Periodic limb movement disorder in children.

Periodic limb movement disorder (PLMD) has been reported in children and is more prevalent in certain conditions such as attention deficit hyperactivity disorder. The International Restless Legs Syndrome Group has recently developed the criteria for diagnosis of PLMD in children. These criteria include both specific clinical features and polysomnographic findings. Exclusion of other causes especially sleep-disordered breathing is essential. Several medications including benzodiazepine and dopaminergic medication have been used in children, although these medications have not been adequately st…

Dystonia gravidarum: a new entity?

We describe cervical dystonia occurring in a 31-year-old, previously well, primigravid, primiparous Chinese woman at 4 weeks' amenorrhea, which was ameliorated with low-dose clonazepam and disappeared completely by the end of the second trimester without recurring despite her being tapered off benzodiazepine therapy. Investigations were unremarkable for structural and biochemical causes of dystonia. Chorea, paraballismus, and restless legs syndrome are known to occur during pregnancy, attributable to high estrogen levels. Dystonia, on the other hand, has not been described to occur de novo in …

The efficacy and tolerability of rotigotine on patients with periodic limb movement in sleep: A systematic review and meta-analysis.

OBJECTIVE: There is still no consensus on the treatment for periodic limb movement in sleep (PLMS). This study aimed to determine the efficacy and tolerability of rotigotine in patients suffering from PLMS. METHODS: Publications listed in PubMed, ScienceDirect, The Cochrane Library, and ClinicalTrials.gov were reviewed to assess the efficacy of rotigotine on PLMS. Analyses of PLMS frequency before and after rotigotine treatments (pre- and post-intervention studies) and PLMS frequency between placebo and rotigotine treatments (placebo-controlled trial studies) were included in our study. A sys

What constitutes restless sleep in children?

Parents commonly report in sleep questionnaires and in person to clinicians that their child is restless in sleep. There is little agreement on what constitutes restless sleep and so the significance of this observation is uncertain and its relationship to parental concerns about daytime tiredness, learning difficulties or problematic behaviours is unclear. Restless sleep may be secondary to other sleep problems such as obstructive sleep apnoea or periodic limb movement disorder. However, there may be some evidence to support the concept of restless sleep as a primary sleep problem. Certainly,

Risk Analysis of Lurasidone in Patients with Schizophrenia and Bipolar Depression.

Lurasidone was approved by the United States Food and Drug Administration (FDA) for the treatment of schizophrenia, as well as for the treatment of bipolar depression. However, emerging reports have indicated various adverse drug reactions with the use of lurasidone. Thus, in this article, we have analyzed the risk profile of lurasidone in the established therapeutic indication. A total of 419 studies were published from October 2010-July 2019 regarding lurasidone. After the inclusion and exclusion criteria, 17 studies were selected for the analysis of risk. The adverse drug reactions (ADRs) o

Dopamine agonist withdrawal syndrome - what do we really know?

INTRODUCTION: Dopamine agonist withdrawal syndrome (DAWS) comprises a wide range of neuropsychiatric and autonomic symptoms that occur in temporal relation with the reduction or withdrawal of dopamine agonists (DAs) in a dose-dependent manner. This syndrome was described as an important complication in Parkinson's disease (PD) and restless legs syndrome (RLS) treatment. STATE OF THE ART: Dopamine agonist withdrawal syndrome affects even up to 24% of PD patients treated with DAs in whom the medication was tapered. Predominant symptoms include the following: anxiety, depression, irritability, p…

Restless Sleep Disorder in Children: An Educational Review on Diagnosis, Pathophysiology, and Management.

BACKGROUND: Restless sleep disorder (RSD) is a pediatric movement disorder characterized by frequent large muscle movements during sleep. This disorder leads to disrupted sleep architecture and significant daytime impairments. Research has compared RSD with other conditions, such as restless legs syndrome (RLS) and periodic limb movement disorder (PLMD), to emerge as a distinct diagnostic entity. OBJECTIVES: This review aims to educate clinicians about the prevalence, pathophysiology, diagnostic challenges, and treatment options for RSD. METHODS: A comprehensive literature synthesis focused …

Is "normal" ferritin truly enough? Strong correlation between iron stores and RLS severity in patients with normal ferritin levels.

OBJECTIVE: We aimed to investigate the relationship between peripheral iron stores and symptom severity in drug-naive patients with idiopathic Restless Legs Syndrome (RLS) whose serum ferritin levels were within the laboratory reference range. Additionally, we sought to determine the independent effects of age, sex, comorbidities, and biochemical parameters in predicting RLS severity. MATERIALS AND METHODS: This cross-sectional study was conducted at the neurology outpatient clinic of a single university hospital. A total of 113 drug-naive patients with idiopathic RLS were prospectively enrol…

Narcolepsy with cataplexy and Parkinson's disease. Case Report.

We present the case of an 86 year old female, in whom narcolepsy with cataplexy (NC) manifested at 52 years of age. She was treated by an amphetamine-like drug phenmetrazine and tricyclic antidepressants for more than 10 years. Hypokinetic-rigid syndrome manifested at 83 years of age and Parkinson´s disease (PD) was diagnosed. Detailed examination at the age of 86 confirmed the previous diagnosis of NC and the diagnosis of PD. Severe periodic limb movements in sleep, severe sleep apnea, REM sleep behavior disorder and restless legs syndrome, which are frequently comorbid in NC and PD, were rev…

A selective serotonin reuptake inhibitor (SSRI) - induced case of catathrenia.

Catathrenia is currently classified as a sleep-related breathing disorder characterized by a deep inspiration followed by a prolonged, groaning expiration. While antidepressant- and antipsychotic-induced akathisia and restless legs syndrome (RLS) are well-documented, the potential for these medications to trigger catathrenia remains unrecognized. Selective serotonin reuptake inhibitors (SSRIs) are known to exacerbate or induce various sleep disturbances, including periodic leg movement disorder (PLMD), RLS, bruxism, and REM sleep without atonia. However, an association between SSRIs and catath…

[Gastrointestinal disorders in hyperkinetic movement disorders and ataxia].

Disorders of the gastrointestinal tract in patients suffering from hypokinetic movement disorders, and in particular Parkinson's disease, have increasingly been the subject of more intensive neuromedical research. So far, few data are available for patients with hyperkinetic movement disorders and ataxias. This review article summarizes the currently available and relevant publications on this topic. The particular focus is on essential tremor, restless legs syndrome, Huntington's disease and the group of hereditary ataxias. Further intensive research will be necessary in the future to collect…

Perioperative Management of Insomnia, Restless Legs, Narcolepsy, and Parasomnias.

Obstructive sleep apnea (OSA) has been shown to increase risk of adverse perioperative events. More recently, investigators have begun to examine other common sleep disorders to assess how they may be impacted by the perioperative environment, as well as influence postoperative outcomes. There are a number of mechanisms by which such common sleep disorders (eg, insomnia, restless legs syndrome, narcolepsy, and parasomnias) may have consequences in the perioperative setting, both related to the underlying pathophysiology of the diseases as well as their treatments. This review will highlight th…
