation.

Cystinosis is a rare metabolic disorder characterized by lysosomal cystine accumulation leading to multi-organ damage, with kidneys being clinically first affected. Longer survival of cystinosis patients due to successful renal replacement therapy, revealed previously unknown extra-renal symptoms of cystinosis, generally appearing after the first decade. Respiratory insufficiency caused by overall respiratory muscle myopathy is a severely invalidating and sometimes a life-threatening complication of cystinosis. We report a successful treatment of hypoventilation, due to diaphragm myopathy in a

Imaging obstructed ventilation with NMR using inert fluorinated gases.

We partially obstructed the left bronchi of rats and imaged an inert insoluble gas, SF(6), in the lungs with NMR using a technique that clearly differentiates obstructed and normal ventilation. When the inhaled fraction of O(2) is high, SF(6) concentrates dramatically in regions of the lung with low ventilation-to-perfusion ratios (VA/Q); therefore, these regions are brighter in an image than where VA/Q values are normal or high. A second image, made when the inhaled fraction of O(2) is low, serves as a reference because the SF(6) fraction is nearly uniform, regardless of VA/Q. The quotient of

Periodic breathing in siblings of sudden infant death syndrome victims.

To determine the incidence of periodic breathing (PB) episodes during sleep, 48 subsequent sudden infant death syndrome siblings and 48 control infants, matched by sex, gestational age, and chronologic age, were studied by home pneumogram technique. The sudden infant death syndrome siblings had a statistically significant increase in the percent of sleep time spent in PB, in the number of episodes/100 minutes of PB, in the longest episode PB, and in the average duration of all episodes of PB. We conclude that PB is present in a higher amount in sudden infant death syndrome siblings as compared

Respiratory manifestations of panic disorder in animals and humans: a unique opportunity to understand how supramedullary structures regulate breathing.

The control of breathing is commonly viewed as being a "brainstem affair". As the topic of this special issue of Respiratory Physiology and Neurobiology indicates, we should consider broadening this notion since the act of breathing is also tightly linked to many functions other than close regulation of arterial blood gases. Accordingly, "non-brainstem" structures can exert a powerful influence on the core elements of the respiratory control network and as it is often the case, the importance of these structures is revealed when their dysfunction leads to disease. There is a clear link between

Signs and symptoms of sleep-disordered breathing in trauma survivors: a matched comparison with classic sleep apnea patients.

Chronic posttraumatic sleep disturbance may include sleep-disordered breathing (SDB), but this disorder of sleep respiration is usually not suspected in trauma survivors. Sleep breathing signs and symptoms were studied in 178 adults-all with SDB-including typical sleep clinic patients (N = 89) reporting classic snoring and sleepiness and crime victims (N = 89) with insomnia and posttraumatic stress. Significant differences (p < 0.0001) were common between groups. Sleep breathing complaints, loud snoring, marked obesity, and obstructive sleep apnea were prevalent in sleep clinic patients; crime

Respiratory and cognitive subtypes of panic. Preliminary validation of Ley's model.

A review of the panic disorder literature strongly suggests subtypes of panic attacks, including a respiratory subtype. This study empirically tested several aspects of Ley's panic subtype theory, measuring end-tidal carbon dioxide (ETCO2) levels at baseline, during psychologic and respiratory stressors, and at recovery. As predicted, Type 1 (classic or respiratory) panickers had significantly lower resting ETCO2 compared to Type 3 (cognitive) and to controls. Type 3 panickers did not differ from controls. Physiologic findings support the existence of respiratory and other subtypes of panic at
